Defect investigations (2)

An investigation means NHTSA has decided a pattern is worth examining. A preliminary evaluation is often closed with no action; an engineering analysis is the stage that usually precedes a recall.

Inadvertent Automatic Emergency Braking

EA24002

Still open engineering analysis · opened April 15, 2024

forward collision avoidance

On February 21, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation (PE22003) to assess reports of inadvertent activation of the Collision Mitigation Braking System (CMBS), an automatic emergency braking (AEB) system, in model year 2017-2019 Honda CR-V and 2018-2019 Honda Accord vehicles.  The reports allege that activation of the AEB system occurs while driving with no apparent obstruction in the vehicle's forward path, resulting in sudden vehicle deceleration. Honda indicated that they were aware of a total of X,XXX reports that may relate to the alleged defect. Honda provided analysis of the alleged defect and alleges that some customers possibly had an inadequate understanding of the CMBS and its limitations.  However, many consumer complaints allege that Honda dealerships were unable to reproduce the condition or state that they were informed that this is considered normal CMBS operation. To date, ODI has received a total of 1,294 consumer complaints of inadvertent activation of CMBS in 2017-2022 Honda CR-V and 2018-2022 Honda Accord vehicles. A total of 31 complaints alleged a crash and 50 alleged an injury.  The Total column in the Failure Report Summary removes duplicate reports and shows the total number of reports with unique VINs from all Manufacturer, ODI, and EWR data sources.  In some cases, there were multiple reports associated with a particular vehicle in which recurring failures were alleged.  In total, there were X,XXX reports, 93 injury incidents and 47 crashes involving vehicles with unique VINs that may relate to the alleged defect. PE22-003 has been upgraded to an Engineering Analysis to further assess the scope, frequency, and potential safety related consequences of the inadvertent AEB activations. The scope has been expanded to include assessment of model year 2020-2022 Honda CR-V and Accord vehicles. To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.

Pedestrian alert sounds

DP22005

Closed defect petition · opened January 27, 2023 · closed August 7, 2023 · led to recall 22V063000

the electrical system

NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

SERVICE BRAKES

crash · 3,803 miles

FROM BEING STOPPED AT A STOP LIGHT, I THEN PROCEEDED THROUGH THE INTERSECTION ONCE THE LIGHT TURNED GREEN. THERE WAS A STEEL CONSTRUCTION ROAD PLATE ON THE ROAD JUST BEYOND THE FAR SIDE OF THE INTERSECTION AND AS I APPROACHED IT AT APPROX 25MPH, THE EMERGENCY BRAKING WARNING SIGNAL ON THE DASH CAME ON AND THE CAR AUTONOMOUSLY BRAKED HEAVILY TO A STOP JUST BEFORE THE ROAD PLATE. THERE WERE NO CARS OR OBJECTS IN FRONT OF ME OTHER THAN THE STEEL ROAD PLATE. THIS HEAVY BRAKING RESULTED IN MY VEHICLE BEING REAR ENDED BY AN SUV. FORTUNATELY NO ONE WAS HURT.

ENGINE
The engine failed catastrophically. White smoke out of the exhaust at 145,000 miles. Dealer checked out the car and they initially though it was a head gasket failure. After taking the engine head off, it turns out it was a 'structural defect' (verbatim words from the mechanic), which was a divot in the engine block. The head gasket was in great shape. Therefore, I needed a 'new' used engine (at $9K) instead of a head gasket replacement (at $4K). A search on the internet shows that forums (driveaccord.net, facebook groups) shows that many Honda Accord owners have similar head gasket failures. I contacted Honda USA and they said there's no recall so I won't get reimbursed.

ELECTRICAL SYSTEM
I am reporting a potential safety issue with my 2020 Honda Accord Touring Hybrid. On September 4, 2025, my dealership identified a non-genuine purple quick-splice connector in the driver’s door harness, which they attribute to error codes B1142 (Driver’s Door Lock Knob Switch Circuit), U0199 (Lost Communication with Driver’s Door Module), and U3003-16 (Electric Servo Brake Control Unit Power Source Low Voltage). This connector, not installed by the recent audio installer who put only upgraded speakers and deemed non-Honda, may be an aftermarket modification (possibly an alarm/tracker or anti-theft device) causing electrical faults. While no water was found, the issue could impact brake

ENGINE
Five times the past year, while driving on the freeway, the first incident I cannot remember, but I do know that the other four incidents were on a slight incline. The car acted as if it wasn’t getting enough gas and slowed down and was shaking. Giving gas would not help, and it kept slowing down. The check engine light would come on and once I turned off the car and turned it back on, the car acted normal again. Sometimes I had to keep it turned off at least a minute before it would run normal again. I took it to the dealership twice and both times I was told that because the engine light was not on, they did not know what was wrong. I asked if they could check the computer to look for

While diving at highway speeds (first at 55mph in may 2025 and then again in July 2025 at 65mph) I got a warning on screen about power loss and a critical failure and that power will be reduced and to stop at the side of the road. Within 30 seconds or a minute of the warning the car power cut out significantly impacting the speed and both times almost causing traffic behind me to hit me from behind. I took it to Honda dealership in dulles, va back in may 2025 and they told me it was the electric water pump which I had replaced. Now in July it happened again and this time I had my wife and child with me in the car as well as a guest. I had to pull over and all the warning lights started to

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
The Collision Avoidance system activated twice for no reason. Inspection is available. I and my passenger were jerked by the rapid stop & could have been rear ended if anyone was behind me. The second event was in an intersection when I and the driver behind me were driving slowly. if the car behind wasn't alert, he or she could have collided with me. No one was travelling in front of me and yet the brake suddenly engaged. The dealership rep. did not confirm the problem after driving it 5 minutes. There were no warning lamps, messages, or symptoms before the brake engaged in both instances.

Where these numbers come from

A complaint is one owner's account and has not been verified by NHTSA or by us. A vehicle with more complaints is not necessarily worse: models that sold in larger numbers collect more of them. How this is counted.
