Defect investigations (1)

An investigation means NHTSA has decided a pattern is worth examining. A preliminary evaluation is often closed with no action; an engineering analysis is the stage that usually precedes a recall.

No Restart After Auto Start/Stop Engages

the engine and its cooling

On June 3, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE22005 after the agency received VOQs and several field reports concerning the Auto Idle Stop (AIS) feature on 2016-2019MY Honda Pilot vehicles. The complaints allege that the engine fails to restart on its own from a complete stop at a traffic light or road intersection with the AIS function engaged. The subject vehicles are equipped with a 3.5L engine with a 9-speed automatic transmission. On January 6, 2023, Honda released service bulletins 23-008 and 23-009. These service bulletins address the failure to restart issue in the 2016-2019MY Honda Pilot vehicles, as well as the 2015-2020 Acura TLX, 2016-2020 Acura MDX, 2019-2022 Honda Passport, and 2020-2023 Honda Ridgeline vehicles. The repair outlined in the service bulletins is a two-stage countermeasure to remedy the issue. The first stage is a Programmed Fuel Injection (“PGM-FI”) software update. The second stage is only deployed if the software update fails to remedy the issue. The second stage, depending on the model and model year, consists of replacement of the starter assembly, starter relays, and a valve adjustment. Additionally, Honda extended the warranty coverage to 10 years with unlimited mileage for vehicles requiring the second stage component replacement. ODI continues to receive complaints of the AIS failure to restart, with many complainants alleging the campaign countermeasures had been completed.NHTSA has reviewed Honda's Information Request Letter responses involving warranty claims, lawsuits, and consumer complaints in addition to ODI's own data. This Engineering Analysis is being opened to further consider the potential safety defect, including gathering additional data regarding Honda’s service campaign efficacy. ODI is additionally expanding the scope to encompass all vehicles addressed in Honda’s service bulletins 23-008 and 23-009 as well as newer model year vehicles, which are substantially similar to the vehicles covered by the service campaign. To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.

On October 18, 2023, at about 11 am, I was approaching to enter our driveway with my foot on the brake system when all of the sudden the car sped up at a very high speed that I was unable to control. The speed was so forceful that the car crashed unto the house resulting in a lot of damage to our house, all this happened in less than one second. I had a next door neighbor in the car with me and thankfully neither of us was hurt, I like to mention at this point that neither air bag deployed to issue protection to us. Of course the safety of both of us was put at risk. We called the insurance with no luck to send an adjuster to inspect the car and/or damage done to the house, so we had to do

The contact owns a 2021 Honda Pilot. The contact stated that while the engine was idling and with the gear shifter in the park position, the contact reached into the vehicle from the front driver's door, and suddenly the vehicle began to slowly roll backwards. The contact was pushed to the ground, causing the front driver's side wheel to roll over the contact's right leg. The vehicle continued to roll backwards before making a U-turn and then rolling forward down the inclined street and increasing speed. To avoid having the vehicle roll down hill into a crowded street with pedestrians, the front passenger inside the vehicle turned the steering wheel, causing the vehicle to crash into the

ENGINE
I am reporting a catastrophic engine failure involving my 2021 Honda Pilot equipped with the 3.5L V6 engine. The vehicle has approximately 77,000 miles. The vehicle developed an engine rattling noise followed by complete engine failure and loss of motive power. I had the vehicle transported to an authorized Honda dealership for diagnosis. Following inspection and engine teardown, the Honda dealer documented a hole in the front side of the short block, a large amount of metal debris in the oil pan, and the cylinder #3 connecting rod broken in half. The dealer concluded that the engine experienced catastrophic internal failure with widespread metal contamination throughout the lower and upper

#1 connecting rod bearing failure at 89,000 miles, all oil changes were done timely and properly. Car is very well maintained. While engine is running it has a very loud rod knock coming from bottom of engine. I did some research and it’s the same symptoms and issues as (NHTSA Campaign 23V751) 2016-2020 Honda pilot recall.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
My 2021 Honda Pilot displayed a U3000 diagnostic trouble code, indicating an internal electronic failure of the front-facing multipurpose camera that operates the vehicle's Honda Sensing safety systems (Collision Mitigation Braking System, Lane Keeping Assist System, Road Departure Mitigation, and Adaptive Cruise Control). As a result, these safety systems became inoperative. I took the vehicle to an authorized Honda dealer, who confirmed the U3000 code and quoted approximately $2,000 to replace the camera unit. I have since learned this is a widely reported failure across multiple Honda and Acura models (Pilot, Odyssey, CR-V, Accord, Civic, HR-V, Clarity, RDX, MDX) and is the subject of a

ENGINE
My 2021 Honda Pilot Touring intermittently fails to restart after the Auto Idle Stop system shuts off the engine at a stop. This occurs about 10% of the time. When I release the brake, the engine does not restart, creating a risk of being stranded in traffic. I must shift to Park and press the Start button to restart it. No warning lights or messages were observed. The problem has not yet been inspected by a dealer. The vehicle is available for inspection. Scheduled appointment at Honda service.

Ive had about 5 times my car Honda pilot 2021 stall at a stop light, a stop sign. The first it took a minute to go and the light was green. I was honked at and yelled at. It finally kicked on and we went, the next time it went into accessory mode at a stop sign, then into N then off…. All while in drive to start. The rest were times I was in auto start stop and it just wouldn’t start. Each 5 times it gave an error code that disappeared right after I had to try snd restart by pushing the button. I went to Honda 2 times now, first time they said they did the program… that didn’t stop the issue, I then took it again after the 4th time and they said it was a bad battery… replaced it mind you
