BACK OVER PREVENTION:DISPLAY FUNCTION

The defect
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
The risk
A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.
The remedy
Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.

The defect
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2023-2024 Toyota Venza, 2023-2025 RAV4 Prime, RAV4, Highlander, GR Corolla, Crown, 2024-2025 Lexus TX, LS, Toyota Tacoma, Grand Highlander, and 2025 Lexus RX, Toyota Crown Signia, Camry, RAV 4 Plug-in Hybrid (PHEV), and 4 Runner vehicles. Due to an error in the instrument panel software at vehicle startup, the instrument panel may fail to display vehicle speed, brake system, and tire pressure warning lights.
The risk
An instrument panel display that does not show critical information can increase the risk of a crash or injury.
The remedy
Dealers will update the instrument panel software over-the-air (OTA) for non-PHEV vehicles, free of charge. For PHEV vehicles, dealers will inspect the instrument panel assembly, and either replace it, or update the software, free of charge. Owner letters were mailed December 5, 2025. Additional letters will be sent, anticipated in May 2026. This is a phased recall. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB08 and 25TA08. Lexus' numbers for this recall are 25LB05 and 25LA05.

Moonroof spontaneously exploded while driving on the highway. The moonroof was closed, no one was trying to open it. All windows were closed, no one was trying to open them. My husband was driving and differently we heard a loud explosion which sounded like a shotgun going off and thought someone was shooting at us. I looked behind the car and could see glass flying everywhere. We safely pulled over and the entire moon roof had randomly and spontaneous exploded. No collision. Nothing fell on us etc. A quick Google search with show you that this is not uncommon for my car's make and model. Something needs to be done! It was terrifying and could have killed is all in the car from the panic

I was stopped at a red light at an intersection, when a car pulled up next to me, and the driver rolled down the window to tell me my vehicle was on fire. I got out of my vehicle while the driver of the other car called 911. Brookline Police & Fire came, firefighters extinguished the fire, and one of the emergency services responders had a wrecker come & take the car away. The car was towed to Watertown, then collected by Safety Insurance, which got a repair estimate of $95,000. The car was deemed a total loss and is in the possession of the insurance company. SERVICE BRAKES

While driving at no more than 5 mph, looking for a parking space, the car suddenly accelerated dramatically. I vigorously applied pressure to the brake but the car continued to go forward, sideswiping a parked pickup truck and crashing through a metal fence and coming up against a rock wall. Fortunately, there were no pedestrians walking through the parking lot and no one was injured. I don’t know if the problem has been reproduced or confirmed by a dealer or independent service center. VEHICLE SPEED CONTROL

The driver was pulling into a friend's carport. After crossing a small curb her foot was taken off the gas and was moving to the brake. The car was going slightly uphill as slow as a car can go with no gas pedal depressed. The car then accelerated as if someone pressed the gas to the floor. In a matter of 8 feet it gained enough speed and momentum that when it hit our friend's parked car the 4 door SUV was pushed forward 4 feet. The gas pedal was never pushed by the driver. No failed component was ever identified by multiple inspections by the Toyota dealership, the insurance company, the body shop and the 3rd party company hired by Toyota to download the ACM data and the Vehicle Control

SERVICE BRAKES
I am reporting a serious safety issue involving my vehicle where the braking system did not respond as expected during an emergency. While traveling southbound on I-575 in Georgia, I experienced a sudden tire blowout. As I moved to the shoulder and applied the brakes firmly, the vehicle did not slow down as expected. Despite pressing the brake pedal continuously and with significant force, the vehicle continued traveling for approximately 1–2 miles before finally coming to a stop near Exit 11. The car was under warranty and experienced a transaxle failure while on a highway between Yuma, AZ and Tucson, AZ. The dealer who inspected and repaired the car determined the cause to be a flawed cooler replacement by a body shop in which transmission fluid was not properly replaced subsequent to a left front fender bender accident, which voided the warranty. (Fluids were dry on inspection; no leaks/issues identified at our residence or during "Toyota Care" X2 following the body shop repair.) The car worked flawlessly for one year after the body shop repair until the transaxle failed. ELECTRICAL SYSTEM
The contact owns a 2024 Toyota Rav4 Prime. The contact received notification of NHTSA Campaign Number: 25V595000 (ELECTRICAL SYSTEM). The vehicle was taken to the local dealer, and the contact was informed that the part needed to perform the recall repair was not yet developed. The contact stated that the vehicle remained with the dealer for four months. The contact was provided with several loaner vehicles, but no ETA was available for parts availability. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was not made aware of the issue. The contact had not experienced a failure.
