The defect
General Motors, LLC (GM) is recalling certain 2024 Cadillac XT4 and Chevrolet Malibu vehicles. The vehicles may have the wrong owner's manual in the glove box. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 225, "Child Restraint Anchorage Systems."
The risk
The wrong owner's manual may contain inaccurate information about vehicle functions and other safety relevant information, which could lead to improper operation of the vehicle and increase the risk of a crash or injury.
The remedy
Dealers will provide owners with the correct manual, free of charge. Owner notification letters were mailed August 26, 2024. Owners may contact Cadillac's customer service at 1-800-458-8006 and Chevrolet customer service at 1-800-222-1020. GM's number for this recall is N242454040.

I am submitting this complaint to report a significant safety system failure in my 2025 Cadillac XT4 during an accident that occurred on April 12, 2025, in Dallas, Texas. During the incident: •The Anti-lock Braking System (ABS) did not activate, and I was unable to stop in time to avoid a collision. •Neither the driver nor passenger airbags deployed, despite sufficient impact that should have triggered deployment. •OnStar Automatic Crash Response did not activate, nor was I contacted by any emergency response service through the vehicle. I WAS STOPPED AT A RED LIGHT, IT TURNED GREEN AND I PROCEEDED TO DRIVE FORWARD THROUGH THE INTERSECTION WHEN ALL OF A SUDDEN THE AUTOMATIC BRAKING SYSTEM ENGAGED BRINING THE CAR TO AN IMMEDIATE STOP IN THE MIDDLE OF THE INTERSECTION. MY PASSENGER AND I WERE THRUST FORWARD INTO THE STEERING WHEEL AND DASHBOARD RESPECTIVELY. THE CAR BEHIND ME HIT ME. BOTH CARS SUSTAINED DAMAGE. THERE WAS NOTHING IN FRONT OF ME THAT SHOULD HAVE CAUSED THIS SAFETY SYSTEM TO ENGAGE.I BELEIVE THEY CALL THIS PHANTOM BRAKING. BECAUSE I HAD JUST STARTED UP FROM THE LIGHT, MY SPEED WAS SLOW AND WE SUSTAINED NO INJURIES. HOWEVER IF THAT HAPPENED ON THE FREEWAY I TRAVEL QUITE FREQUENTLY, I WOULD HAVE BEEN KILLED. I AM

FUEL/PROPULSION SYSTEM
I was driving down the road and my vehicle lost power and quit running. Luckily I was in the lane closest to the shoulder and was able to pull over on the shoulder and was not able to restart my vehicle, but had this happened in another location this could have been a huge safety problem for me and others with the vehicle completely losing power while driving in the middle of traffic on highway 27. The vehicle is still under warranty and has 10k miles. I am the only owner of the vehicle and it has only been serviced at the Cadillac dealer. The Cadillac dealer was able to inspect my vehicle and determined that the fuel pump module went bad and replaced it and advised that this has happened

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
Automatic emergency braking activated on three separate occasions when there weren’t any reasons for it to engage, additionally if there were any cars behind me I would have been rear ended since they would not have anticipated me stopping so suddenly.

STEERING
The vehicle has a chronic defect causing rapid, repeating front-end suspension and steering assembly failures. The right front strut and mount failed and were replaced under warranty twice in 2025 (at 29,968 miles and 35,740 miles). In May 2026, the left front strut and mount failed and were replaced. Two days after picking up the vehicle, a cascading mechanical failure occurred when the left front CV axle boot split, covering the wheel well in grease and requiring a complete CV axle shaft replacement. FUEL/PROPULSION SYSTEM
Vehicle died leaving the doctor's office due to faulty fuel pump driver control module. Upon research found that this has been an ongoing issue for several years affecting many GM vehicles. Vehicle can stall at any time in traffic causing serious injury or death. Apparently GM does not have a fix for this problem & the company's solution is to replace the faulty module with modules that have the same defects. Also, when affected vehicles stall in traffic, the parking brakes set & there is no way to even put the vehicles in neutral so that they can safely be moved out of the roadway. This increases the chances of causing a serious accident if one was not caused by the initial breakdown. GM

FUEL/PROPULSION SYSTEM
I was getting out of a parking lot with my 3 kids and the car shut off right in the middle of the entrance/exit of the lot blocking anyone from coming in or out. The car would not go into neutral to move it, making it unsafe for my family and everyone around us. We tried to charge the battery but that did nothing. We called tow truck then where told we need a special tow truck since the car could not be put into neutral. We waited hours for a tow truck, when we finally go there, the car's emergency breaks keep coming on and would not turn off. We ended up removing the battery. ENGINE
Problem with the fuel pump control module. Car broke down in front of my neighbor’s house. Now there is a problem with the transmission. It is slipping and there’s a clunking noise.
