The defect
Toyota Engineering & Manufacturing (Toyota) is recalling certain 2022 Lexus LX 600, Lexus NX 350h, Lexus NX 450h+, Toytoa RAV4 Hybrid, 2021-2022 Lexus LS 500h, Toyota Mirai, RAV4 PRIME, Sienna, Venza, and 2020-2022 Toyota Highlander Hybrid vehicles. The Skid Control ECU software may not turn on the Vehicle Stability Control (VSC) system when the vehicle is restarted after the driver has disabled the VSC system. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 126, "Electronic Stability Control Systems."
The risk
Driving with a deactivated stability control system increases the risk of a crash.
The remedy
Dealers will update the Skid Control ECU software, free of charge. Owner notification letters were mailed May 27, 2022. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is 22TA03. Lexus' number for this recall is 22LA01.

What component or system failed or malfunctioned, and is it available for inspection upon request? Car suppposed to stop itself as it has all sensors. Still car rear ended other car. How was your safety or the safety of others put at risk? Jeopardy. Since accident occured in residential area my family was okay. What if it was in Free way? Has the problem been reproduced or confirmed by a dealer or independent service center? Not yet, they are not providing me alternate transportation during inspection so not ready yet from my side. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Vehicle was inspected by Insurance as normal

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

- Emergency braking system did not function, nor was there any displayed on the dashboard (as it usually does) on 2022 Mirai with about 17500 miles and under 18 months since purchase - Vehicle is available for inspection - Car resulted in an accident - my car hit the car in front (rear ended the front car) - Manufacturer (Toyota) took the car for inspection and reported "Based on our findings, we are unable to offer any assistance at this time." - No messages were displayed

The accelerator pedal was depressed, and the vehicle continued to decelerate. The contact stated that while driving approximately 15-20 MPH, several warning lights were illuminated, and the stability control symbol was displayed, and a yellow flashing warning light was illuminated. The contact owns a 2022 Toyota Mirai. The contact stated that while driving at various speeds, the vehicle hesitated while the accelerator pedal was depressed and maintained automotive power. The "Parking Support Brake - PSKB" message was displayed. The vehicle was towed to a local dealer; however, the vehicle was not diagnosed or repaired since the extended warranty had expired, and the vehicle had a Salvage Title. The vehicle was taken to another local dealer, where it remained for further investigation. The manufacturer was made aware of the failure. The failure mileage was approximately 48,000.

ELECTRICAL SYSTEM
1) Electrical display goes off on its own. While car in motion and driving,Distract me while I’m driving. 2) car does not start. AAA came to jumpstart, but no luck main battery under the car not responding defective battery. 3) fuel system not working properly cannot fill up the car all the way with hydrogen, it’s giving me 180 miles to maximum of 200 miles to drive ,Car slowed down on it’s own in the freeway lose power very unsafe and unreliable vehicle. 4) Blindspot, not working all the time as I am backing out it does not see other cars or pedestrian coming from left or right side or sometimes stop on it’s own with no car or pedestrian on either side. ELECTRICAL SYSTEM
The battery constantly fails and the hydrogen system will suddenly shutoff bringing the vehicle to an abrupt stop while driving or in traffic. The steering and braking system stops working when this happens.
