Defect investigations (1)

An investigation means NHTSA has decided a pattern is worth examining. A preliminary evaluation is often closed with no action; an engineering analysis is the stage that usually precedes a recall.

the body structure

On January 27, 2023, the Office of Defects Investigation (ODI) opened PE23-001 to investigate allegations of A-pillar trim panel detachment on model year (MY) 2011-2019 Ford Explorer vehicles. At the time of opening, the office had received 164 Vehicle Owner Questionnaires (VOQs) reporting this failure. ODI has received 671 VOQs regarding this failure, including 1 alleged crash and 2 alleged injuries. Some consumers have reported that the failure occurred while they were driving at highway speeds, with the detached trim panel entering the path of travel of the vehicle behind them, requiring the driver of the following vehicle to take evasive action. It is noteworthy that motorists who experience this type of road hazard are less likely to accurately identify the subject vehicle the item detached from and report the event to the NHTSA. In its April 17, 2023 response to ODI’s information request, Ford provided 175 consumer complaints (including field reports) and 14,162 warranty claims related to A-pillar trim panel detachment. Ford relayed that the subject vehicles utilize a plastic A-pillar bracket, which is bolted to the vehicle structure during assembly. The A-pillar trim panel is attached to the bracket utilizing 7 plastic clips. Contributory factors which have been identified by Ford as leading to the subject defect include incomplete installation at the assembly plant, inadequate part stack up, damage to fasteners during installation, and failure to follow proper repair procedure during windshield replacements or other services. On January 19, 2024, Ford filed safety recall 24V-031 for MY 2011-2019 Ford Explorer vehicles to address the subject defect. In the Part 573 Safety Recall Report, Ford identified that a detached A-pillar trim panel can create a road hazard for other road users, increasing the risk of a crash. The recall remedy includes an inspection of the existing A-pillar trim panel and, if necessary, the replacement of the component. The remedy A-pillar trim panels will be installed with an additional adhesive for robustness and utilize an inspection procedure to ensure full engagement of all the retention clips during installation. In view of the recall action being taken by Ford, ODI is closing this Preliminary Evaluation. The agency reserves the right to take additional action if warranted by future circumstances. To review the reports cited in the Closing Resume ODI Report Identification Number document, go to NHTSA.gov.

on 3/6/2020 I was in a serious car accident in the vehicle. which required surgeries for not only myself but also other drivers on the road. The driver air bags did not deploy, the turck was totaled and had body damage around the entire vehicle. There was a blood stain from my head hitting the driver door window but no air bag deployment. The gas pedal was not attached when reviewing the photos taken. The collision detection did not detect. we have asked Ford Motor Company to come and look at the car as it has been in a protective hold but the response from FMC was that due to covid they would not send anyone and we were left on out own. Found significant MOLD (toxic mold) all over passenger side floorboard area, under the carpet and throughout the padding, after increasing moldy/mildew smell. This is a "direct result" of a bad design (i.e. AC drain tube), where condensation flows back into the vehicle without Consumer/Owner knowledge. Technical Service Bulletin 19-2345. RESULT: Consumers spend thousands of hours in a small contained cabin breathing-in toxic mold spores. POWER TRAIN
The transmission consistently slips, stalls, and shifts very aggressively between gears, specifically between gears 3-4, 7-8, and 6-5. This occurs at various speeds and levels of acceleration. This puts everyone on the road at risk as it makes it difficult to control the speed of the vehicle. Vehicle is available for inspection upon request. This was inspected at a Ford dealership and confirmed to be a problem by the transmission specialist. There are no warming lamps or error messages.

The transmission slipped out of gear while entering an intersection. Several slipping issues with transmission and Ford dealers refused to look into issue. They have recalled the 10 speed automatic transmissions for several years but not the mine?

POWER TRAIN
What happened is i bought a truck that Ford knows good & well has a defective CDF drum and outer shell but they refuse to issue a recall to fix the transmissions in those year models! Yet in late 2022 they acknowledged the problem and updated the 22+ year models with the correct parts while leaving those of us with the prior 4yrs of trucks out in the cold! My truck is currently undergoing getting a new transmission and it hasn't even reached 70K yet. At 61K I had to have all timing components along with a new oil pump replaced due to the common oil consumption issues on the 13th gen models. Now 8K miles later I'm having to have a new transmission because it started with all the common

POWER TRAIN
I bought my 2019 F-150 a few months ago used. Since then it’s been nothing but problems and honestly, terrifying one’s. The truck has shifted and driven rough since I bought it. But on some occasions it goes from rough driving to potentially accident causing. The truck will randomly decide it does not want to shift from a gear, meaning I can’t speed up on busy 95 traffic or move out of the way of aggressive drivers when needed. The truck also will occasionally shift up or down on its own without warning causing me to slow down quickly while already at interstate speeds. At this point, because I have no choice but to drive it to work, it’s a matter of waiting for the day it downshifts too

POWER TRAIN
I am filing a safety complaint regarding the transmission in my 2019 Ford F-150 equipped with the 5.0L V8 and 10-speed automatic transmission. My vehicle currently has approximately 88,000 miles. The transmission began experiencing abnormal shifting, including skipping/hesitating between gears, harsh shifting, shuddering, and delayed engagement. At times it would appear to skip 2nd gear and go directly into 3rd. I brought the vehicle to a Ford dealership for diagnosis. Ford of Spartanburg performed testing and a road test. Their written diagnosis states that the PCM was tested and had no related DTCs. The technician documented delayed 1-2-3, 2-4 and 3-4 shifts, a 400-500 RPM flare during
