An investigation means NHTSA has decided a pattern is worth examining. A preliminary evaluation is often closed with no action; an engineering analysis is the stage that usually precedes a recall.

No Restart After Auto Start/Stop Engages

EA25004

Still open engineering analysis · opened March 26, 2025

the engine and its cooling

On June 3, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE22005 after the agency received VOQs and several field reports concerning the Auto Idle Stop (AIS) feature on 2016-2019MY Honda Pilot vehicles. The complaints allege that the engine fails to restart on its own from a complete stop at a traffic light or road intersection with the AIS function engaged. The subject vehicles are equipped with a 3.5L engine with a 9-speed automatic transmission. On January 6, 2023, Honda released service bulletins 23-008 and 23-009. These service bulletins address the failure to restart issue in the 2016-2019MY Honda Pilot vehicles, as well as the 2015-2020 Acura TLX, 2016-2020 Acura MDX, 2019-2022 Honda Passport, and 2020-2023 Honda Ridgeline vehicles. The repair outlined in the service bulletins is a two-stage countermeasure to remedy the issue. The first stage is a Programmed Fuel Injection (“PGM-FI”) software update. The second stage is only deployed if the software update fails to remedy the issue. The second stage, depending on the model and model year, consists of replacement of the starter assembly, starter relays, and a valve adjustment. Additionally, Honda extended the warranty coverage to 10 years with unlimited mileage for vehicles requiring the second stage component replacement. ODI continues to receive complaints of the AIS failure to restart, with many complainants alleging the campaign countermeasures had been completed.NHTSA has reviewed Honda's Information Request Letter responses involving warranty claims, lawsuits, and consumer complaints in addition to ODI's own data. This Engineering Analysis is being opened to further consider the potential safety defect, including gathering additional data regarding Honda’s service campaign efficacy. ODI is additionally expanding the scope to encompass all vehicles addressed in Honda’s service bulletins 23-008 and 23-009 as well as newer model year vehicles, which are substantially similar to the vehicles covered by the service campaign. To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.

Inadvertent Automatic Emergency Braking

EA25002

forward collision avoidance

On March 7, 2024, the Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE24008) to assess reports of inadvertent activation of the Automatic Emergency Braking (AEB) system in model year 2019-2022 Honda Insight and 2019-2022 Honda Passport vehicles. The complainants allege activation of the AEB system with no apparent obstruction in the vehicle's path, resulting in rapid vehicle deceleration. Honda indicated that it is aware of a total of 412 reports that may relate to the alleged defect. Honda provided analysis of the alleged defect and stated that some customers possibly had an inadequate understanding of the AEB system and its limitations. However, many consumer complaints received by ODI allege that Honda dealerships were unable to reproduce the condition or state that Honda dealerships informed the consumer that this is considered normal AEB operation. To date, ODI has received a total of 106 consumer complaints of inadvertent activation of AEB in the subject vehicles. In total, ODI reviewed 475 reports involving vehicles with unique VINs that may relate to the alleged defect. Of the reports reviewed by ODI, three (3) allege a crash caused by the inadvertent activation of AEB and two (2) allege injury. This Engineering Analysis is being opened to further assess the scope, frequency, and potential safety related consequences of the inadvertent AEB activations. Further, the scope of the investigation is being expanded to include assessment of model year 2023 Honda Passport vehicles. To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.

I have has consistent problems with the braking system. It will brake and/or show the warning when nothing is there. The jerking of this has caused me to get in an accident June 2020 while moving to a new duty station. It was raining and there were no other cars on this highway thankfully. It was scary and I was even going below the speed limit. This caused my car to become nearly uncontrollable. My children and I were fortunately unharmed, but pretty shaken up. I had everything repaired and the same thing still happens to this day. That was only about 3 months after I purchased my car. I hear a lot of rattling as if something has come undone inside. Many times when I am driving I can feel

ENGINE
The vehicle engine is dying at intersections and going into “neutral” on its own when stopped. This has been ongoing for over a year, Honda denies any issues. It is becoming more of a safety problem more recently as the vehicle “downshifted” on its own while driving on the highway. (On that instance, the vehicle went from approx 45 mph to a slow crawl - just enough to pull off the road to safety).

SUSPENSION
What component or system failed or malfunctioned, and is it available for inspection upon request? The rear subframe, specifically the passenger-side rear suspension mounting bracket, is failing due to severe, premature corrosion. How was your safety or the safety of others put at risk? My safety was put at extreme risk because a local authorized Honda dealership fraudulently closed out an active, critical safety recall (NHTSA Campaign Number 26V-365) without actually performing the mandated repair. They returned the vehicle to me to drive on public roads with a heavily rusted, structurally compromised rear subframe where the flange seams are actively separating. Driving this vehicle poses

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
All safety features failed to work at the same time associated with forward looking camera u-3000-49. Dealer inspected vehicle stating that the issue was the failed camera. It was during the winter after a snow storm, we went to dinner and a mile from home at Rockwell and NW Expressway in Oklahoma City, OK. All of the warning lights went haywire. We went straight to dealer service center, Where they wiped the sensor/camera. Resolved for 1-2 hours and started again. After another evaluation, told it was the noted camera above malfunctioning and would be $2000 to repair.

LANE DEPARTURE: WARNING
About 1 year ago when the car was only 6 yeas old a series of warning lights lit up on the dashboard of our 2019 Honda Passport. The warning lights showed failures in several of the Honda sensing suite safety features including adaptive cruise control, lane keeping assist, collision mitigation braking system, road departure mitigation, vehicle stability assist, and auto high beam problem. The Honda dealership told us it could be a problem with the front facing multi-purpose camera or the radar and the problem was triggering a full failure of the entire Honda sensing suite and all of the safety features. They were unable to diagnose the issue and suggested replacing and recalibrating both

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
There has been multiple times that my 2019 passport has had the “brake” indicator pop up on my dash telling me to brake when there is nothing there I mean like a completely empty road. There has been a handful of times that it has actually stopped my car like slammed the brakes when there is nothing around or a couple times when a car was passing on the other side of the roadway. This has happened multiple times with someone driving behind me and I fear it could cause a crash due to the car randomly braking so hard.

NHTSA complaint 11748679. Filed by an owner and not verified by NHTSA or by us.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
The safety features connected to the rear view mirror and vehicle sensors malfunctioned. The Adaptive Cruise Control, Collision Mitigation System, Road Departure Mitigation, Lane Keep Assist and High Beam - brake system - have stopped working. It was diagnosed as an internal failure of multipurpose camera. I have noticed there have been a lot of complaints of this failure. My car doesn’t have that many miles on it and isn’t that old. It seems like a defect in the car.
