The defect
Daimler Trucks North America, LLC (DTNA) is recalling certain 2017-2023 Freightliner Cascadia, 2020-2023 Western Star 49X, 2021-2023 Western Star 47X, and 2022 Freightliner 114SD vehicles. The brake modulator valves on the front axle may corrode and cause the vehicle to pull to the affected side during braking initiated by Electronic Stability Control (ESC) or Roll Stability Control (RSC).
The risk
Brakes that pull to one side may cause a sudden change in vehicle direction, increasing the risk of a crash.
The remedy
Dealers will inspect and replace the front brake modulator valves, as necessary. Some vehicles will have a software update which will alert driver when a replacement is required. The defect
Daimler Trucks North America, LLC (DTNA) is recalling certain 2017-2023 Freightliner Cascadia, 2020-2022 Freightliner Business Class M2, and 2022 Freightliner 114SD, and 2020-2023 Western Star 49X, 2021-2023 Western Star 47X, and 2021-2022 Western Star 57X vehicles. The brake modulator valves on the front axle may corrode and cause the vehicle to pull to the affected side during an Adaptive Cruise Control (ACC) braking event.
The risk
Brakes that pull to one side during an ACC braking event may cause a sudden change in vehicle direction, increasing the risk of a crash.
The remedy
Dealers will inspect and replace the front brake modulator valves, as necessary. Some vehicles will have a software update which will alert driver when a replacement is required. All repairs will be performed free of charge. Owner notification letters were mailed beginning October 16, 2023. DTNA's number for this recall is FL-958. This recall expands and replaces previous recall 20V-390. All vehicles previously repaired under recall 20V-390 will need to have the new remedy performed.

Daimler Trucks North America LLC (DTNA) is recalling certain 2017-2021 Cascadia P4 vehicles. The brake lights may remain illuminated after the brake pedal has been released.
The risk
If the brake lights remain illuminated, drivers may be unaware when the truck is actually slowing or stopping, increasing the risk of a crash.
The remedy
DTNA will notify owners, and dealers will replace the 3-pin brake pressure switch with a 2-pin brake pressure switch along with a jumper harness, free of charge. The defect
Daimler Trucks North America, LLC (DTNA) is recalling certain 2017-2022 Freightliner Cascadia vehicles equipped with halogen headlight assemblies. The halogen headlight bulb may dislodge during replacement and contact other vehicle components, possibly causing them to melt.
The risk
Melting components can increase the risk of fire.
The remedy

The defect
Daimler Trucks North America LLC (DTNA) is recalling certain 2017-2021 Freightliner Cascadia tractor trailers. The single brake modulator valve may be affected by chemical corrosion which can slow down the release timing and cause the brakes to pull to the affected side during automatic braking events.
The risk
Brakes that pull to one side during automatic braking events may cause a sudden change in vehicle direction, increasing the risk of a crash.
The remedy

Daimler Trucks North America LLC (DTNA) is recalling certain 2006-2020 Freightliner Columbia, 2006-2021 Freightliner Business Class M2, 2007-2020 Freightliner Coronado, 2014-2021 Freightliner 122 SD, 2007 Sterling A 9500, 2006 Freightliner Argosy, 2006-2007 Sterling AT9500, 2008-2021 Freightliner Cascadia, 2006-2008 Freightliner Classic, 2006-2011 Freightliner Century Class, 2006-2010 Freightliner FLD, and 2017 Western Star 5700 vehicles. The rear reflective tape may be partially covered by the mudflap hanger bracket, which may reduce the vehicle's visibility to other drivers on the road. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 108, "Lamps, Reflective Devices, and Associated Equipment."
The risk
The vehicle's rearward visibility may be reduced, increasing the risk of a crash.
The remedy

The defect
Daimler Trucks North America LLC (DTNA) is recalling certain 2004-2019 Freightliner 108SD, 114SD, 122SD, Cascadia, Columbia, Classic, FL80, FLD 120SD, Business Class M2, Western Star 4700, 4900, 6900, Sterling Acterra, L7500, LT7500, L8500, LT8500, L9500, and LT9500 vehicles. The vehicles may have an insufficient service brake air reservoir capacity. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 121, "Air Brake Systems."
The risk
An insufficient air reservoir volume may reduce brake effectiveness, increasing the risk of crash.
The remedy

The defect
Daimler Trucks North America LLC (DTNA) is recalling certain 2017-2019 Freightliner 114SD, 122SD, 108SD, Cascadia and Business class M2 trucks, Freightliner Custom Chassis XBS, XCL, XCM, MT45, MT55, S2G, S2RV, XCR and XCS custom chassis and Western Star 4700, 4900 and 5700 trucks equipped with Detroit Axle axles. The affected vehicles may have loose steering and tie rod arm bolts.
The risk
Loose tie rod arm bolts may lead to separation of the tie rod arm, resulting in a loss of vehicle control, increasing the risk of a crash.
The remedy
DTNA has notified owners, and dealers will inspect the steering arm and tie rod arm bolts for to make sure that they are properly tightened, installing new fasteners as necessary, free of charge. The defect
Daimler Trucks North America LLC (DTNA) is recalling certain 2017-2019 Freightliner Cascadia, Coronado and 122SD, and Western Star 4900 trucks, equipped with Cummins ISX15 or X15 diesel engines. In certain driving conditions, such as on a long down-hill grade, the fuel line may burst if the fuel pump cooling circuit screen becomes restricted.
The risk
If the fuel line bursts, fuel may leak onto the road resulting in a roadway hazard for other motorists. The engine may also stall without warning, resulting in the vehicle's inability to restart, increasing the risk of a crash.
The remedy

The defect
Daimler Trucks North America, LLC (DTNA) is recalling certain 2017-2019 Freightliner Cascadia trucks. The Instrument Cluster Unit (ICU) may not perform the required bulb checks for the Anti-lock Brake System (ABS) or Electronic Stability Control (ESC) if the ignition is turned from on to off to on too quickly. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 121, "Air Brake Systems," and 136, "Electronic Stability Control Systems on Heavy Vehicles."
The risk
Without an ABS or ESC bulb check, the operator may unaware that the warning light bulbs are not working. If the warning bulbs are not working, in the event of an ABS or ESC malfunction, the driver may not be notified, increasing the risk of a crash.
The remedy
DTNA will notify owners, and dealers will reprogram the ICU's to ensure a bulb check is performed each time the ignition is turned on, free of charge. Also, the sub-population with Tire Pressure Monitoring (TMPS) will also require replacement parts, free of charge. DTNA issued owners an interim notification on September 18, 2018. The defect
Daimler Trucks North America LLC (DTNA) is recalling certain 2017-2019 Freightliner M2 Business Class, 108SD, 114SD and Cascadia trucks, Western Star 4700, 4900, and 5700 trucks, FCCC S2C 106 Cab, S2G Cab, XBR, XBS, XCL, XCM, XCP, XCR, and XCS chassis equipped with air disc brakes. The brake caliper mounting bolts on these vehicles may be insufficiently tightened, allowing the bolts to loosen.
The risk
If the brake caliper mounting bolts loosen, braking performance may be reduced, increasing the risk of a crash.
The remedy
DTNA will notify owners, and dealers will inspect the brake caliper mounting bolts and tighten them as necessary, free of charge. DTNA issued owners an interim notification on July 25, 2018. The defect
Daimler Trucks North America LLC (DTNA) is recalling certain 2002-2017 Freightliner Cascadia, Century Class, Columbia and Coronado trucks and Western Star 4700, 4900, 5700, 5900 and 6900 trucks. These vehicles are equipped with Kidde Plastic-Handle or Push Button 'Pindicator' Fire Extinguishers that may become clogged, preventing the extinguisher from discharging as expected or excessive force may be needed to activate the extinguisher. Additionally, in certain models, the nozzle may detach from the valve assembly with enough force that it could cause injury and also render the product inoperable. The risk
In the event of a fire, if the fire extinguisher does not function properly, it can increase the risk of injury.

The risk

The risk
A loss of steering control increases the risk of a crash.

The defect
Daimler Trucks North America LLC (DTNA) is recalling certain 2017 Freightliner Cascadia and Western Star 5700 trucks built with NFD tandem rear axles. During the hardening process, the axle assembly output shaft may have been made brittle, possibly resulting in the output shaft fracturing while the vehicle is in use.
The risk
If output shaft fractures while descending a grade, the compression braking will be ineffective. Additionally, large debris may land in the road. Both scenarios increase the risk of a crash.
The remedy

The defect
Daimler Trucks North America LLC (DTNA) is recalling certain model year 2017 Freightliner Cascadia, Business Class M2, and 122SD trucks manufactured January 24, 2016, to October 10, 2016 and equipped with Goodyear G399A LHS 295/75R22.5 Load Range G tires. The tires on the affected vehicles may have cured improperly, possibly resulting in a partial tread separation.
The risk
Partial tread separation can increase the risk of a crash.
The remedy
Goodyear will notify the owners of the affected Freightliner vehicles and Goodyear dealers will replace the affected tires, free of charge. The recall began on December 14, 2016. The defect
Daimler Trucks North America, LLC. (DTNA) is recalling certain model year 2013-2017 Freightliner Cascadia vehicles manufactured September 24, 2012, to May 27, 2015 and equipped with compressed natural gas (CNG) engines. The affected vehicles have a Power Distribution Module (PDM) that is mounted near the CNG fuel tank. Due to its location, the PDM may be sprayed with water and contaminants from the road, possibly causing the PDM to short circuit.
The risk
If the PDM short circuits, there would be an increased risk of a fire, especially if the nearby CNG tank leaks.
The remedy

The defect
Daimler Trucks North America LLC (DTNA) is recalling certain model year 2014-2017 Freightliner M2 Business Class, 108 SD, 114 SD, Cascadia, Coronado, Western Star 4700, 4800, and 5700 XE trucks manufactured March 15, 2013, to July 29, 2016 and equipped with Conmet aluminum non-high capacity hubs. These vehicles specify a front axle weight limit that may be greater than that of the hub capacity. As a result, the hub may fail and the wheel may separate from the axle.
The risk
If a wheel separates from the axle, there would be an increased risk of a crash.
The remedy

The defect
Daimler Trucks North America LLC (DTNA) is recalling certain model year 2017 Freightliner Cascadia, 122SD, Western Star 4900SA and 4900FA, and FCCC XCM and XCP trucks manufactured from March 14, 2016, through May 18, 2016, equipped with a Cummins ISX12 or ISX15 engine using certain engine control modules (ECM). These ECM's may experience an internal electrical short that can cause a fuse to blow, resulting in an unexpected engine stall without the ability to restart the engine.
The risk
An engine stall without the ability to restart the engine can increase the risk of a crash.
The remedy
Cummins will notify owners, and Cummins dealers will install a new ECM, and correct ECM calibrations, free of charge. The recall began on July 8, 2016. On May 26, 2023, the Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE23010) to assess reports of false positive activation of the Automatic Emergency Braking (AEB) system in model year (MY) 2017-2022 Daimler Trucks North America (DTNA) vehicles. The reports alleged that activation of the AEB system occurred while driving with no apparent obstruction in the vehicle's forward path, resulting in sudden vehicle deceleration. DTNA indicated that it received of a total of 315 reports that may relate to the alleged defect in MY 2017-2022 DTNA vehicles. DTNA stated that the vast majority of false positive activation of the subject systems occur in very limited durations and result in a five mile per hour deceleration. ODI considered four AEB systems offered by DTNA: Detroit Assurance 2.0, Detroit Assurance 4.0, Detroit Assurance 5.0, and Wabco OnGuard.  ODI’s analysis shows that of the Detroit Assurance versions, all are failing at similar rates. The OnGuard AEB systems equipped on the same model and model year vehicles have experienced a significantly lower claim rate. To date, ODI has received a total of 32 consumer complaints of inadvertent activation of AEB in MY 2017-2022 DTNA vehicles. In total, ODI reviewed 347 unique (i.e., non-duplicative Vehicle Identification Numbers) reports that may relate to the alleged defect, which include 24 reports involving a crash, 13 reports involving an alleged injury and one report with a fatality. PE23010 has been upgraded to an Engineering Analysis to further assess the scope, frequency, and potential safety related consequences of the inadvertent AEB activations in MY 2017-2022 DTNA vehicles. The Office of Defects Investigation (ODI) opened this Preliminary Evaluation to investigate reports of engine electrical wiring harness chafe on the subject vehicles.ODI analysis found thousands of unique harness part numbers, spanning the two generations of vehicle chassis assembled at multiple vehicle assembly plants in two different countries, with multiple sub-models, incorporating over thirty powertrain choices.Each wiring harness could contain several dozen individual wires for power, ground and communication purposes depending on the multitude of equipment options available for custom vehicle configuration. ODI identified three primary areas where a wiring harness bundle may chafe: the air compressor support bracket, the transmission case, or the coolant surge tank. In each case, the root cause was some combination of inadequate design clearance, installation variability and relative motion between the wiring harness and the point of contact, such as from vibration. The chafing could result in a range of consequences from none to various diagnostic fault codes, telltale illumination, driver warning center messages, erratic electrical component operation, difficult or no engine start condition, engine de-rate (power reduction), engine shut down, inoperative transmission gear selection, or an unexpected shift to neutral.These potential consequences vary depending on the specific damaged wire in the harness and the exact failure mode, such as intermittent short circuit to ground or open circuit. Generally, the chafing resulted in a progressive series of driver warnings with high levels of detectability over the course of several days and ignition cycles. Drivers usually heed warnings before an engine shutdown condition occurs and the driver may override an engine shutdown countdown timer using the override switch. If the driver allows the shutdown event to occur, the engine can almost always be immediately restarted. Incident rates for an engine shutdown with no warning or a transmission shift to neutral while driving are both very low. ODI received one complaint (NHTSA ID Number 11366557) from a fleet mechanic, who reported in a subsequent interview that he repaired at least six subject vehicles that experienced the alleged defect condition. However, there have been no new Vehicle Owner Questionnaire complaints received by ODI since opening this investigation, and there are no known crashes, fires or thermal events, or injuries or deaths related to the alleged defect. Accordingly, no safety-related defect trend has been identified. Based on available information, this investigation is closed.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. On August 31, 2020, the Office of Defects Investigation (ODI) opened a recall query, RQ20001, to investigate the allegations of water intrusion into the truck cab signal-detect and activation module (SAM cab).The investigation further evaluated the effectiveness of the original recall remedy, and the installation of an updated drip shield as it relates to continual water intrusion resulting in the loss of electrical functions and potential fire.The potential hazard was the increased risk of a crash and injury in certain model year (MY) 2007-2021 Freightliner Cascadia P3 vehicles manufactured by Daimler Trucks North America (DTNA). ODI sent DTNA an information request (IR) letter on September 29, 2020.DTNA's November 20, 2020, IR response detailed how the company had implemented various improvements through recall 09V024 and multiple technical service bulletins (TSBs) to mitigate water intrusion.The effectiveness of new windshield seals and revisions to the drip shield is evident by the downward trend in claims rate from an average high of 7.64% in the beginning product life of Cascadia P3 (MYs 2007-2009), to 1.79% average for MYs 2011-2018, and down to 0.49% towards the end of the Cascadia P3 production.Moreover, the data shows that most failures occurred within the first three years of service with a significant downward trend thereafter. In view of the current overall low failure rate, the nature of early-life failure, and failure rate trending downward since model year 2017, this Recall Query (RQ) is closed.  The Agency reserves the right to take additional action if warranted by future circumstances. Intermittent Loss of Power Steering Assist

On May 1, 2020, the Office of Defects Investigation (ODI) opened PE20006 to investigate complaints of intermittent loss of power steering assist in Daimler Truck North America (DTNA), Volvo Trucks of North America (Volvo) and PACCAR vehicles equipped with a ZF/TRW (TRW) THP60 steering gear.  The investigation focused on 2010-2020 DTNA Freightliner Cascadia, M2 and WST; Volvo VN, VAH and VHD and PACCAR 337, 365, 389, 567, 579 model vehicles.Field reports and interviews with fleet drivers allege that high power steering fluid temperatures could result in internal seal and bearing failures causing pressure fluctuation, affecting left and right-hand turns.  The inability of the driver to steer the vehicle as intended can increase the risk of a crash and injury. ODI analyzed data provided by TRW, DTNA, Volvo, and PACCAR in response to individual information request letters.ODI found that most failures occurred within the first two years and/or 50,000 miles of service.  The claim rate decreased with time and miles in service and remained less than 0.5%. ODI normalized data from each of the manufacturers and determined that there was not a substantial difference in the failures.Additionally, TRW improved a seal in the steering gear in December 2015.Trucks built on or after 2016 (as 2017 and newer model years) show a decreasing trend of failures. In view of the overall low failure rate, the nature of early-life failure, and failure rate trending downward for model years 2017 and newer, this Preliminary Evaluation (PE) is closed.  The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist.The Agency reserves the right to take additional action if warranted by future circumstances. On April 23, 2018, the National Highway Traffic Safety Administration (“NHTSA”) opened Audit Query (AQ18002) to assess the timeliness and scope of four recalls filed by Daimler Trucks North America, LLC (“DTNA”). NHTSA subsequently expanded AQ18002 to examine the timeliness and scope of three additional DTNA recalls and compliance with early warning reporting and manufacturer communications reporting requirements. NHTSA and DTNA entered into a Consent Order on December 29, 2020, to resolve NHTSA’s assertions that certain recalls within the scope of AQ18002 were untimely, DTNA failed to timely submit certain manufacturer communications to NHTSA, and DTNA failed to submit field reports to NHTSA as required. As part of the Consent Order, DTNA agreed to a total civil penalty of thirty million dollars ($30,000,000), of which ten million dollars ($10,000,000) was paid on January 12, 2021. Also, of that total civil penalty, DTNA agreed to spend five million dollars ($5,000,000) on the specific performance obligations to develop advanced data analytics capabilities to enhance its ability to detect and study emerging safety-related defect trends on its vehicles (“Safety Data Analytics infrastructure”) and an advanced data analytics and reporting platform that integrates data on emerging safety-related defect trends on its vehicles and recall lessons learned (“Safety Communications Platform”). The Consent Order also held fifteen million dollars ($15,000,000) of the civil penalty in abeyance pending DTNA’s satisfactory completion of the requirements of the Consent Order, and its compliance with the Safety Act, and regulations thereunder. NHTSA and DTNA agreed to a two-year term for the Consent Order—with an option for NHTSA to extend the Consent Order for an additional year, if warranted. In the Consent Order, DTNA agreed to numerous performance obligations, including monthly meetings with NHTSA to discuss potential safety issues, recent manufacturer communications, and recall decision-making, among other things; quarterly meetings with NHTSA to discuss DTNA’s progress on the performance obligations under the Consent Order, including the Safety Data Analytics infrastructure and Safety Communications Platform, and any issues related to compliance with the Consent Order, Safety Act, or regulations; development of written procedures and employee training materials to help DTNA comply with its legal obligations to timely identify and report safety defects and noncompliances, submit complete and accurate recall filings, and timely submit manufacturer communications and field reports to NHTSA; and identifying and reporting previously unreported manufacturer communications and field reports. Also, via the Consent Order, DTNA acknowledged that the failure or inability to identify a specific root cause, develop an adequate remedy or repair, or confirm the affected population of vehicles are not bases for delaying filing a recall with NHTSA. On December 22, 2022, NHTSA extended the Consent Order for a term of one year due to concerns about DTNA’s compliance with its recall-related obligations. Specifically, NHTSA expressed concerns about the completeness of DTNA’s recall reports and issues identified with owner notification letters during the term of the Consent Order, as well as the length of time that DTNA took to consider a straightforward noncompliance issue before filing a recall and DTNA’s interactions with NHTSA during the term of the Consent Order. NHTSA addressed these concerns with DTNA during the third year of the Consent Order. The extended term of the Consent Order expired on December 28, 2023. Based on the Consent Order, this investigation, AQ18002, is closed. Closure of this AQ has no bearing on the terms of the Consent Order or DTNA’s ongoing legal obligations, and it does not represent a determination by NHTSA as to DTNA’s performance under the Consent Order.

The Office of Defects Investigation (ODI) opened this investigation based on one (1) Vehicle Owner Questionnaire (VOQ) and associated Police Accident Report (PAR) that described a crash where the vehicle had a separated steering linkage at one wheel-end as the result of lower steering arm capscrew failures.It was unknown at the time whether the separation caused the crash or was the result of the crash.Recall 15V-613 corrected an under-torque issue in upper steering arm capscrews on these vehicles.ODI opened this Recall Query to assess the scope and remedy of the 15V-613 recall.ODI collected the front axle from the crash vehicle for inspection and sent an Information Request Letter (IR) to Daimler Trucks North America (DTNA).The Vehicle Research and Test Center (VRTC) analyzed the crash truck's front axle.Based on the fracture surfaces of the capscrews and numerous witness marks on the tires, wheels, and vehicle, VRTC determined that the steering linkage separated due to the crash and was not the cause of the crash.However, VRTC noted that the capscrews on the opposite wheel end were at a much lower torque value than specified by the manufacturer.In DTNA's IR response, they reported one additional crash where a separated steering linkage was observed.DTNA determined the separation was the result of the crash and not the cause of the crash.DTNA had no complaints and all warranty reports were related to the upper steering knuckles and recall 15V-613.During the investigation ODI received one crash report from a transportation investigator retained by a fleet which involved a similar vehicle and alleged defect.The steering linkage was observed to have separated at the lower knuckle arm capscrews on the driver's side.The passenger side capscrews were found to be at a lower torque value than specified.In order to prevent a future assembly defect as identified in recall 15V-613, Detroit Axle, who manufactured the axles in the subject vehicles, made several production process changes to ensure capscrews were properly torqued.On October 14, 2015, the assembly tool was modified to monitor torque and turn of each capscrew.On this date, the tool was also changed to ?Lock on Reject.? If a bolt was not properly torqued the tool would not release until the operator acknowledged and corrected the defect.ODI conduced a roadside inspection study to help explain the low torque observed in the crash vehicle.With the help of the Virginia State Police Motor Carrier Enforcement Division ODI inspected thirty-nine (39) trucks and 156 capscrews over a two day period.No trucks were found to have under-torqued capscrews.ODI included some older trucks outside the scope of this investigation to offer additional time in service.One such truck was twelve (12) years old and had nearly 1 million miles.It too, was within specification.Based on the information collected, this investigation is closed. THE WHEELS SEPARATED FROM THE AXLE CAUSING THE VEHICLE TO VEER TO THE RIGHT AND THE FRONT PASSENGER SIDE WHEEL FRACTURED, BUT DID NOT DETACH FROM THE VEHICLE. THE CONTACT MENTIONED THAT THE STEERING ROD SNAPPED AND BECAME LOOSE FROM THE HUB.
