Defect investigations (1)

An investigation means NHTSA has decided a pattern is worth examining. A preliminary evaluation is often closed with no action; an engineering analysis is the stage that usually precedes a recall.

Complete loss of motive power due to engine failure

PE23023

Closed preliminary evaluation · opened December 13, 2023 · closed July 18, 2025

the engine, the engine and its cooling

On December 13, 2023, the Office of Defects Investigation (ODI) opened PE23-023 to investigate allegations of engine failures leading to loss of motive power in model year (MY) 2021-2023 Nissan Rogue; MY 2019-2021 Nissan Altima; and MY 2019-2021 Infiniti QX50 vehicles. At that time, ODI had received 6 Vehicle Owner Questionnaires (VOQ) and multiple early warning reports, with consumers reporting engine knocking noises accompanied by a loss of engine power while driving. The investigation identified bearing failures in certain vehicles equipped with the subject 3-cylinder 1.5L or 4-cylinder 2.0L variable compression turbo (VC-Turbo) engines. ODI also identified early warning reports describing thermal incidents that occurred immediately after the vehicle suffered a catastrophic engine failure.  The failures were the result of a manufacturing defect in specific engine bearings (main, A-, C-, and L-link) or other engine components that may cause engine damage and potentially lead to engine failure. Engine failure can result in a loss of motive power event that can increase the risk of a crash.  Engine failure might also result in a fire if internal components detach and impact the engine block creating a hole that allows engine oil to reach hot surfaces. In its April 2024, May 2024, and June 2025 responses to ODI’s information requests, Nissan provided 998 customer reports (including complaints, field reports, and legal claims) and 1,107 warranty claims related to the alleged defect.  When combining ODI and manufacturer report data, there are 1,878 unique VINs that have experienced failures across all subject vehicles.  In one of the early warning reports, ODI identified an incident involving a 2022 Infiniti QX55 that resulted in an engine fire following an alleged engine failure. The 2022 Infiniti QX55 was not included in the scope of this investigation but the vehicle model shares the same 4-cylinder 2.0L variable compression turbo engine.  Based on the information included with the early warning report, the fire can be directly related to the alleged defect.   A further review of ODI and manufacturer data, identified 12 allegations of engine compartment fires in subject vehicles; three were confirmed as directly related to the engine failure, and nine reports described an under-hood fire that occurred following a loss of power event. Additionally, ODI identified multiple reports describing an engine block crack or hole surrounded by oil residue following an engine failure. On June 27, 2025, Nissan submitted recall 25V-437 involving 443,899 MY 2021-2023 Nissan Rogue, MY 2019-2021 Nissan Altima; MY 2019-2021 Infiniti QX50; and 2022 Infiniti QX55 vehicles. The recall remedy provides the dealer instructions to inspect the engine oil pan for the presence of specific metal debris. For customers with the 3-cylinder 1.5L VC-Turbo engine, if no debris is detected during the inspection, dealers will replace the oil pan gasket, engine oil, and reprogram the vehicle’s Engine Control Module (ECM). For customers with the 4-cylinder 2.0L VC-Turbo engine, if no debris is detected, dealers will replace the engine oil. In the case where specific debris is detected and confirmed by Nissan Powertrain Call Center, dealers will be instructed to replace the engine. In addition, Nissan will extend the vehicle warranty to 10 years and 120,000 miles for the recall subject vehicle population after the dealer performs the inspection. Nissan has implemented multiple corrective actions to address the identified manufacturing defects of certain engine block internal components.  In view of the recall action (25V437) taken by Nissan, ODI is closing this Preliminary Evaluation. The agency reserves the right to take additional action if warranted by future circumstances. ENGINE

fire

I was driving when I was pulled over by a police officer for speeding I was going 5 over the speed limit (Speed limit was 65). As the officer approached my vehicle, he noticed flames coming from the engine compartment before I was aware there was any issue. He immediately instructed me to exit the vehicle for my safety. After I got out, the officer attempted to put out the fire with a fire extinguisher, but the flames continued to spread despite his efforts. The fire quickly intensified, causing extensive damage to the vehicle. Emergency responders were called to the scene to extinguish the fire, but by the time the fire was under control, the vehicle had sustained severe fire damage and

VEHICLE SPEED CONTROL

crash · towed

I took my Rogue to the dealership at City Limits Nissan on Monday 10/6/25 as it had power issues and the engine light came on stating 'Engine Malfunction Power reduce Service now' I then called NHTSA and filed a complaint about a recall for my Nissan Rogue that Nissan has yet to have a remedy. I got the car back on Tuesday afternoon 10/7/25 and they said it did not have to do with the recall. I drove the car straight home. I went to work the next morning and as I was pulling into a parking spot in parking garage, all of the sudden, I was crashed into the wall at a high rate of speed. I am extremely concerned that this may have been a malfunction with the Nissan Rogue as this occurred less

SERVICE BRAKES

crash

I was caught in a snowstorm, driving about 5 MPH. Suddenly, the vehicle swerved to the left and I lost complete control; the brakes locked up. The vehicle spun about 145° degrees before coming to a stop perpendicular to the road; subsequently rear-ended from behind. The moment was surreal; I’ve never felt closer to a life-threatening incident because I had ZERO CONTROL over the car: I was spared ONLY because there was no oncoming traffic. My biggest concern is why the safety systems failed to intervene. During the incident, the ABS did NOTHING. Traction Control, NOTHING. NO warning lights came on. It’s mind blowing that the car skidded about 145° degrees without any intervention from the

ENGINE
Engine failure, known issue per Nissan service employees, second and third combustion cylinders fill with anti-freeze coolant with no errors or warning lights in vehicle. This was noticed by the temperature gauge climbing during acceleration and falling while at speed. I am told it is a manufacturing defect by the service department but due to the 76,000 miles on the 2022 vehicle it is not covered by Nissan, an $8000 bill for the long block replacement. This needs your attention as in my local dealership there have been 5 instances of this in the last month. Thank you, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

ENGINE
Driving down the highway on a family road trip the car suddenly flashed all warning lights and goes in to limp mode. We’re on the highway surrounded by cars going 80+ and trying to get to the side. We’re reach an exit and limp to an autozone where they read a code about an EGR system. Car gets towed to the dealer and they’ve determined there is coolant in the oil and the engine needs replaced. There is a recall for engine failure but they say this isn’t related. They can’t say what caused the failure but assured me it wasn’t the engine bearing recall.

ENGINE
I purchased the vehicle about 4 months ago and I've only driven it maybe 1 month out of 4 due to being in the shop for multiple egr and throttle issues. I am currently sitting with it broke down once again for the same issues for the 3rd time. The engine loses power out of no where while driven and gives a warning that says engine malfunction power reduced and all safety features are then shut down. This leaves my car only traveling no more than 10mph. I was on the interstate!!

ENGINE
I was driving to work. I noticed what I thought was some gravel dust behind me. I got to a stop light and my car started shaking. No engine or warning lights came on. I shut my car off and started it up again. It was still shaking. When I pulled away from the stop light, I notice there was white smoke coming out my tailpipe. I immediately pulled over and called the dealership I thought it from and a tow truck. I haven’t been given anything official from the dealership, but the service department emailed me this yesterday: “Car was smoking from exhaust, pressure tested coolant system did not hold pressure, inspected cylinders with boar scope found coolant in cylinder 1, coolant saturation
