Defect investigations (1)

An investigation means NHTSA has decided a pattern is worth examining. A preliminary evaluation is often closed with no action; an engineering analysis is the stage that usually precedes a recall.

Rear Differential Seizure

PE22011

Closed preliminary evaluation · opened November 28, 2022 · closed July 22, 2024

the power train

On November 29, 2022, the Office of Defects Investigation (ODI) opened PE 22-011 after reviewing field reports alleging rear differential seizure in 2018-2022MY Honda CR-V and HR-V vehicles equipped with all wheel drive. To date, ODI has not received any consumer complaints. Some of the field reports received alleged the vehicle’s wheels locked up with no warning while the vehicle was in motion. These vehicles were towed to a dealership. Examination of these vehicles showed the driveshaft had broken and the differential unit was completely damaged. The field reports cited a lack of oil due to damaged oil seals as the cause. Honda provided data including consumer complaints, field reports, and warranty claims they received relating to the loss of oil retention and resulting differential damage. Honda’s data cited 1201 incidents with unique vehicle identification numbers (VINs). The rear differential oil seals  prevent the oil from leaking out and protect internal components from contaminants. A loss of oil lubrication results in failure of the vehicle differential.  Honda stated that the oil seal’s lip contact with the driveshaft may become unstable due to intermittent changes in internal pressure. Their review of field report and warranty data indicated that these failures occurred early in the vehicle’s life thus the failures were covered by the manufacturer’s warranty. Honda stated that in November 2021 they changed the seal specifications and improved the packaging process of the parts for transport.  Honda also enacted an enhanced visual inspection process both before and after the oil seal installation on the assembly line. Honda stated that in the event of differential failure, the operator can control the vehicle without stability loss. After the drive shafts break due to differential failure, the operator will hear noises and the vehicle will still function as a 2WD vehicle preventing complete loss of motive power. Based on the following factors, ODI is closing this investigation: ·       Information regarding field failures does not indicate hazardous outcomes ·       The failures are occurring early in the life of the vehicle ·       Repairs are covered under the manufacturer’s warranty ·       The vehicle can continue functioning as a 2WD without loss of stability control in the event of failure The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist, and the agency reserves the right to take additional action if warranted by new circumstances.

The contact owned a 2019 Honda HR-V. The contact stated while driving 70 mph on cruise control, the contact heard a weird noise and the vehicle decelerated to 0 mph without warning. The vehicle lost power and the contact could not restart the vehicle while sitting in the middle lane of the highway. The check engine and several other warning lights illuminated. While stuck in the middle lane, the contact was rear-ended on the right side by an 18-wheeler and tossed to the left side of the highway crashing into a barricade on the left front side of the vehicle. The air bags failed to deploy. The contact sustained injuries to the collar bone, left leg, and upper front teeth and had a blood

I was parallel parking and when I put the car in reverse and barely touched the gas it suddenly accelerated into the car behind me. It was so fast it shot backwards like a rock into the car parked behind me. This was not driver error. It is now with an independent mechanic so I do not know at this point if they can reproduce the incident. I did not take it to Honda as I wanted an independent an unbiased report on the car. No warning lights, etc. before hand. The car behind me had somebody sitting in it and he hurt his hand. BUT if there had not been a car parked behind me I would have shot out backwards into a very busy street. Or had somebody been walking behind the car the would have been

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
The 2019 Honda HRV with low miles of around 44,000 miles experienced a series of warning alerts on the dash board consisting of the following list of five problems: Road Departure Mitigation System Problem; Auto High-Beam Problem; Adaptive Cruise Control Problem ACC; Lane Keeping Assist Problem LKAS; and Collision Mitigation System Problem. This started with warm weather at the end of July 2026 and is continuing intermittently in August 2026 as of the date of this write up. I described this to a Honda service person verbally in person in Huntington Beach, CA and they said it was a Multipurpose Camera problem and could fix it for $1,971.23 plus tax. They indicated that the camera was

ELECTRICAL SYSTEM
The Multipurpose camera has internal failure and the unit is running at 130 degrees per diagnostic tests per mechanic. It's affecting the lane keep assist, automatic high beams, road departure, mitigation, adaptive, cruise control, collision mitigation system. These safety features are not working.

ENGINE
My 2019 HR-V transmission is suffering from the premature CVT belt deterioration outlined in Honda's internal service bulletins TSB 21-047. My car has 99,000 miles and Honda refuses to replace the faulty transmission. The intermittent loss of acceleration poses an immediate safety hazard when merging onto highways or navigating heavy traffic.

STEERING
The contact owns a 2019 Honda HR-V. The contact stated that while inspecting the vehicle for an Emissions test. The contact stated that it was determined that the vehicle had failed the inspection due to a steering box failure. The contact stated that the failure of the part occurred every two years, and the current failure was the third occurrence. The dealer was contacted; however, the vehicle was not diagnosed or repaired. The manufacturer was contacted and referred the contact to the NHTSA Hotline to report the failure. The failure mileage was unknown.

ENGINE
The transmission belt deteriorated. I had noticed my gears slightly delayed over past couple months. On July 10th, i was on freeway during morning commute traffic and the car lost all power. All the dash warning lights came on and I luckily got to side of road. I towed to dealer and they confirmed that it was premature belt deterioration. I sent a claim to honda and they agreed that was the issue but they would not honor any warranty against it. I was never informed by either Honda America, nor my dealership that there could be any issue with my transmission. Apparently, honda issued bullitin 21-047 knowing that an issue exists but unfortunately my car is 2mos over in time.

NHTSA complaint 11752667. Filed by an owner and not verified by NHTSA or by us.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
Honda sensing error codes started lighting up all of a sudden. My HRV has never been in any kind of minor or major accident. We bought it new back in 2019. It currently has 13424 miles. This error is unexplainable as we have maintained this car so thoroughly. This error is not safe for drivers as it causes confusion especially when it just suddenly pops out while driving.

Where these numbers come from

A complaint is one owner's account and has not been verified by NHTSA or by us. A vehicle with more complaints is not necessarily worse: models that sold in larger numbers collect more of them. How this is counted.
