Rivian Automotive, LLC (Rivian) is recalling certain 2022-2025 R1T and 2022-2026 R1S vehicles. The bolts that secure the second-row driver and passenger side seat belt retractor assemblies may be improperly tightened.
The risk
An improperly secured seat belt retractor may not adequately restrain the seat occupant, increasing the risk of injury during a crash.
The remedy

The defect
Rivian Automotive, LLC (Rivian) is recalling certain 2022-2025 R1S and R1T vehicles that previously had service performed. The toe link may have been reassembled incorrectly during service procedures performed before March 10, 2025.
The risk
The toe link joint may separate, increasing the risk of a crash.
The remedy

The risk

Rivian Automotive, LLC. (Rivian) is recalling certain 2022-2023 R1S and R1T vehicles. The maximum vehicle capacity weight value on the tire placard label is incorrect. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 110, "Tire Selection and Rims."
The risk
A vehicle with an incorrect maximum capacity weight value may be overloaded, which can increase the risk of a crash.

The defect
Rivian Automotive, LLC (Rivian) is recalling certain 2022-2024 R1T and R1S vehicles. The attachment clips on the upper B-pillar and/or C-pillar trim panels may have been damaged during a service procedure.
The risk
Damaged pillar trim panel clips can result in improper side curtain air bag deployment, increasing the risk of injury during a crash.
The remedy

The defect
Rivian Automotive, LLC (Rivian) is recalling certain 2022 R1T and R1S vehicles. The accelerator pedal may not detect when the driver releases the pedal, failing to activate "auto-hold" or "park" as intended.
The risk
Failure to activate "auto-hold" or "park" can result in unintended vehicle movement, increasing the risk of a crash.
The remedy

The defect
Rivian Automotive, LLC (Rivian) is recalling certain 2022 R1T and R1S vehicles. A faulty automatic locking retractor (ALR) sensor in the front passenger seat belt system may incorrectly report as ON when it is actually OFF, causing the suppression or improper deployment of the air bag. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 208, "Occupant Crash Protection."
The risk
Failure of the air bag to deploy as intended increases the risk of injury to the front passenger during a crash.
The remedy

The risk
A loose steering knuckle fastener could separate, causing a loss of vehicle control and increasing the risk of a crash.

The defect
Rivian Automotive, LLC (Rivian) is recalling certain 2022 R1T vehicles. The Occupant Classification System (OCS) may not deactivate the air bag when a child or child seat occupies the front passenger seat. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 208, "Occupant Crash Protection."
The risk
A child in the front seat may be at a greater risk of injury in the event of a passenger air bag deployment during a crash.
The remedy

The Office of Defects Investigation (ODI) has received two Vehicle Owner Questionnaires (VOQs) reporting left rear toe link separation in certain Model Year (MY) 2023-2024 Rivian R1S vehicles. Both VOQs report component separation while driving, causing the vehicles to swerve across multiple lanes of traffic. One incident resulted in a collision with an adjacent vehicle and roadside barrier.  In both separations, the bolt that maintains the integrity of the toe link fractured. Evidence collected from the complaint vehicles included repair histories, onboard video, imagery of the damaged components, and a police accident report. The two vehicles with reported separation have different vehicle histories. One vehicle received prior service and the other was involved in a previous collision. In both instances, the vehicles operated for multiple months and thousands of miles of usage with no apparent problems between the previous service or the previous collision and the failures that prompted this investigation. Rivian recognized the subject toe link joint’s sensitivity to service procedures in March of 2025 in its updated service procedures. In January 2026 through recall 26V-003, Rivian extended the improved repair procedure to vehicles that received toe link service prior to the March 2025 improvement. ODI is opening a Preliminary Evaluation (PE) to (1) assess the sensitivity of the rear toe link joint to foreseeable road and service conditions, (2) compare the physical failure evidence from the two VOQs to  identify apparent similarities and potential differences, (3) evaluate Rivian’s current toe link repair procedure, and (4) assess the in-field subject population’s toe links conditions. NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

While preparing to pull out of a parallel street parking space in my R1T with a car 3 feet in front of me, I stepped on the brake to engage Drive from Park. On releasing the brake, before stepping on the accelerator, the R1T surged at a high rate of speed hitting the car straight on, deploying the air bags in the car and blowing the rear window out of the car. The car was pushed past an empty space, 20 ft or more, into another parked car. This all happened in 2-4 seconds. The R1T surged as if the accelerator had been push to the floor surprising me and pushing me back on the driver seat. The R1T stopped when I braked. The car was totaled. I was driving late night to a campsite in a very rural part of the Big Island of [XXX] , about 45 minutes from where I live. I was by myself except for my dog, in the passenger seat. I was on a country road with a 30 mph speed limit, going about 25 mph. I engaged the cruise control and it immediately went up to 40-45 mph, far faster than the limit and far faster than was safe given the wet conditions and windy road. As I tried to slow the car down, using the steering while cruise control adjustment buttons, I steered through a curve and rather than turn with the road, as I had intended, the truck slid off the road to the left, effectively hydroplaning, into two trees, at about 40 mph. We were driving on the highway going 70 MPH when we heard a loud bang, and then the car immediately catapulted to the right off the road and slammed into the concrete right median, spun around hit a sign and then ended up in the right bridge barrier (shoulder). During the crash 6 airbags deployed from the impact of the crash. The highway patrolman stated that you could see the driver-side rear wheel started skidding based on the marks on the road. We thought it was a blown tire. But the tires were all in tact. The driver-side rear wheel was turned out and it was clear that the toe-link joint had separated from the wheel axel and this is what caused our crash. The car has been totaled by

Insert, Padding
After gently removing the fabric cover of our Cybex Eternis S All-In-One car seat to wash it according to the manufacturer's care instructions, and then putting the cover back on, the car ride immediately following, the red plastic pieces designed to hold the cover in place completely snapped off. This structural failure exposed two sharp, unshielded metal screws directly in the child's seating area. Because these screws were left fully exposed by the broken plastic, they created an immediate laceration hazard. As a direct result of this defect, my young daughter's leg was cut by the exposed screws while using the seat.

ELECTRICAL SYSTEM
SYSTEMS AFFECTED: Electrical System (12V Power Supply / Low-Voltage), Climate Control (HVAC System / Thermal Management), Towing / Recovery DESCRIPTION OF SAFETY HAZARD & DEFECT: My 2022 Rivian R1T experienced a sudden, complete electrical failure due to a defective HVAC system, resulting in a dangerous hard-bricked state that rendered the vehicle totally inoperable. Approximately 30 days prior to the incident, the vehicle underwent a comprehensive inspection at a Rivian Service Center where both the HVAC system and 12-volt battery system were tested and cleared. SUSPENSION
In July of 2025 while driving my entire air suspension collapsed on the highway slamming the vehicle down onto it's bumpstops. Rivian covered a "fix" of this under warrant. In December the rear shock was leaking. Now in July of this year while driving the front air suspension collapsed to it's bump stop again.

BACK OVER PREVENTION: REARVIEW SYSTEM BRAKING
The R1T has four cameras to assist the driver and provide warnings for lane changes, etc. The cameras are routinely failing while I am driving the vehicle. This started after an OTA software update. A software reset will turn the cameras back on, though they have repeatedly failed. Rivian attempted to correct the issue by replacing the front camera, which they determined was faulty. The cameras have continued to fail since then.

ELECTRICAL SYSTEM
Vehicle gave a warning to service the electrical system and then shut down preventing any operation. Luckily I was in a parking lot and not on a roadway impeding traffic or jeopardizing safety while driving. Could smell burnt electrical from within cabin. Had to have towed to service center and explained that condensation from air conditioner leaked onto high voltage heater coils and shorted out electrical system. Seems like a poor design that this was even possible. Dealer indicated this was not uncommon.
