Defect investigations (3)

An investigation means NHTSA has decided a pattern is worth examining. A preliminary evaluation is often closed with no action; an engineering analysis is the stage that usually precedes a recall.

Inadvertent Automatic Emergency Braking

EA25002

Still open engineering analysis · opened January 17, 2025

forward collision avoidance

On March 7, 2024, the Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE24008) to assess reports of inadvertent activation of the Automatic Emergency Braking (AEB) system in model year 2019-2022 Honda Insight and 2019-2022 Honda Passport vehicles. The complainants allege activation of the AEB system with no apparent obstruction in the vehicle's path, resulting in rapid vehicle deceleration. Honda indicated that it is aware of a total of 412 reports that may relate to the alleged defect. Honda provided analysis of the alleged defect and stated that some customers possibly had an inadequate understanding of the AEB system and its limitations. However, many consumer complaints received by ODI allege that Honda dealerships were unable to reproduce the condition or state that Honda dealerships informed the consumer that this is considered normal AEB operation. To date, ODI has received a total of 106 consumer complaints of inadvertent activation of AEB in the subject vehicles. In total, ODI reviewed 475 reports involving vehicles with unique VINs that may relate to the alleged defect. Of the reports reviewed by ODI, three (3) allege a crash caused by the inadvertent activation of AEB and two (2) allege injury. This Engineering Analysis is being opened to further assess the scope, frequency, and potential safety related consequences of the inadvertent AEB activations. Further, the scope of the investigation is being expanded to include assessment of model year 2023 Honda Passport vehicles. To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.

the electrical system

NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

SERVICE BRAKES

1 injured

I have filed a complaint to the Honda Motor Company in Torrence, California. I have notifed Honda of a serious defect with our vehicles Collision Mitiagation Braking System. Here are the defects with our Honda Insight 2022 vehicle: 1) On three occasions while my wife Nino is driving on the 470 freeway in Colorado between Lone Tree and Littleton CO. Brake warning light comes on, light comes on randomly while driving 65 mph, then the Collision Mitiagation braking system comes on and slows the vehicle down to 15 mph while on the Freeway speed of 65 mph. While the brakes are engaging a forceful vacuum and pressure enters the cabin and that pressure goes to my wife's chest which casues pain. 2)

While I was going 5-10mph over a railroad crossing, the steering wheel airbag deployed as my front tires went over the grade crossing. The airbag also went left instead of straight. My hand is bruised and it sounded like a gunshot went off in my vehicle. After the deployment, the vehicle was still driveable. I drove it to a Honda dealership and they were very concerned and called corporate.

ELECTRICAL SYSTEM
The radio is experiencing "ghost taps" as though someone invisible finger is pushing buttons on the radio, rifling through stations, making calls, or deleting navigation functions. It's very distracting as a driver.

NHTSA complaint 11747027. Filed by an owner and not verified by NHTSA or by us.

ELECTRICAL SYSTEM
BCM Body Control Module failed/malfunctioned. We had to pay for the repair at DCH Honda Mission Valley. The car does not start when it is cold outside. Many times, my husband, who primarily drives the car, was stranded and would have to wait for the weather to warm up a bit and then eventually, the car would then start. The dealership where we initially purchased the car new was able to determine, with the assistance of the corporation, that what we said happened. All the warning lamps/messages appeared every time this issue happened, beginning January 13, 2025.

NHTSA complaint 11726149. Filed by an owner and not verified by NHTSA or by us.

FUEL/PROPULSION SYSTEM
This issue actually started in November of 2022. Driving down from the mountains the car started losing power. I took the car to the dealership on Monday. I was then advised that there was not an issue they could they could find. A few months later the car would not start. They replaced the battery and the issue was resolved (I don’t recall the exact month the battery was replaced). November 29,2025, I started my car and when I accelerated to about 30+ miles the car started to hesitate and it felt as if the car was going to stop. The check engine light came on and stayed on. I had over a half a tank of gas and temperature was around 28-30 degrees. I drove the car for about 2 miles to fill

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
Automatic brake system activated even though there were no cars in front of me or behind me. Car came to a complete stop which would have been a huge risk if someone was behind me. I have also had other instances where the auto brake would randomly activate and stop the car, but not as severe as a full stop on this one.

Where these numbers come from

A complaint is one owner's account and has not been verified by NHTSA or by us. A vehicle with more complaints is not necessarily worse: models that sold in larger numbers collect more of them. How this is counted.
