BACK OVER PREVENTION:SOFTWARE

The defect
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2025 Lexus NX350, NX250, 2023-2026 RX350, and 2024-2026 TX350 vehicles. The rearview camera image may not display when the vehicle is placed in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
The risk
A rearview image that does not display reduces the driver's view behind the vehicle, increasing the risk of a crash.
The remedy
Dealers will update the rearview camera software or replace the rearview camera, as necessary, free of charge. Owner notification letters were mailed May 15, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 26LB02 and 26LA02.

The defect
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
The risk
A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.
The remedy
Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.

NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

ELECTRICAL SYSTEM

My car was hit from the back by another vehicle. After the collision, I parked the vehicle and turned the ignition switch off, before getting out of the vehicle, the doors automatically get locked and the vehicle does not allowed to open the door even with the key card inside. I had to open the car with the manual handle from the inside. After I went out of the car, immediately the car again was automatically locked with the key card inside. I tried to open the car several times, and even with the phone application, however its remains locked. The car never opened, so I have to use an external hard ket to get access to the inside. I am very concerned about the lock system of the vehicle

[XXX] I was Braking while pulling into my Carport almost at the Rear curb, when Suddenly and without Any Warning, my 2022 NX 450h Accelerated Rapidly and sent Me up a short Hill and Crashed into a Large Tree While I still had my foot on the Brake. Then it kept Accelerating and the Front tires Dug a 4in Deep Hole while they were Spinning still with My foot on the brake! I was able to get the car into Reverse and backed it down the hill and turned it off. It wasn't Drivable due to the Extensive Front Damage, so I had it Towed it to my Local Roseville CA Lexus Dealership service and they Told Me to "Get The [XXX] out of Here". Then I had it Towed to a Repair Shop recommended by Lexus.

SEATS

2022 Lexus NX driver's seat is crooked, confirmed by Lexus dealership and Lexus field rep. The left side of the seat is back further while the right side of the seat is closer to dashboard. Lexus has confirmed that the seat is properly attached and bolted to the seat rails. An identical seat from an unaffected 2022 Lexus NX was placed in my vehicle and did not fix the crooked orientation; the replacement seat sat crooked as well. The vehicle was delivered brand new to me from Lexus with this defect. This defect may affect the crash worthiness of vehicle, seat strength, and the driver's body in the event of a car crash, as well as how the seatbelt fits the driver due to crooked orientation

LANE DEPARTURE: BLIND SPOT DETECTION

The contact owns a 2022 Lexus NX 350. The contact stated that upon starting the vehicle, the blind spot cameras failed to properly operate. The contact stated that the Mobile App was used to activate the blind spot monitoring system while driving manually. The dealer was notified of the failure; however, the vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was 47,748.

The sunroof exploded while I was driving on the highway, traveling at approximately 70 mph. There was frost on the car & I had the defroster up high when initially getting into the car. However, by the time I got on the highway, it was turned down to the 2nd level. Prior to this occurring, I did not hear any noise or anything that would cause me to think there was something wrong with the sunroof. The explosion caused me to duck down immediately, but there were no other vehicles around me. The vehicle has been inspected and repaired. I filed a claim with my Insurance company & had the repairs completed at a local body shop. However, I do believe they took the vehicle to Lexus to be repaired.

The transmission (Torque Converter) is shuddering at a certain RPM range when it is in 3rd gear (in between 1000-2000 RPM) in my case. (Roughly 15-25 mph) This could lead to an uneven acceleration and cause an accident. Especially in a bad traction situation ex)snow, rain, uneven surface and etc. Also, it could cause and accident due to unable to accelerate in time. The vehicle was inspected and dealership were saying it was the tire and balancing issue, but not fixed. There is no warning light associated with.
