19V500000 · June 27, 2019

The defect
Honda (American Honda Motor Co.) is recalling certain 2013 Acura ILX, 2015 RDX, 2005-2010 and 2012 RL, 2009-2014 TL, 2010 and 2012 ZDX, 2007-2011 CR-V, 2011-2013 and 2015 CR-Z, 2009-2013 Fit, 2013 Fit EV, 2010-2011 and 2013 Insight and 2007-2014 Ridgeline vehicles. These vehicles are equipped with driver frontal air bag inflators assembled as a recall remedy part or replacement service part, that may explode due to propellant degradation occurring after long-term exposure to high absolute humidity, temperature and temperature cycling.
The risk
An inflator explosion may result in sharp metal fragments striking the driver or other occupants resulting in serious injury or death.
The remedy
Honda will notify owners, and dealers will replace the driver frontal air bag inflator, free of charge. The recall began August 12, 2019. Owners may contact Honda customer service at 1-888-234-2138. Honda's numbers for this recall are S5B and W59.

Defect investigations (1)

An investigation means NHTSA has decided a pattern is worth examining. A preliminary evaluation is often closed with no action; an engineering analysis is the stage that usually precedes a recall.

Closed defect petition · opened January 27, 2023 · closed August 7, 2023 · led to recall 22V063000

the electrical system

NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

NOTE: THIS IS THE SECOND FRONTAL BASED CRASH LISTED ON THIS SITE FOR A HONDA 2011 CRZ PERHAPS THE DRIVER THAT WAS INVOLVED WITH THE PREVIOUS CRASH DID NOT REALIZE THE BRAKES FAILED. AT APPROXIMATELY 17:17 I NOTICED THE VEHICLE IN FRONT OF ME WAS STOPPED IN TRAFFIC I TRIED TO APPLY MY BRAKES AND PUSHED THE BRAKES AND THE CAR DID NOT SLOW FOR AT LEAST 1 1/2 CAR LENGTH. IN A PANIC I TRIED TO PRESS HARDER WITH BUT NOTHING HAPPENED THE CAR NEVER SLOWED. THE CRASHED INTO THE VEHICLE IN FRONT OF ME WITH THE FULL FORCE OF FORTY-FORTY FIVE MPH AND TOTALLY COLLAPSED THE ENGINE INTO THE DRIVERS CAB BOTH OF MY KNEES WERE BLEEDING AND SWOLLEN FROM THE IMPACT OF THE ENGINE BEING IN THE DRIVERS

****REVISION TO PREVIOUS COMPLAINT**** I WAS RECENTLY INVOLVED IN A FRONTAL COLLISION IN THIS VEHICLE. I AM FINE, HOWEVER, THE SEAT BELT DID NOT HOLD ME TO THE SEAT PROPERLY. AS THE IMPACT HAPPENED THE SEAT BELT LOCKED AND HELD ME MIDWAY, FROM THE BACK OF THE SEAT TO THE STEERING WHEEL, AND DID NOT 'PRETENSION' ME PROPERLY (PULL ME BACK AND HOLD ME SUNG TO THE SEAT. BECAUSE OF THIS, WHILE THE SEAT BELT WAS HOLDING ME MIDWAY, THE AIR BAG CAME OUT AT FULL FORCE AND THE CENTER OF THE AIRBAG IMPACTED ME IN THE MIDDLE OF MY CHEST, MORE SPECIFICALLY MY STERNUM. IF I WAS GOING TO COME INTO CONTACT WITH THE AIRBAG IT SHOULD HAVE BEEN AFTER THE AIRBAG WAS FULLY DEPLOYED, INSTEAD, THE AIRBAG DEPLOYED

I HAD JUST BOUGHT THIS CAR IN SEPTEMBER, AND ON THE MORNING OF WEDNESDAY NOVEMBER 26, 2014 IT CRASHED. I WAS DRIVING ON THE HIGHWAY, I495-SOUTH. IT WAS RAINING AND I THINK THE CAR HIT A WATER PUDDLE OR SLEET, AND STARTED FISH TAILING. I TRIED TO GET IT BACK UNDER CONTROL, AND I DID FOR A FEW SECONDS AS IT WAS TRYING TO PULL TO THE RIGHT. THEN IT JUST SUDDENLY TOOK OFF TO THE LEFT, VERY STRANGE, ALMOST LIKE THERE WAS EITHER A WHEEL/TIRE ISSUE, TRANSMISSION, ELECTRICAL OR STEERING WHEEL.. WHATEVER IT WAS IT SEEMED LIKE IT TOOK OVER AND JUST TOOK OFF LIKE A CRAZY HORSE TO THE LEFT. I WAS LUCKY THERE WERE NO CARS TO THE LEFT! THE CAR JUST TOOK OFF TO THE LEFT AS I HELD THE WHEEL AND COULDN'T

STEERING
On February 15 I took a short drive into Woodstock, Vt. My trip into Woodstock was without incident, except for the fact that I got a "check tire pressure" alert, probably due to the very cold weather. When I finished my business and restarted the car, there was an additional alert: "check power steering". The car previously had no issues with steering. I drove the car to another nearby parking spot. There was no power assist at all. I wrestled the car into the parking space and called AAA for a tow. The tow truck driver arrived in about an hour. He asked for the keys and started the car. The problem had disappeared. With a friend tailing me I drove to a nearby auto repair shop, asked them

POWER TRAIN
As I was pulling away from a stop sign, the right-hand CV shaft snapped in the middle. Fortunately, I had just enough speed to coast into a parking lot, so I was not a traffic obstacle. On closer inspection, the failure occurred underneath the rubber/metal dynamic damper clamped to the center of the CV shaft. It looks like the dynamic damper was not adequately sealed and trapped salt and water until enough corrosion happened, causing the failure of the CV shaft.

Where these numbers come from

A complaint is one owner's account and has not been verified by NHTSA or by us. A vehicle with more complaints is not necessarily worse: models that sold in larger numbers collect more of them. How this is counted.
