Defect investigations (1)

An investigation means NHTSA has decided a pattern is worth examining. A preliminary evaluation is often closed with no action; an engineering analysis is the stage that usually precedes a recall.

Front Seat Belt Assemblies may be Missing the Rivet that Secures the Quick Connector and Wire Plate

RQ24006

Closed recall query · opened April 4, 2024 · closed March 26, 2025

the seat belts

On April 4, 2024, the Office of Defects Investigations (ODI) opened RQ24006 to investigate American Honda Motor Co.’s (Honda’s) proposed remedy for recall 23V-782 as well as scope of the vehicles involved. Honda issued recall 23V-782 on November 21, 2023, which concerns certain model year (MY) 2023-2024 Honda Accord, Honda Accord Hybrid, and Honda HR-V vehicles. Recall 23V-782 states that the driver and front passenger seat belt pretensioner assemblies may have been manufactured without a rivet that secures the pretensioner quick connector and wire plate. Recall documentation states that seat belt pretensioner assemblies that are missing the rivet will not properly restrain the occupant in the event of a collision, increasing the risk of injury.Subject defective seat belt assemblies are installed as original equipment for both the driver and front passenger positions for the HR-V, but only for the driver position for the Accord and Accord Hybrid. The recall remedy for 23V-782 instructs the technician to inspect both front seat belt pretensioner assemblies and, if necessary, replace them. The inspection procedure requires the technician to utilize a pull force gauge to determine whether the subject component is missing the subject rivet and requires replacement. Parts requiring replacement are determined by whether the pretensioner quick connector separates from the pretensioner assembly while the technician is pulling on the seat belt. If no separation occurs, then the part is determined to be not defective and not require replacement. There have been no reports alleging a failure of seat belt pretensioner assemblies after the inspect and replace remedy for recall 23V-782 was performed. Additionally, no comparable failures have been observed in field data for similar vehicles outside of the scope of the recall. As of the 4th quarter of 2024, the recall completion rate is at approximately 84% of the total recalled population. In view of the lack of failures after the remedy for recall 23V-782 was performed, and the lack of failures in similar vehicles outside of the scope of the recall, ODI is closing this Recall Query (RQ). The Agency reserves the right to take additional action if warranted by future circumstances.

AIR BAGS

1 injured · crash · towed

I got in a car accident where the front of the car was hit and the car is totaled and the airbags in the front did not deploy. I got injured because my face hit the steering wheel. I submitted a complaint to Honda but they are taking a very long time to have someone go out and investigate the airbags. My insurance needs to take the car away this week

STEERING

1 injured · crash · towed

Before the collision, while turning left from a complete stop, the steering wheel suddenly became extremely stiff and would not turn normally. There were no dashboard warning lights (no EPS, brake, or hybrid warnings). The steering problem occurred before impact and contributed to the head-on collision.

SERVICE BRAKES

SERVICE BRAKES

This is a second formal supplemental complaint. Vann York Honda sold this car as new, but a Honda America provided Carfax proved it was a prior personal use vehicle. The car has catastrophic phantom braking, causing a physical friction rash on my skin. Multiple witnesses are now afraid to ride in the car. The vehicle has suspension failures documented by irregular tire readings. Despite an active federal recall, Flow Honda refused mandated repairs. During a prior headliner repair, Flow Honda returned the car with a collapsed headliner and a Maintenance Minder A127 code. The oil life dropped from 45% to 5% without added mileage, yet I was told to drive it. Weatherstripping has been replaced.

STEERING
Sticky steering at center position" Notchy/Clicking feeling when turning past center point"。("A noticeable catch and clicking noise when steering wheel passes through the center position")。"This failure causes inconsistent steering effort and poses a potential safety risk, especially for high-mileage commercial drivers like Uber."

TIRES

The contact's fiancée owns a 2023 Honda Accord. The contact stated that the rear tires showed severe inner wear and damage due to a misalignment, requiring three sets of rear tires to be replaced within a year. The vehicle was taken to a dealer, who notified the manufacturer of the failure; however, the technical engineer denied assistance. The dealer informed the contact that the lower control arms needed to be replaced. The vehicle was not repaired. Additionally, the contact stated that the tire repair kit bottle cap detached while inflating a tire. The contact received notification of NHTSA Campaign Number: 26V366000 (Tires); which was associated with the tire repair kit failure. The

STEERING
I own a 2023 Honda Accord Touring Hybrid. At approximately 42,000 miles, all of the warning lights illuminated while driving. The warnings included Adaptive Cruise Control, Forward Collision Warning, Lane Departure Warning, Blind Spot Warning, Parking Collision Warning, and multiple other system alerts. At the same time, I experienced a loss of electric power steering assist, increased steering resistance, and clicking/popping noises coming from the steering rack. The Honda dealer diagnosed the vehicle with a failed Electric Power Steering (EPS) steering rack and quoted nearly $5,000 for the repair. A critical steering component should not fail at such low mileage. This is a serious safety

ENGINE
While driving my 2023 Honda Accord Hybrid, the vehicle suddenly displayed multiple warning messages, including a Check Engine Light, and immediately entered limp mode. Engine power was significantly reduced, making it difficult to accelerate and safely keep up with surrounding traffic. This unexpected loss of power created a potential safety hazard because the vehicle could not respond normally to throttle input. The vehicle was diagnosed by an authorized Honda dealership. The dealer confirmed Diagnostic Trouble Code (DTC) P2101 and determined that the throttle body assembly had failed. The throttle body assembly and gasket were replaced, and a software update was also performed before the

FORWARD COLLISION AVOIDANCE: WARNINGS
The car has continually asked to to be updated through the driver control center screen. IT states that this is a dealer update and must be brought in to have this done. Until this update is completed the above systems will not work. I have brought the car in for the lat 25k miles 4 oil changes and have been told each time that it has been fixed. It still has not been fixed, and the dealer is telling me there is nothing they can do. This is definitely a safety issue as these important systems are not working.

Where these numbers come from

A complaint is one owner's account and has not been verified by NHTSA or by us. A vehicle with more complaints is not necessarily worse: models that sold in larger numbers collect more of them. How this is counted.
