Defect investigations (3)

An investigation means NHTSA has decided a pattern is worth examining. A preliminary evaluation is often closed with no action; an engineering analysis is the stage that usually precedes a recall.

Loss of Motive Power

PE25016

Still open preliminary evaluation · opened October 28, 2025

the engine

The Office of Defects Investigation (ODI) has received 48 Vehicle Owner Questionnaire (VOQ) reports and some Early Warning Reporting (EWR) field reports alleging intermittent loss of motive power in Model Year (MY) 2020–2022 Honda CR-V Hybrid vehicles.  These incidents are alleged to occur without warning at highway speeds and are accompanied by a flashing check engine light immediately before the loss of motive power occurs.  Motive power can be restored by turning the ignition off and back on.  However, this action also clears the check engine light and any fault codes that were set.  The fault’s intermittent nature, combined with the lack of fault codes, often results in many visits to repair facilities yielding no repairs. Honda indicated it was unaware of any related complaint or repair trends. ODI is opening this Preliminary Evaluation (PE) to determine the scope and severity of the alleged fault and to fully assess the possible safety consequences. To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.

Inadvertent Automatic Emergency Braking

forward collision avoidance

On February 21, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation (PE22003) to assess reports of inadvertent activation of the Collision Mitigation Braking System (CMBS), an automatic emergency braking (AEB) system, in model year 2017-2019 Honda CR-V and 2018-2019 Honda Accord vehicles.  The reports allege that activation of the AEB system occurs while driving with no apparent obstruction in the vehicle's forward path, resulting in sudden vehicle deceleration. Honda indicated that they were aware of a total of X,XXX reports that may relate to the alleged defect. Honda provided analysis of the alleged defect and alleges that some customers possibly had an inadequate understanding of the CMBS and its limitations.  However, many consumer complaints allege that Honda dealerships were unable to reproduce the condition or state that they were informed that this is considered normal CMBS operation. To date, ODI has received a total of 1,294 consumer complaints of inadvertent activation of CMBS in 2017-2022 Honda CR-V and 2018-2022 Honda Accord vehicles. A total of 31 complaints alleged a crash and 50 alleged an injury.  The Total column in the Failure Report Summary removes duplicate reports and shows the total number of reports with unique VINs from all Manufacturer, ODI, and EWR data sources.  In some cases, there were multiple reports associated with a particular vehicle in which recurring failures were alleged.  In total, there were X,XXX reports, 93 injury incidents and 47 crashes involving vehicles with unique VINs that may relate to the alleged defect. PE22-003 has been upgraded to an Engineering Analysis to further assess the scope, frequency, and potential safety related consequences of the inadvertent AEB activations. The scope has been expanded to include assessment of model year 2020-2022 Honda CR-V and Accord vehicles. To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.

Pedestrian alert sounds

DP22005

Closed defect petition · opened January 27, 2023 · closed August 7, 2023 · led to recall 22V063000

the electrical system

NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

As I was backing out of a driveway the car suddenly accelerated in reverse. The brake did not stop or slow the vehicle. As a result the car's right fender hit a large oak tree. There was damage to the fender, the door and to the undercarriage. In addition, my wife who was sitting in the passenger seat was thrown back and she struck her head on the top seat belt connection. She had a cut in the back of her head that required an emergency room visit to the local hiospital. They did a CAT scan and they used seven staples close the wound. There were several messages flashed on the dash that indicated brake and cruise control issues. These messages wer of very short duration and could not be

In April 2022, my dad purchased a brand new Honda CRV 2022 hybrid touring edition. This car was deemed as one of the safest cars on the market and was his main intent while buying it. A couple of weeks ago this car was the prime suspect of a fire that impacted both the car and our house. Let me break down exactly how this occurred and where the blame should fall on this situation. When I was slowly backing out of my garage, I noticed that my car was not straight and that I needed to turn my wheel in order to straighten it out. When doing so, 3/4 of my car made it out of the garage, even the side mirrors. However, the area on the top left on my wheel scratched the side of my garage. When

I was going south bound up a hill. Less then 20 mph and was blinded by the sun. The old folks in front of me were also blinded by the sun and stopped in the middle of the hill. My automatic emergency braking system did not kick in and I ran right into them. We could have been hurt. My car is being fixed in a collision repair shop now.

UNKNOWN OR OTHER
When driving the vehicle will lose acceleration and not allow me to accelerate. It feels like it wants to shut off. The check engine light will come on when this happens. I turn the car off and turn it back on and it goes back to baseline and lets me drive as normal. This has happened while accelerating to get on the highway from an on-ramp, at around 45-60 miles an hour and also at city speeds slowing down to a stop light. I notice some rough idling/shaking during idle at times as well. I have not taken it into the dealer yet because it started happening over the weekend when the dealer was closed. Vehicle has not yet been inspected by anyone. I do believe the first time it happened that

POWER TRAIN
I am reporting a systemic safety fraud involving three critical points:1. INHERENT DEFECT & CRASH LINK: This vehicle has a pathological history of 6 transmission fluid changes starting from 308 miles and multiple alignment failures. There is strong suspicion of inherent defects in both the POWERTRAIN and REAR DRIVETRAIN/STEERING systems. These defects likely contributed to the previous owner's 5-side collision in May 2022. The record suggests a "Sudden Loss of Motive Power" and "Control Instability" risk existed from day one.2. RECALL MALPRACTICE (#23V-844): Despite the massive structural damage from the crash (bent hinges and frame misalignment), the dealer (Planet Honda) certified the

POWER TRAIN
Transmission failure without any warning while driving. Vehicle engine immediately shut down resulting in no power and loss of power steering. Vehicle came to a complete stop within seconds. Fortunately I was on a country road with no traffic. Had I been traveling on a highway I could have been rear ended and suffered major injuries! I had to push the vehicle to the side of the road since there was no power. There was a recall of the 2023 model for the exact same transmission issue that I experienced. There are Honda owners driving 2022 CRV models like mine who are unaware that their vehicle has a faulty transmission who could suffer serious injury. These owners need to be informed

FUEL/PROPULSION SYSTEM
I am getting increasing frequency of cylinder misfire, so far cylinder 1 and 2, and the car goes to a limb mode and loose power. It happens during acceleration to a highway ramp. Engine lights and other many warning lights come on, the car goes into limb mode, I pull over and when I turn the engine off and turns back on the warning lights clear and the car drives normally until next time, to start all over again. I took it two Honda dealers at three occasions. Since the engine light goes off, and they cannot capture the issue, they are telling me there is nothing they can do. It seems to happen during a normal driving than sport mode. It is frustrating and dangerous situation. I thought I

STEERING
My car quickly became unable to be driven safely when the steering started to fail. It started with a feeling of the tires sticking to the road when trying to make small adjustments to position with the steering wheel. I was not able to maintain a straight line drive without the constant small jerks associated with the sticky feeling. The steering quickly deteriorated to the point of barely being able to keep the car on the road because of the inability to steer. The issue was confirmed by the dealer and the driveshaft components replaced, at my expense. I was told that there is a known defect in the 2022 CRV hybrids but my VIN was not included in the recall for some reason. There were no

on [XXX] around 5 PM, when driving below 30 miles per hour within a residential community, automatic braking was activated without any obstruction in front of the vehicle, leading to sudden decrease in speed. Vehicle is in Baton Rouge, LA and all service/maintenance were done according to recommended schedule by Honda at the authorized dealership where the vehicle was purchased. Through online search, I found NHTSA opened an investigation for the same problem. If possible, I would appreciate an update about the status of NHTSA investigation and what action I should take.
