Defect investigations (2)

An investigation means NHTSA has decided a pattern is worth examining. A preliminary evaluation is often closed with no action; an engineering analysis is the stage that usually precedes a recall.

Inadvertent Seat Belt Unlatch

PE24021

Closed preliminary evaluation · opened August 7, 2024 · closed December 12, 2025

the seat belts

On August 7, 2024, the Office of Defects Investigation (ODI) opened PE24021 to investigate complaints alleging inadvertent unlatching of seat belts in model year (MY) 2020-2023 Hyundai Palisade vehicles manufactured by Hyundai Motor America. The complaints alleged that, without warning, the seat belts sometimes inadvertently unlatch, increasing the risk of injury due to full loss of seat belt protection in the event of a crash. As a result of information exchanged as part of the investigation, on April 3, 2025, Hyundai initiated a warranty extension policy for seat belt buckles on model year 2020-2025 Palisade vehicles in the U.S. and Canada.  Hyundai subsequently initiated a request for warranty parts returns to support further analysis by the buckle supplier, ZF Lifetec. The warranty extension (reference “Z06”) noted that certain 2020 – 2025 model year Palisade vehicles may experience abnormal seat belt buckle(s) operation.   Hyundai extended the warranty coverage for the seat belt buckle under this condition to 15 years/150,000 miles (whichever occurs first) from the date of original retail delivery or date of first use.  This action is valid for original and subsequent owners. On September 11, 2025, Hyundai filed recall 25V-607 for certain 2020-2025 Hyundai Palisade vehicles.  Hyundai explained in its recall filing that the subject vehicles are equipped with seat belt buckle assemblies in the front row and second row outer seating positions that may contain out-of-specification components manufactured by the supplier.  These components can cause interference within the latch channel, potentially increasing friction, particularly under cold ambient temperatures. Improperly produced seat belt buckles, combined with slow insertion of the seat belt tongue plate, may prevent the seat belt buckle from fully latching. The recall remedy is currently under development. In view of Hyundai’s recall action, ODI is closing this Preliminary Evaluation, but will continue to monitor the issue as a remedy is developed and implemented. ODI will take additional action if warranted by future circumstances. To review the reports cited in the Closing Resume ODI Report Identification Number document, go to NHTSA.gov.

Windshield Wiper Failure

Closed recall query · opened December 2, 2022 · closed May 26, 2023 · led to recall 23V180000

visibility, visibility and the wipers

On August 15, 2022, Hyundai Motor America (Hyundai) filed a safety recall (NHTSA Recall 22V-627) of 122,919 model year (MY) 2020-2021 Hyundai Palisade vehicles produced from April 10, 2019 to November 30, 2020. This recall addressed windshield wipers that may function intermittently or stop functioning when impeded by the accumulation of snow and/or ice at the bottom of the windshield. Intermittent or inoperative windshield wipers could reduce driver visibility while operating the vehicle in inclement weather, increasing the risk of a crash. On December 2, 2022, the Office of Defects Investigation (ODI) opened this Recall Query (RQ) after receiving 6 consumer complaints alleging windshield wiper failures while driving during wintry precipitation in MY 2021-2023 Hyundai Palisade vehicles (subject vehicles). These vehicles were excluded from Recall 22V-627. At the time of opening the RQ, the root cause and recall remedy were still unknown and under investigation. Thus, the RQ was opened to assess the scope and remedy of Recall 22V-627.In its February 10, 2023 response to ODI's information request on the RQ, Hyundai provided 17 customer complaints and 75 warranty claims related to the alleged defect in the subject vehicles. Hyundai stated that the wiper systems in the subject vehicles are equipped with a different wiper motor design than the earlier-built Palisade vehicles included in Recall 22V-627. Hyundai also stated it does not believe there is an unreasonable risk to motor vehicle safety in the wiper system in subject vehicles. On March 17, 2023, Hyundai filed a new safety recall (NHTSA Recall 23V-180) of 167,085 MY 2021-2023 Palisade vehicles produced from December 1, 2020 to November 22, 2022 when a design change was implemented. The design change, as well as the recall remedy (for both Recalls 22V-627 and 23V-180), is a redesigned driver-side wiper arm that allows for additional snow accumulation between the wiper arm and the cowl, thus reducing the load on the wiper motor and circuit breaker activation. In view of the recall action being taken by Hyundai, ODI is closing this RQ. The agency reserves the right to take additional action if warranted by future circumstances. The ODI complaints cited above can be viewed at NHTSA.gov under the following ODI identification numbers:11521365, 11517359, 11512295, 11509965, 11509253, 11508868, 11502146, 11501278, 11497343, 11494119, 11458548, 11450975, 11450045, 11449381, 11443330.

SERVICE BRAKES

While driving, the vehicle in front of me stopped suddenly. When I pressed on the brake pedal it felt stuck and did not respond properly. The forward collision automatic braking system did not activate. This resulted in a crash. After the incident, the vehicle had fluid leaks and severe front-end damage and is not drivable. I am concerned there was brake system failure prior to impact.

SERVICE BRAKES

5 injured · crash

While driving the vehicle ahead brake suddenly. When I applied the brake pedal, it felt abnormally stiff/stuck and did not provide Norma breaking response. Th vehicle did not decelerate as expected despite immediate pedal input. At the same time, the Forward Collision Avoidance Assist system did not activate or provide any braking assistance. Due to the lack of effective braking, I was unable to stop in time and a collision occurred. The primary concern is a potential failure or malfunction in the braking system occurring prior to impact combined with the apparent failure of the automatic emergency braking system to engage. This created a situation where the vehicle could not be stopped

While my vehicle was stopped on the left turn lane, another vehicle coming from opposite direction was veered into the left turn lane which resulted in striking driver side of my vehicle. The damage was severe enough that my vehicle was eventually totaled. Despite the such strong impact, no airbag was deployed including side airbags.

AIR BAGS

A car attempted to make a left hand turn across our lane of traffic. We didn't have any chance to break to avoid the collision and all front airbags were deployed. The front passenger airbag didn't seem to deploy properly and seems that it inflated upwards to the windshield and not toward the passenger. The windshield was shattered by the airbags sending glass into the passenger compartment.

ENGINE
I had an oil change in Feb 2026 at 64000 miles. In June, I was driving the car and suddenly the car started jolting while I was idle and would turn off completely when I was stopped. This happened to me while driving on the freeway in traffic and also at a traffic light. I immediately went to the dealership and was told my car had no oil in it. Since the oil change I drove about 5900 miles. I was told there were no leaks so the issue was either the engine or that when I had the oil changed that they forgot to put the oil in. They advised to get an oil test. I drove 1000 miles and took the car back in today 8/11. I had the oil tested and I burned .4 quarts in the 1000 miles which was under

SEATS
The 3rd row power seat does not detect obstacles. If the folding button is inadvertently hit, the 3rd row will fold and does not stop even if there is an obstacle. Items or people. We had a scare with one of our toddlers where it started to close and didn't stop. Thankfully we were quick enough to hit the open button before anyone was hurt. Multiple time's we've accidentally closed the 3rd row on different items, a few of which were rather large. I am concerned that someone might get hurt or worse if this isn't addressed.

Component/system, available for inspection? Rear visibility system — AVN head unit's processing of the rear camera image. On shift to reverse, screen flickers, shows blinking green, or no rear image. Vehicle available for inspection. Original camera was replaced Jan 2026 and retained by the dealer, not returned. Safety risk? Daily driver used for school pickup. Failure is warm-restart dependent — engine running, repeated shifts into reverse — which is exactly the pickup-line condition. Rear visibility is lost while children move behind the vehicle. System works reliably only after 25+ minutes shut off. Reproduced/confirmed by a dealer? Yes. Crain Hyundai of Bentonville, RO H3CS1330630,

A complaint is one owner's account and has not been verified by NHTSA or by us. A vehicle with more complaints is not necessarily worse: models that sold in larger numbers collect more of them. How this is counted.
