Defect investigations (2)

An investigation means NHTSA has decided a pattern is worth examining. A preliminary evaluation is often closed with no action; an engineering analysis is the stage that usually precedes a recall.

Inadvertent Automatic Emergency Braking

EA24002

Still open engineering analysis · opened April 15, 2024

forward collision avoidance

On February 21, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation (PE22003) to assess reports of inadvertent activation of the Collision Mitigation Braking System (CMBS), an automatic emergency braking (AEB) system, in model year 2017-2019 Honda CR-V and 2018-2019 Honda Accord vehicles.  The reports allege that activation of the AEB system occurs while driving with no apparent obstruction in the vehicle's forward path, resulting in sudden vehicle deceleration. Honda indicated that they were aware of a total of X,XXX reports that may relate to the alleged defect. Honda provided analysis of the alleged defect and alleges that some customers possibly had an inadequate understanding of the CMBS and its limitations.  However, many consumer complaints allege that Honda dealerships were unable to reproduce the condition or state that they were informed that this is considered normal CMBS operation. To date, ODI has received a total of 1,294 consumer complaints of inadvertent activation of CMBS in 2017-2022 Honda CR-V and 2018-2022 Honda Accord vehicles. A total of 31 complaints alleged a crash and 50 alleged an injury.  The Total column in the Failure Report Summary removes duplicate reports and shows the total number of reports with unique VINs from all Manufacturer, ODI, and EWR data sources.  In some cases, there were multiple reports associated with a particular vehicle in which recurring failures were alleged.  In total, there were X,XXX reports, 93 injury incidents and 47 crashes involving vehicles with unique VINs that may relate to the alleged defect. PE22-003 has been upgraded to an Engineering Analysis to further assess the scope, frequency, and potential safety related consequences of the inadvertent AEB activations. The scope has been expanded to include assessment of model year 2020-2022 Honda CR-V and Accord vehicles. To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.

OUR BRAND NEW 2019 HONDA ACCORD, WITH ABOUT 1,000 MILES ON IT, WAS INVOLVED IN A FRONT END MOTOR VEHICLE COLLISION ON OCTOBER 13, 2019 AT 1:24 PM ON A CITY STREET WITH A SPEED LIMIT OF 45 MPH. WE STRUCK A 2001 F250 AT 45 MPH IN THE SIDE AFTER THE INDIVIDUAL FAILED TO YIELD AND STOPPED SIDEWAYS DIRECTLY IN OUR LANE OF TRAFFIC. MY HONDA RECEIVED SERIOUS FRONT END DAMAGE AND THE ENGINE COMPARTMENT WAS SMOKING, BUT NOT A SINGLE AIR BAG DEPLOYED DURING THIS ACCIDENT. THERE WERE THREE (3) ADULTS AND ONE (1) CHILD IN THE HONDA, THE CHILD AND TWO (2) ADULTS WERE SENT TO THE EMERGENCY ROOM VIA AMBULANCE. THERE WERE NO MAJOR INJURIES OTHER THAN SEVERE BRUISING AND SOME PULLED MUSCLES WHICH WERE

While driving my 2019 Honda Accord at approximately 65 mph on the highway, the car suddenly lost power, went into limp mode, and the steering wheel locked up. I was unable to turn or control the vehicle, which resulted in another car crashing into me. I sustained injuries in the accident and required medical attention. The incident was extremely dangerous and could have been fatal. After being towed to Russell & Smith Honda, they confirmed a blown head gasket was the cause of the failure. This issue is already the subject of a class-action lawsuit due to known design flaws in the 1.5T engine. Honda only offered partial assistance, leaving me responsible for thousands in repair costs. ENGINE
Coolant is leaking into my cylinders due to the head gasket design flaw. Engine now needs to be replaced and it’s over $6,000. My car has about 150k miles. About 18 months ago it started misfiring, even while on the highway multiple times with my children. Codes came back saying i needed to replace ignition coils and spark plugs. Did that over and over and over. Turns out this is an on going issue with this specific year and model. I bought this car with 4 miles on it. I’m left without a car. This need to be recalled. I have read over 4,000 stories of others having the same issue.

ENGINE
While my vehicle was in transit, it experienced a sudden and severe loss of engine power. Simultaneously, multiple malfunction indicator lights (MILs) illuminated on the dashboard, and my vehicle abruptly engaged "limp mode." This drastic restriction of speed and maneuverability while in active traffic created an immediate and severe safety hazard, which nearly resulted in a catastrophic collision. Following the incident, a formal complaint was initiated with Honda customer relations. My own independent research clearly indicates that premature head gasket failure is a well-documented, systemic defect within these specific vehicle models; therefore, a constructive resolution was

I own a 2019 Honda Accord Sport 1.5T that has an ongoing, unresolved engine/powertrain problem causing severe loss of acceleration, jerking, and a flashing check-engine light. The exact component causing the failure is UNKNOWN. The vehicle is available for inspection upon request. I initially took the vehicle to an authorized Honda dealership and paid over $300 for diagnosis. Honda diagnosed the fuel injectors. I then paid approximately $800 at an independent shop to replace the fuel injectors, but the same problem continued. The vehicle was later evaluated by other repair providers. I was advised that the problem could involve the turbocharger wastegate actuator and, separately, the spark

A complaint is one owner's account and has not been verified by NHTSA or by us. A vehicle with more complaints is not necessarily worse: models that sold in larger numbers collect more of them. How this is counted.
