The defect
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2010-2014 Toyota Prius and 2012-2014 Toyota Prius V vehicles. Excessive voltage in the Intelligent Power Module (IPM) within the inverter may cause the hybrid system to shut down, causing the vehicle to stall while being driven.
The risk
If the hybrid system shuts down unexpectedly, it can increase the risk of a crash.
The remedy
Toyota will notify owners, and dealers will update the software for the motor/generator control electronic control unit (ECU) and the hybrid control ECU as necessary, free of charge. The recall began January 22, 2019. Owners may contact Toyota customer service at 1-888-270-9371. Toyota's number for this recall is J0V. Note: The software updates in this recall are needed even if the vehicle had the software previously updated under recalls 14V-053 or 15V-449.

HYBRID PROPULSION SYSTEM: INVERTER

The defect
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2012-2014 Prius V vehicles manufactured August 22, 2011, to June 30, 2014. A component within the hybrid inverter assembly may overheat, causing the hybrid system to reduce output power allowing the vehicle to only drive a short distance. In some circumstances, the hybrid system may shut down causing the vehicle to stop while being driven.
The risk
The vehicle may enter a fail-safe/limp-home mode that limits the drivability of the vehicle. The hybrid system could also shut down completely resulting in a vehicle stall, increasing the risk of a crash.
The remedy
Toyota will notify owners, and dealers will update the software for the motor/generator control electronic control unti (ECU) and the hybrid control ECU, free of charge. The recall began on July 30, 2015. Owners may contact Toyota customer service at 1-888-270-9371.

The defect
Southeast Toyota Distributors, LLC (SET) is recalling certain models interspersed through model years 2009 through 2013 as follows: model year 2009-2012 Tacoma, 4Runner, Camry, Camry Hybrid, Prius, and RAV4; model year 2009-2010 Avalon, FJ Cruiser, and Highlander Hybrid; model year 2010-2013 model year Corolla, Sienna and Tundra; model year 2009-2013 Highlander and Venza; model year 2012 Prius V; and model year 2010-2012 Sequoia. During modification by SET to include accessories such as leather seat covers, seat heaters or headrest DVD systems, these vehicles may not have had the passenger seat occupant sensing system calibration tested. Without passing the calibration test, the occupant sensing system may not operate as designed.
The risk
If the front passenger seat occupant sensing system is out of calibration, the front passenger airbags may not deploy or they may deploy inappropriately for the passenger's size and position. This could increase the risk of personal injury during the event of a vehicle crash necessitating airbag deployment.
The remedy
Southeast Toyota will notify owners, and dealers will test the sensitivity of the occupant detection sensors, and recalibrate them as necessary. The recall began on March 21, 2013. Owners may contact Southeast Toyota at 1-800-301-6859.

The Office of Defects Investigation (ODI) has received a petition dated September 19, 2023 from the owner of a 2012 Toyota Prius V requesting that ODI reevaluate its decision to deny DP19-004. In the letter, the petitioner reported experiencing an unexpected and complete loss of brake power while driving, accompanied by multiple dashboard warning lights. This petition is based on the petitioner's direct experience and assessment that ODI closed DP19-004 because its prompting petition was withdrawn by that petitioner. ODI will review the work conducted during DP19-004 and a review of any additional related complaints and crashes via a new defect petition The petition and related materials can be reviewed at NHTSA.gov under the following ODI number:11553736.

8/2/22 My ABS light came on. Took the car to a dealer. They kept it for six weeks and returned it on 10/3/22. No charge and no explanation. 11/7/22 engine light came on. Took in to dealer, they returned it 11/17/22 indicated incorrect air/ fuel mixture. 10/9/23 Traveling 47 mph, came around a mountain and encountered stopped cars. SERVICE BRAKES
Brake actuator assembly failure on a vehicle with 99,818 miles. Whirring sound when braking followed by lighting of several brake alert lights on dash including PCS and ABS and others. The whirring noise has occured for several weeks, but the light indicators happened on 6/28/26. Toyota has an acknowledged Customer Support Program (20TE07) for this exact failure on this exact and Toyota declined to cover the repair. This vehicle is 14 years old but has well under 150k miles and Toyota will not honor claim though they clearly are aware of the safety hazard.

Driving down the highway, the engine suddenly threw a rod, causing a terrible jolt, after which the engine burst into flames. There had been no warning lights.

SERVICE BRAKES
The ABS system has failed. All brake related warning lights have appeared on the dash board and the ABS no longer works. The car will brake for now, but the pedal is mushy. The issue was diagnosed by a dealer. The diagnosis from the dealer, based on diagnostic trouble codes they did not provide, is that brake fluid is leaking in the master cylinder and brake booster. The recommendation is to have both the master cylinder and booster replaced. This vehicle has 143,000 miles on it. No accidents have occurred as a result of the brakes yet but how long before the brakes ultimately fail is unknown.

SERVICE BRAKES
ABS, brake, and traction control lights, all illuminated at once on the dashboard. Inspection found DTC C1391 indicating the accumulator is leaking or failing to hold pressure correctly. We are advised that the braking system performance will decline, eventually failing. An very expensive repair is required and Toyota apparently has a shortage of the parts required. We cannot drive the vehicle given the potential of braking failure and cannot get it repaired in a timely manner for lack of Toyota parts.

140,000 miles

The contact owns a 2012 Toyota Prius V. The contact stated that while driving at an undisclosed speed, the brake master cylinder was constantly cycling. No warning lights were illuminated. The vehicle was not taken to the dealer or an independent mechanic to be diagnosed or repaired. Most recently, while the vehicle was turned off, the brake master cylinder was cycling. The contact stated that while the keys were in his hands and standing several feet away from the vehicle, the brakes started cycling. The manufacturer was notified of the failure but offered no assistance. The approximate failure mileage was 140,000.

SERVICE BRAKES
The power assist braking system failed while I was driving in the rain. I was approaching a stoplight and I had to push down a lot harder and farther than I typically have to in order to stop. Luckily I was not going that fast and there were no cars ahead of me, but I stopped beyond where I had intended at a busy intersection. No warning lamps or messages had been illuminated prior to this happening. After the failure, the Brake system, ABS, and SLIP indicator lights were on and remained on when the car was restarted. An independent service center used a scan tool and found code C1391 stored in memory, indicating that the brake booster is unable to hold pressure due to an internal leak. The
