The defect
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2025 Lexus NX350, NX250, 2023-2026 RX350, and 2024-2026 TX350 vehicles. The rearview camera image may not display when the vehicle is placed in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
The risk
A rearview image that does not display reduces the driver's view behind the vehicle, increasing the risk of a crash.
The remedy
Dealers will update the rearview camera software or replace the rearview camera, as necessary, free of charge. Owner notification letters were mailed May 15, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 26LB02 and 26LA02.

BACK OVER PREVENTION:DISPLAY FUNCTION

The defect
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
The risk
A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.
The remedy
Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.

ELECTRICAL SYSTEM: INSTRUMENT CLUSTER/PANEL

The defect
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2023-2024 Toyota Venza, 2023-2025 RAV4 Prime, RAV4, Highlander, GR Corolla, Crown, 2024-2025 Lexus TX, LS, Toyota Tacoma, Grand Highlander, and 2025 Lexus RX, Toyota Crown Signia, Camry, RAV 4 Plug-in Hybrid (PHEV), and 4 Runner vehicles. Due to an error in the instrument panel software at vehicle startup, the instrument panel may fail to display vehicle speed, brake system, and tire pressure warning lights.
The risk
An instrument panel display that does not show critical information can increase the risk of a crash or injury.
The remedy
Dealers will update the instrument panel software over-the-air (OTA) for non-PHEV vehicles, free of charge. For PHEV vehicles, dealers will inspect the instrument panel assembly, and either replace it, or update the software, free of charge. Owner letters were mailed December 5, 2025. Additional letters will be sent, anticipated in May 2026. This is a phased recall. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB08 and 25TA08. Lexus' numbers for this recall are 25LB05 and 25LA05.

SEAT BELTS:REAR/OTHER:WEBBING

The defect
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2025 Toyota Camry Hybrid, Lexus NX250, NX350, NX350 Hybrid, 2024-2025 Lexus RX350, RX350 Hybrid, and RX500 Hybrid vehicles. The second-row center seat belt webbing may be damaged. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 209, "Seat Belt Assemblies."
The risk
A damaged seat belt may not adequately restrain an occupant, increasing the risk of injury during a crash.
The remedy
Dealers will inspect the seat belt webbing and replace the seat belt assembly if necessary, free of charge. Owner notification letters were mailed March 20, 2025. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TA03 and 25LA03.

The contact owns a 2025 Lexus RX 350. The contact stated that while driving into a parking space, the vehicle suddenly experienced unintended acceleration and drove forward at a high rate of speed before crashing into the brick wall of an office building. During the crash, no air bags deployed, and the brake pedal was hard when pressed and did not stop the vehicle. Also, the manufacturer's APP indicated a malfunction in the engine and engine throttle during the incident. The driver sustained injuries to the knees, arms, and hips, which required medical treatment. Around 11 am on Sat., May 2, 2026, I was sitting in a very long line in Starbuck's drive thru when my 2025 Lexus RX hybrid suddenly and uncontrollably accelerated--like an uncontrollable bucking horse. Brakes didn't work. I couldn't stop my out of control car, and the multiple computer sensors also didn't stop the car. It was like the throttle was engaged because of a computer glitch. In an effort to avoid hitting the car in front of me, I immediately tried to swerve, but still hit the car, then jumped the exceptionally high curb and ended up in the grass below and almost into the busy road ahead of me. SERVICE BRAKES

On April 1, 2026, while pulling into a parking space at our church, I was applying the brake when the vehicle suddenly and forcefully surged forward without warning. The unintended acceleration was intense and unexpected, occurring despite continuous braking input. This resulted in the vehicle striking the building and causing significant damage to both the vehicle and the structure. This incident has occurred twice within a 10 day period, both times during low-speed parking/stopping maneuvers. There were no warning lights, alerts, no automatic emergency braking engagement, no air bag deployment, or prior indications of malfunction. TIRES
The passenger rear tire (235/60R19 - Michelin Primacy A/S) randomly blew out while driving at high speeds on the highway. No trauma via a pothole or other object was noted. Tire was inflated to the manufacture recommended PSI (33 PSI). No signs came up until the tire blew out. The tire just randomly decided to give out while driving.

AIR BAGS
The car does not alert when a seat belt is not worn in the back seats. This feature is mentioned in the owner's manual, but the car fails to alert when a rear passenger does not buckle their seat belt. I have raised the complaint twice and proved to lexus service center, that car doesn't warn or alert the safety belt. It was also proven and checked by Lexus with their technician. The problem is accuring right from the time car was purchased, When check with the dealer it was designed , but their manual confirms the seat belt not worn warning light is included in the package. It put the any passener riding mainly children at serious risk

VISIBILITY/WIPER
I bought a CPO lexus 2025 RX 350 in Jan 9, 2026. This car was lightly used in less than 3 months and right now it's around 7685 mileage. However, just with 3 months I purchased this car, the A/C stops cooling and the dealership said this is due to the condenser was damaged by small road stones where the small road stones can easily pass through the grille. - We have noticed that there are multiple reports of Lexus/Toyota SUVs with condensers easily punctured by normal road debris. - Dealers acknowledge it as a common pattern. - Loss of A/C can affect defogging/visibility and is risky in extremely hot weather, e.g. causing hard breathing, etc for driver and passengers in the car.
