The defect
Honda (American Honda Motor Co.) is recalling certain 2021-2025 Acura TLX, 2023-2025 Acura MDX, and 2023-2025 Honda Pilot vehicles. The brake pedal pivot pin was not secured properly, which can cause the pedal to shift out of position. As such, these vehicles fail to comply with Federal Motor Vehicle Safety Standard numbers 108, "Lamps, Reflective Devices, and Associated Equipment" and 135, "Light Vehicle Brake Systems."
The risk
A brake pedal that shifts out of position can prevent the driver from applying the brakes as intended, increasing the risk of a crash or injury.
The remedy
Dealers will inspect and replace the brake pedal assembly as necessary, free of charge. Owner notification letters were mailed July 29, 2025. Owners may contact Honda's customer service at 1-888-234-2138. Honda's numbers for this recall are FLX and XLY.

Defect investigations (1)

An investigation means NHTSA has decided a pattern is worth examining. A preliminary evaluation is often closed with no action; an engineering analysis is the stage that usually precedes a recall.

the engine and its cooling

On June 3, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE22005 after the agency received VOQs and several field reports concerning the Auto Idle Stop (AIS) feature on 2016-2019MY Honda Pilot vehicles. The complaints allege that the engine fails to restart on its own from a complete stop at a traffic light or road intersection with the AIS function engaged. The subject vehicles are equipped with a 3.5L engine with a 9-speed automatic transmission. On January 6, 2023, Honda released service bulletins 23-008 and 23-009. These service bulletins address the failure to restart issue in the 2016-2019MY Honda Pilot vehicles, as well as the 2015-2020 Acura TLX, 2016-2020 Acura MDX, 2019-2022 Honda Passport, and 2020-2023 Honda Ridgeline vehicles. The repair outlined in the service bulletins is a two-stage countermeasure to remedy the issue. The first stage is a Programmed Fuel Injection (“PGM-FI”) software update. The second stage is only deployed if the software update fails to remedy the issue. The second stage, depending on the model and model year, consists of replacement of the starter assembly, starter relays, and a valve adjustment. Additionally, Honda extended the warranty coverage to 10 years with unlimited mileage for vehicles requiring the second stage component replacement. ODI continues to receive complaints of the AIS failure to restart, with many complainants alleging the campaign countermeasures had been completed.NHTSA has reviewed Honda's Information Request Letter responses involving warranty claims, lawsuits, and consumer complaints in addition to ODI's own data. This Engineering Analysis is being opened to further consider the potential safety defect, including gathering additional data regarding Honda’s service campaign efficacy. ODI is additionally expanding the scope to encompass all vehicles addressed in Honda’s service bulletins 23-008 and 23-009 as well as newer model year vehicles, which are substantially similar to the vehicles covered by the service campaign. To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.

On more than several occasions the car has slammed on the breaks for no apparent reason. This has happened multiple times and thr dealer can’t fix the issue because the computer is showing no error. This is a serious concern for me as the last time it occurred a child hit her face on the back of the passenger seat causing her lip to bleed.

UNKNOWN OR OTHER
I am reporting a potential safety defect involving the rear windshield glass on my Acura MDX. The rear windshield unexpectedly shattered while the vehicle was stationary, with no known impact or contact with an outside object. I was present when the glass shattered and immediately took multiple photographs and videos before the vehicle was moved. Based on what I observed, the glass appeared to break outward rather than from an outside object striking the vehicle. I later drove the vehicle from my home to the Acura dealership after being advised to do so. By the time the vehicle was inspected, portions of the shattered glass were understandably missing. The dealership later identified an

VISIBILITY/WIPER
Went to get into my 2024 Acura MDX parked car this morning temperature around 68 degrees and noticed the back window of my 2024 MDX had shattered out. There was a large section of glass missing and the glass looks as if it was pushed from the inside out. The vehicle had not been driven or started since the last evening. It seems as though this was a problem with prior Acura models in previous years. Went to the dealer and talked to the technician who said it is not covered under Acura warranty. This is definitely a safety issue as I could have been driving down the interstate when this happened putting my life and others in danger. I can only assume this is an issue Acura is aware of with

ELECTRICAL SYSTEM
Our infotainment system stopped working and when we took it one of Acura’s authorized dealers they told us that back half of the car is flooded and that has shot down all the electronics including the infotainment system. They also gave me a bill of $10K USD to fix the leak and replaced damaged electronics. Their argument is that the car was parked outside and that caused this issue. This has never come up in any of our previous maintenance visits where Acura certified dealerships always check our car and tell us that everything is okay.

After 3 visits to the dealership they identified the problem and fixed it Also, in a separate instance, car stalled after trying to start up after stop signs or while waiting in traffic several times. Could have been rear ended or worse. Dealer replaced the starter at 30k miles. Still had the same issue after this and they say the car is fine.

Where these numbers come from

A complaint is one owner's account and has not been verified by NHTSA or by us. A vehicle with more complaints is not necessarily worse: models that sold in larger numbers collect more of them. How this is counted.
