Defect investigations (1)

An investigation means NHTSA has decided a pattern is worth examining. A preliminary evaluation is often closed with no action; an engineering analysis is the stage that usually precedes a recall.

back-over prevention

On July 22, 2022, the Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE22-006) to assess alleged failures of the rear-view camera (RVC) in Model Year (MY) 2020-2021 Cadillac XT5, XT6, and GMC Acadia vehicles. The complaints report that the screen for the RVC is black, sometimes with a red triangle and a circle with a line through it. To date, ODI has received a total of 18 complaints and several EWR Field Reports with allegations of the RVC image failing to display when the vehicle is in reverse. The reports have been received over the past 28 months with most occurring with low vehicle mileage.During the investigation, ODI learned incorrect crimping specifications were utilized in the production of the coaxial cable connector. Improper crimping leads to loss of connection between the RVC and dashboard screen that results in failure of the RVC image to display. Failure or malfunction of the RVC image to display results in reduced visibility of the area behind the vehicle. This limited view may lead to an increased risk of incidents resulting in injury and/or property damage while the vehicle is in reverse. The improperly crimped connectors were only present on subject vehicles equipped with the optional RVC surround vision feature (UV2). Vehicles not equipped with the optional RVC surround vision feature were not affected by this failure. On September 28, 2022, General Motors (GM) filed a defect information report (NHTSA number 22V-709) recalling certain MY2020-2021 Cadillac XT5, XT6, and GMC Acadia vehicles equipped with the optional RVC surround vision feature (UV2). GM will replace the coaxial cables with properly crimped connectors. In view of the recall action being taken by GM, ODI is closing this preliminary evaluation. NHTSA reserves the right to take additional action if warranted by future circumstances. The ODI complaints cited above can be viewed at www.nhtsa.gov. FUEL/PROPULSION SYSTEM
Your site shows no recalls. I got a recall for a repair I had to have done a few months ago. I just got the recall notice from GM, but when I called to inquire about a reimbursement, they state the recall was released in 10/2025. Even though your site says there was no recall at the time.

POWER TRAIN
Cadillac XT6 – VIN: [XXX] echanical Failures, Ignored Transmission Recall(s), and Dealer Negligence I write this letter to formally place General Motors Company on notice of a pattern of serious mechanical failures, ignored safety recalls, and dealership indifference involving my Cadillac XT6, referenced above. This vehicle has been presented to Sam Leman Cadillac of Champaign on multiple occasions for mechanical issues, including—but not limited to—cooling system failures, thermostat defects, persistent check-engine warnings, and abnormal vehicle operation. Service records confirm repeated interventions, temporary fixes, and diagnostic acknowledgments without meaningful long-term

The contact owns a 2021 Cadillac XT6. The contact stated that while sitting inside the vehicle, the vehicle felt unbalanced. Upon inspection, the contact noticed that the H-pipe was leaning more to the right side. The H-pipe was not centered as intended. The vehicle was not taken to the dealer or an independent mechanic. The vehicle was not repaired. The contact stated that in the past, while the vehicle was serviced, no one had noticed that the H-pipe was off-center. The manufacturer was not notified of the failure. The failure mileage was approximately 37,000.

ELECTRICAL SYSTEM
The vehicle was previously recalled for an intermittent backup camera failure. Prior to me purchasing the vehicle, the recall was completed and documented as repaired. I went in to the Myrtle Beach Cadillac dealership for reoccurring issues with the camera failing exactly as disclosed in the recall. It will randomly cut to a black screen with a camera icon crossed out in red. It is a true safety issue that leaves my family as risk of a collision or injuring someone else.It cannot be recreated on request as it is happening intermittently. The dealership refused to inspect the safety recall repair for proper operation and only stated that the recall repair was completed.

UNKNOWN OR OTHER
[XXX] My 2021 XT6 occasionally stumbles when accelerating from a stop, both when the engine is running or when it has auto stopped, only occasionally. If the car won't accelerate properly, it can cause a collision, especially if making a left hand turn at an intersection. It started happening about June 2025 and kept reoccurring more frequently as time passed. It was brought to a dealer early August, they said that it might be a bad battery, I installed a new battery on August 19, 2025, it was ok until late November 2025, then it started happening again. Yes, it can be inspected upon request. There were no warning lamps or messages, symptoms or problems prior to the failure. This has been

Where these numbers come from

A complaint is one owner's account and has not been verified by NHTSA or by us. A vehicle with more complaints is not necessarily worse: models that sold in larger numbers collect more of them. How this is counted.
