NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

TL* THE CONTACT OWNED A 2015 LEXUS CT. WHILE STOPPED IN TRAFFIC, THE CONTACT'S VEHICLE WAS REAR ENDED BY A 2005 SUBARU CROSSFIRE. THE CONTACT THEN CRASHED INTO A 2011 HONDA CR-V, HYDROPLANED, AND CRASHED INTO THE SUBARU CROSSFIRE. THE VEHICLE WAS TOWED AND DEEMED A TOTAL LOSS. THE AIR BAGS DID NOT DEPLOY. THE CONTACT SUSTAINED FIVE BULGING DAMAGED NERVES AND MEDICAL ATTENTION WAS PROVIDED. THE DRIVER OF THE HONDA CR-V SUSTAINED UNKNOWN INJURIES. A POLICE REPORT WAS FILED. SERVICE BRAKES
While driving normally one day, all the brake lights on the dash came on and my stopping power was diminished. I pulled the trouble codes and the only code was C1391 which indicates an "Abnormal Leakage in the Accumulator". This is a major safety concern and a costly repair if left to the customer.

For several years now, I have been experiencing incidents where someone and/or a group is hacking into my radio and/or satellite radio systems and trying to lure me into conversation, make comments, or actually sing to whatever it is I'm listening to. Most incidents have been reported to the NHTSA, which they have been filing. Please know that sometimes the "noise is constant", as well as it appears that someone is actually connecting to my vehicle because I can feel it (also reported). I also hear people speaking from the outside of my vehicle (usually the front end on both sides). PROBLEM PARTS: Satellite Radio and/or AM-FM Radio; Bluetooth; Speaker System; Unknown Device The parts have not malfunctioned however, there appears to be a person and/or group attaching to my vehicle via several conduits related to any radio input and/or speakers. This has been ongoing for years where I have submitted previous reporting based on hearing voices not directly affiliated with the broadcaster(s); being the victim of Havana Syndrome; Radio-Frequency Assaault; Satellite Terrorism, and other unwanted Amateur Radio broadcasters that may or may not be acting with malintent. SERVICE BRAKES
received CUSTOMER SUPPORT PROGRAM 24LE01 NOTIFICATION for issues related to Brake Booster and Brake Booster Pump Assemblies. Indicator lights, such as Master Warning Light (Yellow) and SLOP Indicator (Yellow) have appeared while reversing out of a parking spot when no water puddles are present and moving foward (clear day/road) where the vehicle doesn't catch the gear to move initially. Once braking, the system seems to reset itself and allows for a "normal drive". The experience is intermittent, and not often- the dashboard warning lights come on infrequently and is not consistent. Lexus Dealer/Service Shop advised a charge of $145 to look for diagnostic codes C1391, C1252, C1256, C1253.

ELECTRICAL SYSTEM
The following two events happened during the first week of April 2024 EVENT #1: While driving at 65 MPH on a freeway, the car decelerated and failed to travel above 40 MPH. Simultaneously, three warming lights appeared on the dash: a yellow triangle, the check engine light and a "CHECK HYBRID SYSTEM" warning. EVENT # 2: Same symptoms and dash displays as event number one while traveling at 40 miles up a hill.
