Timing Belt Failure

EA26004

Still open engineering analysis · opened July 31, 2026

the engine, the engine and its cooling

On December 18, 2025, the Office of Defects Investigation (ODI) opened PE25020 to investigate instances of timing belt failure in model year (MY) 2015-2017 Ford Fiesta and MY 2015-2018 Ford Focus vehicles manufactured by Ford Motor Company (Ford), equipped with a 1.0L engine and manual transmission (subject vehicles). Additionally, MY 2018-2021 Ford EcoSport and MY 2016-2018 Focus vehicles with a 1.0L engine and automatic transmission were analyzed as peer vehicles because they utilize a wet timing belt comprised of similar material as the timing belt in the subject vehicles. Through the PE, ODI investigated a potential failure/degradation of the timing belt that may affect oil pump function resulting in a lack of engine lubrication and potential loss of motive power. The complaints commonly allege a low engine oil pressure warning lamp immediately preceding a complete loss or reduction of motive power while the vehicle is in motion. Some complaints allege that a sudden loss of motive power or engine seizure occurred without warning or prior detectable symptoms. ODI’s analysis supports that this failure can occur without sufficient and advanced warning to the driver that a loss of motive power/engine seizure is imminent. ODI found that the timing belt material may degrade and create debris that clogs the mesh oil pump pick-up screen, resulting in reduced oil pressure in the engine. As additional debris accumulates on the screen, this can progress to a loss of motive power while driving due to a lack of engine lubrication. The failure data provided in response to this investigation revealed an average failure mileage of approximately 70,000 miles, and that 98% of the failures occurred prior to the 150,000 mile timing belt replacement interval per the vehicles’ maintenance schedule. Based on evidence collected via consumer surveys and an engine teardown, failures have occurred despite evidence of proper and routine oil maintenance. The oil pump pick-up screen is not serviceable, so debris accumulation cannot be inspected or manually removed without removing the engine from the vehicle. On June 2, 2026, Ford informed ODI of a non-safety customer satisfaction program for global vehicles with the 1.0L Fox Classic Timing Belt. This program will notify customers globally of a revised maintenance interval for the timing belt, reducing the original 150,000 mile interval down to 100,000 miles/6 years. This customer satisfaction program will also offer reimbursement to eligible customers who previously paid to have their engine repaired or replaced due to a timing belt related issue. Based on NHTSA’s analysis of the data, failure rates, information provided by Ford, preliminary engine teardown analysis, and precedent recalls regarding loss of engine oil pressure with the presence of driver facing warnings, ODI believes there is an unreasonable risk to motor vehicle safety. ODI is opening this Engineering Analysis to perform component-level and vehicle testing. To review the reports cited in the Closing Resume ODI Report Identification Number document, go to NHTSA.gov.

Door Latch Failures

RQ19005

Closed recall query · opened December 19, 2019 · closed February 7, 2022 · led to recall 20V177000

the latches, locks and linkages, the body structure

In December 2019, the Office of Defects Investigation (ODI) opened this investigation based on 235 Vehicle Owner Questionnaires (VOQs) alleging doors failed to latch when closed, and/or inadvertently opened while driving, on model year (MY) 2012-2014 Ford Fiesta, and 2013-2014 Fusion and Lincoln MKZ. Some VOQs reported the failure occurred after the latch had been remedied under a prior recall which involved fracture of the latch pawl spring tab due to thermally induced cyclic fatigue. Other VOQs alleged a similar failure in vehicles not previously recalled. A latch with a fractured pawl spring tab typically results in a ?door will not close? condition, however in certain somewhat rare circumstances, may result in the door coming open while driving. In response to ODI?s investigation, Ford has announced three field actions, as discussed below. On March 23, 2020, Ford submitted Safety Recall 20V-177 (Ford 20S15), a regional recall campaign to address door latch failures in vehicles not previously recalled under Safety Recalls 15V-246, 16V-643 and 17V-210. The recall affects vehicles currently or ever registered in certain high ambient temperature states (see 20V-177 for affected states). As a remedy, Ford will replace all four door latches free of charge. On August 24, 2020, Ford issued a non-safety regional field action (Ford 20R01) to address door latch failures on the same model/MY vehicles not covered under 20V-177 (i.e., not currently/ever registered in affected states). According to Ford, spring tab failure rates for vehicles covered by this action are about half that of the 20V-177 vehicles. Under this action, dealers will replace all door latches, free of charge, if either; a) the vehicle exhibits a related latch concern, or b) if the vehicle owner expresses a concern about the latches in their vehicles. Details can be found under NHTSA TSB No. 10190990, a copy of which is available in this investigative file available at NHTSA.gov.On June 8, 2020, Ford submitted Safety Recall 20V-331 (Ford 20S30) addressing vehicles previously remedied under NHTSA Recalls 15V-246, 16V-643 and 17V-210, where the repair may not have been properly completed, specifically not all of the door latches were replaced, or replaced correctly. This recall involves an inspection of the latches, with the option for a consumer self-inspection; owners could view the date codes on their latches (and confirm child safety locks are present on the rear doors) and then submit the information to Ford?s mobile app or website. A dealership inspection option was also available. Any vehicle found to have unreplaced defective latches requires the replacement of all door latches as directed by Ford.Regarding the 4,494 reports cited above, 1,485 involve scope-related failures (with 13% alleging door opening), and the remaining 3,009 reports involving remedy concerns (with 18% alleging door opening - note ODI was unable to verify the allegations of door opening). The three ODI injuries were all relatively minor. Ford?s reported injury incidences include one (unconfirmed) report of an ejection, three minor injuries with the others involving undisclosed injuries. Ford?s crashes involved two (unconfirmed) crashes alleging the incident door contacted another vehicle and three involving doors striking other property. The recalled vehicles include MY 2011-2015 Ford Fiesta, 2013-2016 Ford Fusion and Lincoln MKZ, 2015 Ford Mustang and Lincoln MKC, 2013-2015 Ford Escape and Ford C-MAX, 2012-2015 Ford Focus and 2014-2016 Ford Transit Connect.Ford's actions address ODI's concerns at this time. ODI will take further action if warranted by new information. The reference (OD

My vehicle has an active status under Ford Safety Recall 20S15 (NHTSA Campaign 20V177) for a fractured door latch pawl spring. The defect has fully manifested: my side door physically will not latch or stay shut. There was no warning that the latch has malfunctioned. I was driving and my door swung open when I made a turn. I’ve been trying to get my car serviced because of the safety recall involving the door latch. Unfortunately, my driver’s side passenger door won’t physically shut. While my recall allows for free repairs, I can’t drive my car to the dealership because it’s unsafe. I’ve spoken to supervisors in Customer Relations, Services, and Body Shop, as well as Recall Corporate. I

Transmission was supposedly rebuilt back July 26, 3021. Now transmission has had a new dual clutch replaced but other concerns are still there jerkiness on accelerations. In 2021 the transmission was not fully rebuild as per recall. Only clutch assembly was done and not the back end. Which now could be the problem. Second, colonial Ford sated transmission rebuilt has lifetime warranty which a lie. Colonial Ford doesn’t offer lifetime. The service advisors lie about this all the time. Now the car jerks on acceleration after clutch replacement. Dealer and Ford still take no responsibility over the product! The car has 145,000 miles still under the 150,000 mile warranty isssue by you!

SEAT BELTS
Seat Belt retractor/pretensioner failed and was causing the passenger seatbelt light to go off. Had to repair due to automatic failing of inspection for this issue and the major safety issue this poses. Auto shop charged me $784.82 for today's repair and over $300 (don't remember exact amount, if claim moves forward I will contact them for another copy of that invoice) for a different repair on the seatbelt. I called Ford and they told me their 5 Year/60K warranty on the car expired. I told them that under Federal Code 49 USC Subsection 30120 the manufacturer is required to repair such safety, they told me they couldn't help me and to go away, despite the clear language of this code.

I’m reporting a failure with the Transmission Control Module (TCM) in my 2014 Ford Fiesta. The vehicle has exhibited consistent issues tied to the known PowerShift DPS6 transmission defect: including gear slipping, delayed acceleration, hesitation, and sudden loss of power while driving—particularly dangerous during intersections or merging situations. Ford previously issued Customer Satisfaction Program 14M02 to address this issue, but I was told my VIN is no longer eligible due to build date. I also requested coverage under Ford’s one-time TCM replacement extension announced in 2023, which was meant to support customers outside the 10-year window through June 30, 2025—but I was denied
