The defect
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2025 Lexus NX350, NX250, 2023-2026 RX350, and 2024-2026 TX350 vehicles. The rearview camera image may not display when the vehicle is placed in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
The risk
A rearview image that does not display reduces the driver's view behind the vehicle, increasing the risk of a crash.
The remedy
Dealers will update the rearview camera software or replace the rearview camera, as necessary, free of charge. Owner notification letters were mailed May 15, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 26LB02 and 26LA02.

BACK OVER PREVENTION:DISPLAY FUNCTION

The defect
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
The risk
A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.
The remedy
Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.

The defect
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2025 Toyota Camry Hybrid, Lexus NX250, NX350, NX350 Hybrid, 2024-2025 Lexus RX350, RX350 Hybrid, and RX500 Hybrid vehicles. The second-row center seat belt webbing may be damaged. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 209, "Seat Belt Assemblies."
The risk
A damaged seat belt may not adequately restrain an occupant, increasing the risk of injury during a crash.
The remedy
Dealers will inspect the seat belt webbing and replace the seat belt assembly if necessary, free of charge. Owner notification letters were mailed March 20, 2025. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TA03 and 25LA03.

SEATS:FRONT ASSEMBLY:HEAD RESTRAINT

The defect
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2024 Lexus RX 350, RX 350h, 2024-2025 NX 250, NX 350, and NX 350h vehicles. The head restraints on the driver and front passenger seats may be inadvertently removed without pressing the lock release button. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) Number 202, "Head Restraints."
The risk
A head restraint that is inadvertently removed and not reinstalled can increase the risk of injury during a rear-end crash.
The remedy
Dealers will replace the driver and front passenger seat head rest assemblies, free of charge. Owner letters were mailed February 14, 2025. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's numbers for this recall are 24LB06 and 24LA06.

STEERING

The car comes with proactive driver assist. The system when activated will cause the car to unexpectedly turn the wheel and lunge usually left if the system suspects you are going off of track. It works even on unmarked 2 lane country roads. Very disconcerting and dangerous. My wife was then subsequently in a serious accident on a slippery and snowy night that totaled the car. We are not convinced that the proactive driver assist system was the cause of the accident but can't help but think it may have been a contributing factor to the accident. Her car swerved into the oncoming lane on a unmarked (snow covered) highway.

ELECTRICAL SYSTEM

I am reporting a recurring electrical and mechanical system failure on a 2024 Lexus RX 350h Luxury. On [XXX], the vehicle suffered a total power loss that rendered the electronic door latches unresponsive to all inputs. Because the occupants were a [XXX] and a [XXX] strapped into car seats, they were physically unable to reach the interior manual overrides. Critically, the mechanical backup key also failed to unlock the vehicle from the exterior, preventing immediate rescue. This created a life-threatening entrapment scenario. The doors only responded after multiple minutes of repeated manual attempts. Once entry was gained, the vehicle would not start and the headlights began blinking

I experienced a sudden release of toxic fumes from the vehicle between the steering column and instrument panel area on June 29, 2025, while driving. The fumes struck my face, caused immediate bitter chemical-like taste and made me feel physically ill and extremely concerned for my safety. The fumes were strong enough to leave a lingering taste described as “bitter and green” with initial symptoms of headache, lightheadedness, nausea, vomiting green substances, eye irritations, nose and throat irritations. The local auto dealership inspected the vehicle, no mechanical defect was identified, and they noted that they cannot reproduced the event that led to the release of the toxic fumes.

ELECTRICAL SYSTEM

The contact owns a 2024 Lexus RX. The contact stated that while idle at a red stoplight, the entire vehicle began to violently jerk back and forth. No warning lights illuminated. The vehicle was taken to a local dealer, where it was diagnosed with a software malfunction, and the failure was replicated. The vehicle was repaired. Medical attention was sought for severe depression, anxiety, PTSD, memory loss, concussion, head, neck, and back injuries. No police report was filed. In addition, the contact stated that after the injuries, she was no longer able to work or drive. The manufacturer was made aware of the failure. The failure mileage was approximately 22,790.

POWER TRAIN
While on a long road trip on a hot day, on the interstate, the car suddenly lost speed and would not accelerate. It was like the car had shifted into neutral. I was coasting on the interstate, steadily slowing, and was forced to pull over to the shoulder. I turned the car off and waited a couple of minutes then turned the car back on. No error messages were displaying at that time. About 5-10 minutes later, I pulled over for a planned stop. My husband assumed driving responsibilities when we got back into the car. The sunroof exploded spontaneously while driving on major road at 65 MPH, nothing hit it and no other vehicle around. No abnormal weather conditions. No warning just a burst into air. Remaining glass fell into vehicle when I opened shade.

LANE DEPARTURE: ASSIST
When in cruise, the car will merge itself into the next lane if you hold the turn signal partially. If you keep holding it or fully engage the turn signal (maybe you forgot to turn off you signal, or held it too long), the car will merge through the solid white line off the road into a ditch. This happened to me once, and I noticed it trying several other times.

The contact owns a 2024 Lexus RX 350H. The contact stated that while attempting to start the vehicle, several unknown warning lights illuminated. Additionally, the door latches locked independently, and the contact and wife were locked inside the vehicle for several hours. The contact eventually remembered to pull the mechanical door latch and exited the vehicle. The vehicle was towed to the dealer where it was diagnosed with battery failure. The contact was informed that the battery needed to be replaced. The vehicle was repaired. The manufacturer was made aware of the failure, and a case was filed. The failure mileage was approximately 17,500.

The contact owned a 2024 Lexus RX 350H. The contact stated that while the vehicle was parked and running, it unexpectedly turned off with the electronic malfunction error message displayed. The vehicle could not able to be restarted, and the contact was unable exit the vehicle due to the failure of the power locks. Roadside Assistance assisted with removing the contact from the vehicle. The vehicle was taken to the dealer, where it was diagnosed with battery failure. The contact was informed that the battery needed to be replaced. The vehicle was repaired; however, the failure persisted. The vehicle was taken back to the dealer and remained with the dealer. The dealer was not contacted. The

SERVICE BRAKES
I was stopped at a red light with brake on, my vehicle leaped forward on its own three times, with the brake on, the force was so strong I thought someone rear-ended me. No warning lights, the acceleration was load.
