Loss of motive power due to engine failure post recall remedy

RQ26001

Still open recall query · opened January 16, 2026

the engine

The Office of Defects Investigation (ODI) has received 36 Vehicle Owner Questionnaires (VOQs) alleging engine failure in vehicles subject to Recall 25V274. In each VOQ, the engine had the recall remedy completed prior to the reported failure incident. Recall 25V-274 addresses vehicles equipped with a L87 6.2L V8 gas engine that experienced engine damage or failure. Recall 25V-274’s remedy includes an engine inspection.  Based on the results of the inspection the vehicle will receive an oil change to a higher viscosity oil or an engine replacement.  The complaints received allege failures of both remedies. ODI is opening this Recall Query (RQ) to assess the adequacy of the remedy for Recall 25V-274. To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.

Loss of motive power due to engine failure

EA25007

the engine, the engine and its cooling

The Office of Defects Investigation (ODI) has received 1,157 reports of engine bearing failure failures in the L87 6.2L engine produced by General Motors (GM). Of the 1,157 reports received, 173 are outside of the scope of recall 25V-274. These engines are installed in Model Years (MY) 2019–2024 Chevrolet Silverado 1500 and GMC Sierra 1500, and 2021-2024MY Chevrolet Suburban and Tahoe, GMC Yukon/Yukon XL, and Cadillac Escalade/Escalade ESV vehicles. The subject engine failures were investigated as part of a Preliminary Evaluation (PE25001).  GM determined that these engine failures were due to multiple supplier manufacturing and quality issues. GM issued recall 25V-274 to address the supplier manufacturing and quality issues produced between March 1, 2021, and May 31, 2024. Based on the issuance of recall 25V-274, NHTSA has closed PE25001. ODI continues to receive a significant number of reports of engine failure in vehicles outside the scope of 25V-274.  This presents a potential safety risk that warrants further investigation.  ODI is opening this Engineering Analysis (EA) to further evaluate the scope and severity of the potential problem and to fully assess the potential safety-related issues of vehicles built outside the recall scope. To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.

the engine, the engine and its cooling

On January 16, 2025, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE25001 after receiving complaints and field reports concerning engine failures in MY2019-2024 GM’s full-size truck and SUV T1XX platform equipped with the L87 6.2L V8 engine. These complainants alleged engine bearing failures resulting in knocking and growling noises from the engine’s bottom end and in some cases more severe consequences such as broken connecting rods leading to engine damage, or engine failure. If the engine fails during vehicle operation, the vehicle will lose propulsion, increasing the risk of a crash. On April 24, 2025, GM issued recall 25V-274 to address the concern with L87 engines produced between March 1, 2021, and May 31, 2024.  GM cited the root cause as a supplier manufacturing and quality issues. GM’s  recall remedy will be either an oil change to an oil with a different viscosity or engine replacement depending on the results of the remedy inspection procedure. To date, ODI has received 1,157 allegations of engine bearing failure. GM submitted 55,464 reports in total, with 29,752 unique VINs. ODI continues to receive allegations of L87 engine failures which fall outside of the scope of recall 25V-274. Based on GM’s recall, ODI will close this PE. NHTSA will continue to investigate complaints of engine failure outside the scope of recall 25V-274 as part ofEngineering Analysis (EA25007). To review the ODI reports cited in the Closing Resume ODI Report Identification Number document, go to NHTSA.gov.

POST-RECALL 25V274 ( & EA25007, EA25001) EXHAUST INTRUSION WITH CARBON MONOXIDE POISONING HAZARD Following the complete engine replacement under GM recall N252494000, my vehicle has developed a severe and persistent exhaust intrusion that allows toxic exhaust fumes, including carbon monoxide (CO), to enter the passenger cabin. CO levels have measured a peak of 83 PPM and consistent reading between 20-37 over multiple tests. PATTERN OF POST-RECALL EXHAUST SYSTEM FAILURES Vehicle has been in for service 6 times, over 100 days, as part of the engine replacement plus (metal found in the oil pan). During the operation of my 2023 GMC Yukon XL AT4 on [XXX], carbon monoxide (CO) entered the cabin, exposing my children and me to dangerously high levels of CO and causing confirmed carbon monoxide poisoning. We were driving with the engine running and the heater on for approximately 21 minutes when the exposure occurred. Symptoms began to escalate that day and into the proceeding days, including severe headache, stomachache, pallor, chest pressure, shortness of breath, fatigue, and neurological issues. My children, particularly my [XXX] and [XXX] , experienced the most severe effects— both were very ill especially my [XXX] . Unfortunately, we initially did not believe it was possible for

AIR BAGS

While driving in a gravel driveway, the window curtain airbags and the middle console airbags deployed, with no collision what so ever causing injury to the driver and should you have a child this could have been very damaging as the airbags came out in all the way back through the vehicle. This incident was inspected by police and fire as OnStar called them as it detected a collision, and without the vehicle being moved from the location the officer said there is nothing that should have caused the airbags to deploy, there is absolutely nothing here in this driveway that could have possible caused airbag deployment, The running boards go in and out, even when on the interstate constantly,

At 9,900 miles my safety camera system failed which disabled the cruise control and all safety features that used the camera. Car was out of service for nearly two weeks and subsequently fixed by the dealer under warranty at no charge. At 11,300 miles I was driving on an interstate highway in the left lane at rush hour, without warning the car stopped accelerating the car shifted into neutral and totally lost power. It was a very dangerous situation with me in the left lane in heavy traffic. I am a skillful driver and thankfully I was able to make it off the travel lanes on the highway. The temperature was 18 degrees at the time and I had to wait outside for a tow truck. The engine seized

ENGINE
2023 GMC Yukon XL(6.2 liter engine).Engine has failed twice with the first incident being on the interstate and the engine seized without warning with only 38,000 miles. GMC replaced the engine and now the second engine has failed with only 80,000 miles on it. I am told there needs to be a third engine installed and the vehicle is only three years old. If an engine fails without warning this puts everyone in danger. My [XXX] son was in the car when the first engine failed. There is an obvious manufacturer's defect that needs to be addressed before innocent people are injured or killed. The vehicle is currently located at Darrell Waltrip GMC in Franklin, Tennessee. It is not drivable.

LANE DEPARTURE: BLIND SPOT DETECTION
On my 2023 GMC Yukon, XL the infotainment screen freezes and you have no access to any vehicle information, including rearview camera. This issue appeared three months after I purchased a vehicle in 2023 and was taken to the dealership confirmed the issue, but said the resolution was to do a hard reset on the infotainment center. After driving the car for three years and taking the advice of the dealership and resetting the infotainment center every few weeks, the problem escalated and started freezing up at least once a week to eventually doing it every day and the heart reset wouldn’t always work the first time. Dealership eventually confirmed it was a defective radio module. This is a

SERVICE BRAKES
I got into my car to run some errands. As I drove away from my house, I made a left turn downhill. I noticed vehicles ahead, so I began to slow down. At that moment, I was driving between 20 and 25 mph. When I tried to brake, my brakes failed and started flashing a brake failure warning light. I kept pumping the brake pedal, and eventually, the car slowed down and came to a stop. I turned on my hazard lights, and immediately returned home. My nephew and I were both in the car. Our live flashed in front of us. Thankfully, there were no vehicles directly in front of me or pedestrians. This could have turned out to be the worst day of my life if anyone had been injured.

ELECTRICAL SYSTEM
The car just died while going 55mph on the highway. Complete shutdown but luckily both the steering and brakes worked. After stopping it wouldn’t start again. I had my three children in the car aged [XXX], [XXX], and [XXX]. It was a horrific experience. GMC did some repairs to the engine that took over two months and then the car had a few more warnings pop up just a few weeks later. GMC now says there is a misfiring in one of the positions. I’m terrified of driving this car with my children as I don’t know when it will shut off again and how bad it will be! FUEL/PROPULSION SYSTEM
On July 14, 2026, I experienced a serious fuel system failure with my 2023 GMC Denali. That morning, I filled the vehicle with a little over 20 gallons of fuel at a gas station approximately two miles from my home. Later in the day, I made a short trip to my neighborhood swimming pool, roughly 1.5 miles round trip, with no unusual noises or warnings from the vehicle. Approximately two hours after returning home, I entered the vehicle to run an errand. When I shifted into reverse and began backing out of my carport, I immediately heard a scraping noise on the concrete. I stopped the vehicle and inspected underneath. At that point, I discovered that the entire fuel tank had fallen out of the

A complaint is one owner's account and has not been verified by NHTSA or by us. A vehicle with more complaints is not necessarily worse: models that sold in larger numbers collect more of them. How this is counted.
