NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

TL* THE CONTACT OWNS A 2014 CHEVROLET VOLT. WHILE OPERATING THE VEHICLE, THE DOOR AJAR SENSOR INDICATOR ILLUMINATED BECAUSE THE REAR LIFTGATE DOOR WOULD NOT REMAIN LATCHED AND SECURED. IN ADDITION, THE REAR LIFTGATE DOOR WAS HARD TO OPEN AT TIMES AND WOULD COME DOWN VERY HARD WHEN ATTEMPTING TO CLOSE. THE CONTACT'S SON SUSTAINED A FRACTURED RIGHT THUMB WHEN THE DOOR SLAMMED DOWN ONTO THE THUMB. MEDICAL ATTENTION WAS NOT RECEIVED. THE LOCAL DEALER (CABLE DAHMER CHEVROLET IN ST. LOUIS, MO) AND THE MANUFACTURER WERE NOTIFIED, BUT NO ASSISTANCE WAS OFFERED. THE FAILURE MILEAGE WAS 40,000. *TT

3-7-18 WE HAVE A 2014 VOLT WHICH IS CHARGED WITH OUR CHARGING STATION. MY HUSBAND WAS IN THE VEHICLE AND I REACHED WITH MY LEFT HAND TO POP THE CHARGER LID OPEN AND WITH MY RIGHT REACHED FOR THE CHARGER HOSE. MY LEFT HAND FINGER GOT TRAPPED IN THE HINGE OF THE DRIVER DOOR UNKNOWINGLY MY HUSBAND SLAMMED THE DOOR AND CRUSHED MY FINGER WITH SEVERE LACERATIONS. I WAS CARED FOR BY THE FIRE DEPART AND WAS TOLD TO IMMEDIATELY GO TO EMERGENCY BECAUSE OF THE SEVERE INJURY ,NEEDING STITCHES ETC THE USER BOOKLET HAS NO WARNING OF THIS NATURE. THE CHARGER LID XIS 1.5 INCHES FROM THE DRIVER DOOR HINGE. I WROTE TO GMAC AND WAS DENIED ANY CLAIMS TO MY INJURY. I ASKED IF IT IS POSSIBLE TO TAKE MY GRIEVANCE

A USED CHEVY VOLT.IT CAME WITH A LEVEL 1 CHARGER. BASIC 110 V TAKES 8 HOURS TO CHARGE 18KVH BATTERY. THIS CHARGER WAS REPAIRED FROM THE LOOKS OF IT.I USED IT FOR A FEW TIMES AND IT BURNT. LUCKILY IT WAS NOT NEAR ANY FLAME ABLE MATERIAL, OR IT MAY HAVE CAUSED FIRE.

A SIMILAR ISSUE IS CURRENTLY LISTED UNDER THE 2013 VOLT HERE BUT I EXPERIENCED IT ON A 2014 VOLT & IT LED TO COMPLETE ENGINE FAILURE WHILE DRIVING, VERY DIFFERENT THAN JUST "REDUCED PROPULSION POWER". I WAS DRIVING MY VOLT ON ITS EXTENDED RANGE MODE WITH THE GAS ENGINE (DAYS EARLIER, I HAD TAKEN THE CAR TO A REPAIR SHOP & GOTTEN A GREEN LIGHT ON IT) & I GOT AN ERROR MESSAGE "SERVICE POWER STEERING--DRIVE WITH CAUTION". THAT ERROR MESSAGE THEN TURNED TO "SERVICE PARKING ASSIST" & THEN "SERVICE STABILITRAK", BUT IT WAS DRIVING FINE & I INTENDED TO TAKE IT TO THE DEALERSHIP THE NEXT DAY. THEN BEFORE I COULD GET HOME, THE MESSAGE CHANGED TO "PROPULSION POWER IS REDUCED", SO THE CAR COULDN'T

Rear view camera display experiences intermittent failures. The display only occasionally works when the car is placed in reverse. This has led to nearly impacting one or more pedestrians when the car is driven in reverse.

FUEL/PROPULSION SYSTEM
While driving 50mph on a 4 lane highway, propulsion reduced speed and engine unavailable message came on and the car immediately shut down, a car almost hit me because of it. I had to hurry up jump out and try and push it off the road out of oncoming traffic! After about 15 minutes of turning car off and then back on. It finally started again, I then drove 1/4 mile and it did it again this time as turning left across a 2 lane highway with traffic oncoming, once again had to hurry up and jump out and push it out of the road! This was such an extremely scary situation for me and I thank god my wife wasn’t driving with our daughter and grand kids! This is the 2nd time because on 8/1/2025 it

I was slowly maneuvering my car into my garage when the vehicle suddenly and unexpectedly surged forward, despite only lightly tapping the brake and not touching the accelerator. The car had been displaying the warning, “Propulsion Power is Reduced” and was not holding an electric charge in the days before the incident. No malfunction indicator lamp ever turned on. Per the owner’s manual, the vehicle did not need to be serviced because the malfunction indicator light never came on at the same time as the propulsion power reduced message. This led me to believe the vehicle was safe to drive. The unexpected surge caused the car to strike and cause considerable damage to the garage wall.

The contact son owns a 2014 Chevrolet Volt. The contact's daughter stated that while driving 40 MPH, the vehicle loss motive power without warning and the vehicle stalled. The contact was able to pull over to the side of the road. The vehicle was towed to an independent mechanic and was diagnosed with battery failure. The vehicle was not repaired. The manufacturer was notified of the failure and the contact was informed that the time frame for a free battery replacement had expired. The vehicle was towed to the residence and had not been driven since the failure. The failure mileage was unknown.

FUEL/PROPULSION SYSTEM
Loss of propulsion. Vehicle will not start/move. If vehicle does turn on , it will not turn off. Vehicle has been having reduced propulsion issues for multiple years (taken to dealer many times) that dealership has refused to do a HV battery capacity test while under warranty, even after leaving with them for multiple months they claimed they could not replicate error. On day of pickup error presented itself and I showed service advisor and left vehicle for additional testing. Dealer provided explanation of reduced propulsion when in mountain mode as cause even though vehicle was not in mountain mode when error occurred. Dealer performed customer satisfaction GM Program # N172130462 update
