The defect
Southeast Toyota Distributors, LLC (SET) is recalling certain models interspersed through model years 2009 through 2013 as follows: model year 2009-2012 Tacoma, 4Runner, Camry, Camry Hybrid, Prius, and RAV4; model year 2009-2010 Avalon, FJ Cruiser, and Highlander Hybrid; model year 2010-2013 model year Corolla, Sienna and Tundra; model year 2009-2013 Highlander and Venza; model year 2012 Prius V; and model year 2010-2012 Sequoia. During modification by SET to include accessories such as leather seat covers, seat heaters or headrest DVD systems, these vehicles may not have had the passenger seat occupant sensing system calibration tested. Without passing the calibration test, the occupant sensing system may not operate as designed.
The risk
If the front passenger seat occupant sensing system is out of calibration, the front passenger airbags may not deploy or they may deploy inappropriately for the passenger's size and position. This could increase the risk of personal injury during the event of a vehicle crash necessitating airbag deployment.
The remedy
Southeast Toyota will notify owners, and dealers will test the sensitivity of the occupant detection sensors, and recalibrate them as necessary. The recall began on March 21, 2013. Owners may contact Southeast Toyota at 1-800-301-6859.

The defect
TOYOTA IS RECALLING CERTAIN MODEL YEAR 2004-2010 PASSENGER VEHICLES. THE ACCELERATOR PEDAL CAN GET STUCK IN THE WIDE OPEN POSITION DUE TO ITS BEING TRAPPED BY AN UNSECURED OR INCOMPATIBLE DRIVER'S FLOOR MAT.
The risk
A STUCK OPEN ACCELERATOR PEDAL MAY RESULT IN VERY HIGH VEHICLE SPEEDS AND MAKE IT DIFFICULT TO STOP THE VEHICLE, WHICH COULD CAUSE A CRASH, SERIOUS INJURY OR DEATH.
The remedy
TOYOTA FILED AN AMENDED DEFECT REPORT ON NOVEMBER 25, 2009, STATING THAT DEALERS WILL MODIFY THE ACCELERATOR PEDAL AND, ON CERTAIN VEHICLES, ALTER THE SHAPE OF THE FLOOR SURFACE UNDER THE PEDAL. THESE CHANGES ADDRESS THE RISK OF PEDAL ENTRAPMENT DUE TO INTERFERENCE WITH THE FLOOR MAT. REDESIGNED ACCELERATOR PEDALS WILL BECOME AVAILABLE BEGINNING IN APRIL 2010 AND DEALERS WILL REPLACE ANY MODIFIED PEDAL WITH THE NEW PEDAL IF DESIRED. ALSO, DEALERS WILL REPLACE ANY GENUINE TOYOTA OR LEXUS ALL-WEATHER FLOOR MATS WITH REDESIGNED ALL-WEATHER MATS, OR REPURCHASE THE PREVIOUS MATS FROM OWNERS WHO DO NOT WANT THE NEW ONES. ADDITIONALLY, SOFTWARE MODIFICATIONS WILL BE INSTALLED ON CAMRY, AVALON AND LEXUS ES 350, IS 350 AND IS 250 MODELS THAT WILL ENSURE THAT THE BRAKE OVERRIDES THE ACCELERATOR IN THE EVENT BOTH BRAKE AND ACCELERATOR PEDALS ARE APPLIED. TOYOTA WILL BEGIN MAILING LETTERS TO OWNERS ON DECEMBER 31, 2009. OWNERS MAY CONTACT TOYOTA AT 1-800-331-4331, LEXUS AT 1-800-255-3987.

NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

On June 25, 2014, Toyota initiated a Warranty Enhancement Program (ZE1) to extend warranty coverage for the brake actuator assembly and skid control ECU/brake pedal stroke sensor in certain MY 2007 to 2011 Toyota Camry hybrid vehicles. Toyota's program covers repairs related to two conditions:internal malfunctions of the Brake Actuator assembly; and overly sensitive monitoring logic for the Brake Pedal Stroke Sensor.Primary coverage provides warranty enhancement until November 30, 2015, with no mileage limitation.After the primary coverage period ends, secondary coverage is provided for 10 years or 150,000 miles from the date of first use, whichever occurs first.In addition, on July 10, 2014, Toyota initiated Limited Service Campaign E0U to replace the Brake Reservoir Tank in MY 2007 through 2011 Toyota Camry hybrid vehicles.The LSC will be available until June 30, 2017, and will only be available at an authorized Toyota dealer.ODI's analysis of failure data determined that the majority of records were related to the Brake Actuator condition, which appeared to be the causal component in 734 complaints/field reports and 864 warranty claims.The Brake Pedal Stroke Sensor (80 reports/189 claims) and Brake Reservoir Tank (91 reports/44 claims) conditions were identified in far fewer records.Analysis of the failure data indicate that each condition provides warning prior to any reduction in power assisted braking, with less than 10 percent of all reports and claims involving diagnostic codes or complaint narratives indicating increased braking effort or reduced brake effectiveness.In addition, most of the incidents in which reduced braking assist appears to have occurred involved operation in a reduced assist mode and not a sudden, complete loss of power assisted braking.ODI assessed reduced assist mode in a subjective driving evaluation which indicated that the brake system provided normal and harddeceleration capability with reasonable increases in pedal effort.Less than one percent of reports and claims analyzed by ODI included evidence indicating a complete loss of power assisted braking occurred and this mode requires operation for a significant period of time with multiple audible and visual warnings.Repair expense was cited in many of the complaints reviewed by ODI, especially those related to the Brake Actuator, which had repair costs exceeding $3,000 according to some complaints.High repair cost appears to have been a factor in the only crash identified by ODI for the fault conditions identified by Toyota, which involved a Brake Actuator failure that was diagnosed but the owner declined repair approximately eight months prior to the reported crash incident.This preliminary evaluation is closed.The closing of this investigation does not constitute a finding that a safety-related defect does not exist.For additional information regarding this investigation, including analysis of the Brake Actuator, Brake Pedal Stroke Sensor, and Brake Reservoir Tank fault conditions and failure experience, see Attachment A to this closing resume.

TL* THE CONTACT OWNS A 2010 TOYOTA CAMRY HYBRID. WHILE DRIVING APPROXIMATELY 40 MPH AND ATTEMPTING TO ENGAGE THE BRAKES, THE BRAKES MALFUNCTIONED CAUSING THE VEHICLE TO CRASH INTO THE REAR OF A SECOND VEHICLE. WHILE DEPRESSING THE BRAKE PEDAL, THE VEHICLE SLOWED DOWN BUT DID NOT RESPOND NORMALLY. THE AIR BAGS FAILED TO DEPLOY. NO INJURIES WERE REPORTED AND A POLICE REPORT WAS NOT FILED. THE CAUSE OF THE FAILURE WAS NOT DIAGNOSED. THE MANUFACTURER WAS NOT NOTIFIED. THE FAILURE MILEAGE WAS 71,350.

BRAKE ACTUATOR: MY BRAKES WERE MAKING A CREAKING NOISE /THUMPING WHEN PRESSED...IN WHICH CASE I THOUGHT IT WAS JUST TIME FOR REGULAR MAINTENANCE. ABOUT 0430, DECEMBER 21, 2017 (MY WEDDING ANNIVERSARY) I WAS DRIVING ON THE HIGHWAY 99SOUTH HEADED FROM FRESNO, CA TO TULARE, CA...ABOUT A 45MIN DRIVE. WHILE ON THE HIGHWAY, MY CAR STARTED TO MAKE RANDOM CHIMING SOUNDS THAT DID NOT SEEM TO COME FROM ANYWHERE PARTICULAR. THERE WAS NO INDICATOR LIGHT ASSOCIATED WITH THE SOUND...IT WAS JUST RANDOM SOUNDS I HAD NEVER HEARD BEFORE. THAT WENT ON PERIODICALLY FOR THE NEXT 20-30MINS. I FINALLY MADE IT TO THE PROSPERITY AVE OFF-RAMP AND BEGAN TO PRESS THE BREAKS. IMMEDIATELY, ALL OF THE LIGHTS ON MY PANEL

SERVICE BRAKES
THE ABS BRAKE ACTUATOR ON MY TOYOTA CAMRY HYBRID HAS GONE BAD CAUSING MY VEHICLE TO BE UNSAFE FOR OPERATION. I WAS INITIALLY TOLD THAT THE ACTUATOR WOULD BE COVERED UNDER A WARRANTY EXTENSION. HOWEVER AFTER FURTHER INSPECTION, I WAS TOLD THAT IT WOULD NOT BE COVERED BECAUSE THERE ISN'T ANY WARNING LIGHTS ON THE DASHBOARD AND/OR NO ERROR CODES PRESENT WHEN HOOKED UP TO A SCAN TOOL EVEN THOUGH THE PART IS FAULTY AND NEEDS TO BE REPLACED.

LUG NUTS SWELLING MAKING IT IMPOSSIBLE TO REMOVE WITH SUPPLIED LUG WRENCH. ALL 20 LUG NUTS SHOW EVIDENCE OF SEPARATION OF THE STEEL NUT AND THE DECORATIVE CHROME COVER COVERING THE STEEL LUG NUT. DEFECT WAS FOUND BY TIRE DEALER DURING PURCHASE OF NEW TIRES WHEN THE TECHNICIAN HAD DIFFICULTY REMOVING LUG NUTS. ACCORDING TO THE TIRE STORE MANAGER THIS DAMAGE IS CAUSED BY WATER GETTING BETWEEN THE STEEL NUT AND THE DECORATIVE CAP WHICH CAUSES RUST TO FORM. WHEN THE OXIDATION FORMS IT PUTS PRESSURE ON THE DECORATIVE CAP CAUSING IT TO EXPAND. THE NORMAL 21MM WRENCH WILL NOT FIT OVER THE LUG NUT DUE TO THE EXPANSION SO THE NEXT SIZE WRENCH IS NECESSARY WHICH THEN DAMAGES THE NUT. THE NORMAL LUG
