The defect
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
The risk
A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.
The remedy
Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.

The defect
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2023 Mirai, Lexus LS500, LS500H, 2023-2024 Lexus ES250, ES300H, ES350, 2024 Lexus LC500, and LC500H vehicles equipped with a Panoramic View Monitor (PVM) system. Due to incorrect programming of the parking-assist electronic control unit (ECU) software, the rearview camera image may not immediately display within a couple of seconds after the shifter is placed into reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rear Visibility."
The risk
A rearview camera that fails to display an image can reduce the driver's rear view, increasing the risk of a crash.
The remedy
Dealers will update the parking assist ECU software, free of charge. Owner notification letters were mailed April 10, 2024. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's numbers for this recall are 24TA01 and 24LA01.

SERVICE BRAKES

I was able to reproduce the issue. When accelerating from a stop, but not yet passing 7 mph (typically 3-5mph registering on the speedometer), then suddenly braking, the brakes do not immediately engage, but rather engage after several seconds have passed. The system/vehicle is available for inspection upon request. The vehicle did not stop (as the brakes weren't applied) causing a collision. I am able to reproduce the issue in the vehicle. No inspection has yet taken place. No warning lamps or other vehicle messages appeared prior to or following the braking failure and the resulting collision. This undocumented behavior was not noted prior to the failure resulting in a collision. Normal

BACK OVER PREVENTION: AUTOMATIC SYSTEM BRAKING

The contact owns a 2024 Lexus ES 300h. The contact stated that while his wife was in reverse (R) and backing up into a parking space at approximately 2 MPH, she crashed into the rear of another vehicle. The contact stated that his wife did not hear the rear collision proximity alarm, and the vehicle did not automatically brake. The contact stated that the other vehicle was not damaged, but the contact's vehicle had damage to the driver's rear bumper and trunk. The contact stated that his wife was able to drive the vehicle away. The contact had taken the vehicle to a local dealer, who was unable to duplicate the failure and advised that all collision avoidance sensors were operating

Driver side airbag cover is open. Previously reported, dealer states that it was open due to dashcam wiring being placed and cover not being closed completely. Dealer closed the cover completely. Today, the cover is open again.

SEAT BELTS

The contact owns a 2024 Lexus ES 350. The contact stated that while attempting to use the front driver's side seat belt, the seat belt failed to extend as needed, and the contact was unable to use the seat belt. The contact stated that the failure had been occurring intermittently. The vehicle was taken to the dealer, where the failure could not be duplicated. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 10,100.

The contact owns a 2024 Lexus ES300H. The contact stated while the vehicle was parked, the contact noticed that the driver's side air bag cover was detached. The vehicle was not diagnosed or repaired by an independent mechanic or the dealer. The vehicle was scheduled for inspection on the following day. The manufacturer was not made aware of the failure. The failure mileage was approximately 3,072.

LANE DEPARTURE: BLIND SPOT DETECTION
Vehicle Blind Spot indicator did not come on while car was on side of Vehicle on trip to Atlanta. If car did not honk we surely would have had an accident. I started to monitor when the light came on and noticed in was extremely inconsistent with respect to how for an approaching vehicle was from ours before activating. Sometimes it would come on 3 or more car lengths before reaching our bumper, sometimes after car was 1/3 of the way past our bumper, sometimes not before car was completely in blindspot or later. I filmed this but was only able to show the caring distances, and could not get an example of completely in blindspot with monitor not activated. Dealer and Lexus refuse to fix
