The defect
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2024 Toyota Tundra, Lexus LX, and 2024 Lexus GX vehicles equipped with a V35A engine. Debris from the manufacturing process may contaminate the engine and cause the main bearings to fail, which can result in an engine stall and loss of drive power.
The risk
A loss of drive power increases the risk of a crash.
The remedy
Dealers will perform an inspection on the #1 main bearing using vehicle inspection software and drive data, and if necessary, replace the engine. Repairs will be performed free of charge. Interim letters notifying owners of the safety risk were mailed December 16, 2025. Additional letters will be sent once the final remedy is available. The defect
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2025 Tundra, Tundra Hybrid, and 2023-2025 Sequoia Hybrid vehicles. A software error may cause the rearview image not to display when the vehicle is placed in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
The risk
A rearview image that does not display reduces the driver's view behind the vehicle, increasing the risk of a crash.
The remedy

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2025 Toyota Tundra and Tundra Hybrid vehicles. Moisture may enter the reverse light assemblies and cause a light failure.
The risk
Reverse light failure may reduce visibility, and not alert pedestrians and other drivers that the vehicle is backing up, increasing the risk of a crash.
The remedy

The load carrying capacity modification label states the incorrect load carrying capacity, which can result in an overloaded vehicle. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 110, "Tire Selection and Rims."

The risk
A fuel leak in the presence of an ignition source can increase the risk of a fire.

The defect
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2023 Tundra and Tundra Hybrid vehicles that may be equipped with a Toyota genuine accessory tonneau cover. The tonneau cover attachment joints may come loose and allow the cover to detach.
The risk
A tonneau cover that detaches while driving can become a road hazard, increasing the risk of a crash.
The remedy
Owners of the subject vehicles will be notified by first class mail to return their vehicles to a Toyota dealer. Dealer will install new hardware (bolts, spacers, washers, nuts, rubber washers and cotter pins) to the existing bracket and tonneau cover. If the tonneau cover has tears or splits at the mounting holes to the extent that new hardware cannot be installed, it will be replaced with a new tonneau cover and hardware. The defect

A software error in the combination meter's electronic control unit (ECU) may cause the Instrument Panel LCD display to go blank. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 101, "Control and Displays."
The risk
A blank display will not show critical information such as the speedometer or warning lights, increasing the risk of a crash.

The defect
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022 Lexus NX250, NX350, and Toyota Tundra vehicles. The Skid Control ECU within the Brake Actuator Assembly may falsely detect an overcurrent condition of the Electronic Parking Brake (EPB) Actuator and enter a failsafe mode which prevents the EPB from being engaged. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 135, "Light Vehicle Brake Systems."
The risk
An electronic parking brake that cannot be engaged while in "park," may result in a vehicle rollaway, increasing the risk of a crash.
The remedy

Incorrect programming of the parking-assist electronic control unit (ECU) software, may cause the front camera image to appear on the multimedia display instead of the rear camera image when the shifter is placed into reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rear Visibility."

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022 Tundra and Tundra Hybrid vehicles. The flange nuts on the axle shaft sub-assemblies can loosen over time and potentially cause one or both axle shaft sub-assemblies to separate from the axle housing.

On [XXX] I was driving in Connecticut when I realized we had taken the wrong directions. We were following my son’s girlfriend’s iphone which kept changing the directions. When we ended up with an over pass in a parking area I applied my brakes but the brakes didn’t work. I was trying to go left to take an alternate road but couldn’t stop. This kind of thing where the brakes or steering have been happening on and off since we got the truck I believe someone has hacked into it using a car kit is what I found under devices but then it disappeared. FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

AUTOMATIC BREAKING SYSTEMS - I HAVE HAD SEVERAL PROBLEMS WITH THE BREAKING SYSTEMS IN THIS TRUCK GOING OFF WITHOUT ANYTHING AROUND MY TRUCK. THROTTLE STICKING ON TRUCK - ALSO A RECALL AND STATED IT WAS FIXED BUT RECALL DID NOT FIX ISSUE. THE THROTTLE WILL STICK AND KEEP REVVING EVEN AFTER LETTING OF THE GAS. FORWARD COLLISION AVOIDANCE: WARNINGS

LTA feature engaged out of nowhere and my dashboard lit up with red, green and yellow warnings all at once and made my truck swerve back and forth and prevented me from having any control and steered me directly into oncoming traffic almost causing me to have a head on collision. Also the brakes automatically come on out of nowhere for no reason and has almost given me whiplash. I just had major neck surgery with plates and screws put in my cervical spine and if this keeps happening I'm going to need another surgery.

The contact owns a 2022 Toyota Tundra. The contact stated that while driving approximately 40 mph the vehicle crashed into a tree causing severe damage to the front end. During the crash, the air bags did not deploy and the collision avoidance system did not activate to prevent the crash from occurring. No injuries were reported. A police report was filed at the scene and the vehicle was towed away. The cause of the failure was not determined. The manufacturer was notified of the failure. The local dealer was not contacted. In addition, while driving at highway speeds the Tonneau cover nearly came off the vehicle. The cause of the failure was not determined. The failure mileage was 5000.

ENGINE
This is a repeat engine failure on a vehicle that already received the manufacturer's recall remedy. On XXX, at 69,420 miles, the original engine was replaced under Toyota's recall for the V35A 3.4L twin-turbo V6 (manufacturing debris contaminating crankshaft bearings), at Capital Toyota in [XXX]. On XXX, at 109,367 miles — approximately 39,947 miles after the recall replacement engine was installed — the replacement engine failed catastrophically. We first heard knocking in the engine while the truck was idling. The vehicle lost the ability to be driven and had to be towed to Savannah Toyota in Savannah, GA, where it currently remains. Summary of Complaint I am submitting this complaint to report what I believe may be a safety-related defect involving the twin-turbocharger system on my 2022 Toyota Tundra (VIN: [XXX]). The driver’s side turbocharger failed at approximately 58,000 miles and had to be replaced. Approximately 6,000 miles later, at around 64,000 miles, the passenger-side turbocharger also failed. Traveling on the highway with no cars near us and our panoramic sunroof exploded showering myself and my passenger with glass at highway speed. There are no over hanging trees or over passes in this area.

EXTERIOR LIGHTING
Water is leaking into the cab of my 2022 Toyota Tundra Platinum 4x4 through the high-mounted third brake light. The leak allows water to enter the headliner and interior, creating the potential for mold, damage to the interior, and electrical issues. After troubleshooting, the third brake light gasket appears to be the source of the leak. Toyota does not appear to offer the gasket as a replacement part, requiring owners to replace the entire third brake light assembly despite the failure being limited to the seal. ENGINE
Driving on the interstate and the truck surged very hard. The truck began to surge very hard and went into decreased speed mode. We were told by the dealer there is a TSB for the PCM. This decreased power caused the truck to not be able to travel over 25 MPH in a 70 MPH zone. This caused an unsafe situation.
