The defect
Chrysler (FCA US, LLC) is recalling certain 2020-2022 Pacifica Plug-In Hybrid Electric (PHEV) vehicles. The battery pack may cause a vehicle fire, even when the vehicle is parked with the ignition in the "Off" position.
The risk
A vehicle fire increases the risk of injury.
The remedy
Owners are advised not to charge their vehicles, and to park outside and away from structures, until their vehicle is repaired. Dealers will update the high voltage battery pack control module software. In addition, dealers will inspect and replace the battery pack assembly, as necessary. All repairs will be performed free of charge. Owner notification letters were mailed beginning June 23, 2026. Owners may contact FCA US LLC customer service at 1-800-853-1403. The defect
Chrysler (FCA US, LLC) is recalling certain 2022-2026 Pacifica and Voyager vehicles. The right and left side curtain air bags may have insufficient pressure retention from improperly sealed seams. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 226, "Ejection Mitigation."
The risk
Air bags with insufficient pressure retention increase the risk of ejection and injury during a crash.
The remedy
Dealers will replace the side curtain air bags, as necessary, free of charge. Owner notification letters were mailed May 14, 2026. Owners may contact Chrysler customer service at 800-853-1403. Chrysler's numbers for this recall are 06D, 10D, and 11D. Vehicle Identification Numbers (VINs) involved in this recall will be searchable on NHTSA.gov beginning April 2, 2026. Vantage Mobility, LLC (VM) is recalling certain 2021-2026 Honda Odyssey, 2021-2025 Chrysler Pacifica, and Toyota Sienna vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. The retractors may not lock, preventing the wheelchair from being properly secured.
The risk
An unsecured wheelchair can move during transit, increasing the risk of injury.

The defect
Chrysler (FCA US, LLC) is recalling certain 2020-2023 Pacifica and 2020-2021, 2023 Voyager vehicles. An improperly secured connector for the side curtain air bags may prevent the air bags from deploying. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard numbers 214, "Side Impact Protection" and 226, "Ejection Mitigation."
The risk
An air bag that does not deploy during a crash increases the risk of injury.
The remedy

BACK OVER PREVENTION: SENSING SYSTEM: CAMERA

The defect
Chrysler (FCA US, LLC) is recalling certain 2021-2022 Dodge Durango, 2021-2023 Chrysler Pacifica, Jeep Grand Cherokee L, 2022 Ram 1500, 2500, 3500, Chrysler Voyager, 2022-2023 Jeep Compass, Grand Cherokee, Wagoneer, Grand Wagoneer, and Ram Promaster vehicles. The radio software may prevent the rearview image from displaying. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rear Visibility."
The risk
A rearview camera image that does not display decreases the driver's visibility, increasing the risk of a crash.
The remedy

The defect
Chrysler (FCA US, LLC) is recalling certain 2017-2023 Pacifica Plug-In Hybrid Electric vehicles (PHEVs). An internal transmission wiring connector may short circuit, resulting in an unexpected engine shutdown.
The risk
An unexpected engine shutdown can cause a loss of drive power, increasing the risk of a crash.
The remedy
Dealers will update the Power Inverter Module (PIM) software and, if necessary, update the instrument panel cluster (IPC) and the electronic control module (ECM) software, free of charge. FCA US will notify dealers and begin notifying owners on or about: 03/21/2023 for 2021 model year vehicles, 05/02/2023 for 2019-2020 model year vehicles, and 06/15/2023 for 2017-2018 model year vehicles. The defect
Chrysler (FCA US, LLC) is recalling certain 2022 Voyager and Pacifica vehicles. The windshield wiper arm nuts may be improperly tightened, which can cause the wipers to not function properly.
The risk
Improperly functioning wipers can reduce the driver's visibility in certain weather conditions, increasing the risk of a crash.

The defect
Chrysler (FCA US, LLC) is recalling certain 2021-2022 Pacifica and 2021 Voyager vehicles. The second-row seat-to-floor mounting latch may bind and stick open, preventing the seat from properly securing to the floor. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 225, "Child Restraint Anchorage Systems."
The risk
An unsecured seat increases the risk of injury in a crash.
The remedy

On July 22, 2022, the Office of Defects Investigation (ODI) opened investigation PE22-008 regarding complaints alleging a loss of motive power condition potentially due to a transmission malfunction in Model Years 2019-2021 Chrysler Pacifica Plug-In Hybrid Electric Vehicles (PHEV).  On January 17, 2023, Fiat Chrysler Automobiles (FCA) issued safety recall (NHTSA Recall 23V-010) involving 2017-2023 Model Years (MY) Pacifica Plug-In Hybrid Electric Vehicles (PHEV) produced between August 12, 2016, through January 9, 2023. The recall states that the loss of motive power condition in these vehicles is caused by an internal transmission wiring connector that may short circuit, resulting in an unexpected engine shutdown.  An unexpected engine shutdown can cause a loss of drive power, increasing the risk of a crash.  In discussions with FCA on Recall 23V-010, ODI confirmed the recall was related to investigation PE22-008.  The remedy for 23V-010 is to update the Power Inverter module (PIM) and/or Instrument Panel Cluster (IPC) software to provide drivers a message stating “Stop Safely - Vehicle Will Shut Off Soon” followed by a chime and MIL, PRNDL blinking (on cluster and shifter knob), low SOC (State of Charge) warning and reduced power mode.  The recall remedy does not replace the defective hardware that was described in Recall 23V-010, and the wiring connector short still exists on these vehicles. To date, ODI has received 132 reports for the population of vehicles subject to recall 23V-010.  These reports allege a loss of motive power condition that reduces the vehicles speed, followed by dashboard warning lamps displaying messages associated with recall 23V-010.  The presence of these warning lamps and messages indicates the vehicle likely had the recall remedy applied. Follow up discussions with FCA confirm their remedy does not prevent a loss of motive power, but instead limits torque to the engine and after an unspecified number of miles or period of time, the engine will shut down.  When this occurs, FCA relies on the owner to transport the vehicle to a dealership after they experience a loss of motive power condition, at which time the defective hardware is replaced.  Furthermore, the dashboard messages introduced in the recall remedy may cause the driver to pull over in an unsafe location. Pulling over in an unsafe location may expose the occupants to undue harm. The Office of Defects Investigation is opening this Recall Query (RQ) to assess the adequacy, frequency, and safety consequence of the remedy for recall 23V-010.  To review the ODI reports cited in the opening resume ODI report identification number document, go to NHTSA.gov.

On July 22, 2022, the Office of Defects Investigation (ODI) opened PE22008 to investigate allegation of loss of motive power, potentially due to transmission malfunction, in certain Chrysler Pacifica vehicles. In a January 17, 2023 letter to NHTSA, Fiat Chrysler Automobiles (FCA) submitted recall notice 23V-010 involving a transmission motor wiring defect in 67,118 model year (MY) 2017-2023 Chrysler Pacifica plug-in hybrid electric vehicles (PHEVs) manufactured from August 12, 2016 through January 9, 2023. FCA determined that an internal transmission connector short could result in an unexpected engine shutdown under certain conditions. Follow up correspondence with FCA confirmed this defect results in an electric motor position trouble code that was the basis for opening PE22008, therefore NHTSA is closing this investigation. The 23V-010 recall remedy is a software update to address the hardware failure associated with PE22008. When a hardware failure is detected, the vehicle limits motive power while instructing the driver to pull over to a safe location. NHTSA is concerned that drivers will pull over to an unsafe location when the warning message associated with the recall remedy displays on the dashboard. Due to the nature of the recall remedy and related reports, NHTSA has opened Recall Query RQ24004 to assess the recall remedy. The reports and failure report summary values included with this closing action are related to allegations of Recall 23V-010 remedy inadequacy. The Agency reserves the right to take additional action if warranted by future circumstances. 09/17/23 Was driving 65mph, car was slowing Down in front of me and turned cruise control off. Immediately car started stalling. Was in middle lane with cars on both sides. Turned on flashers and no power in gas pedal. At about 30mph, was struck from behind by Jeep! Third car behind struck Jeep and Jeep struck me again. There were injuries and a death. My car was towed. 10/02/23; Now car is in body shop. Contacted Chrysler, they inspected car, but did not drive it. 10/12/23 Just received email stating that they found no problem. Traveling down a country road on a clear afternoon, the vehicle traveled approximately 35 mph while cresting a small hill. On the other side of the hill was a "T" shaped intersection where another vehicle stopped in the intersection from a stop sign several feet behind. The other driver failed to complete the turn when they saw oncoming traffic. The driver noticed the intersection had an obstructed path and attempted to move into the oncoming traffic lane since no oncoming traffic was visible. Just then, the onboard computer installed in the control panel of the drivers seat had acted upon either the obstruction or the switch in lanes and assumed complete control of the braking and steering

The contact owned a 2022 Chrysler Pacifica. The contact stated that while driving approximately 55 MPH, a very loud snapping noise had suddenly occurred, and the message "Stability System Activated" displayed before the steering wheel suddenly turned to the right, causing the driver to lose control of the vehicle to crash and roll over multiple times. The police and first responders were called to the scene, and the occupants were transported to the hospital. During the crash, the driver sustained multiple neck and back fractures and a right leg injury from the seat belt strap. The front passenger sustained multiple rib fractures. SEAT BELTS

I was in an accident. Where the van rolled 17 times. On or about the 12th roll. The retaining lock on the shoulder strap released. This allowed me to slide out of the seatbelt. This caused me more complicated injuries. The vehicle is currently at copart impound lot in Oklahoma City, OK. Will be available for inspection upon your request. The Oklahoma highway patrol report indicated. I was wearing my seatbelt but the retaining lock on shoulder strap had broken. The Oklahoma highway patrol report # OHP25-024333. There were no warning lights or messages to indicate that there was a problem.

SEATS
The head rest of the second row broke and will not return to its upright position. It happened when we putting the seat in the stow position. Having the headrest sticking into your neck is very dangerous so we basically lost a seat in our vehicle. We can choose to remove the headrest, which is a danger for whiplash OR repair it. The repair is estimated over $300 which is ridiculous! This should be a covered repair under a recall as it seems to happen frequently.

ELECTRICAL SYSTEM
My vehicle is currently being recalled for the recall 52D, I have reached out twice in a thirty day span asking about a fix and been told both times there are no parts for this fix. The risk of this recall includes fire, while charging or while parked and off.

ELECTRICAL SYSTEM
My car is a part of NHTSA Campaign No. 26V362 / Manufacturer Recall 52D and on July 30th, 2026 it suffered a fatal transmission error and stopped on the highway. I am unable to drive it and it officially needs its entire transmission rebuilt. My wife was driving back from work and the car shutdown in the middle of the highway and she had to risk her life to get it to an emergency zone. The turtle symbol came on the dash and the dealer confirmed the transmission needs to be rebuilt because of a manufacturer defect.

SEATS
The captains chairs in the second row of the vehicle both had their reclining mechanism fail. This causes the seats to move forward and back sliding when the car is in motion as they will not stay locked in place. They cannot be used for transportation due to it being unsafe for the passenger as the seat does not remain upright. The seat is available for inspection. The seats have not been inspected by the dealer/manufacturer as they are wanting to charge a large service fee and do not have appointments available in a timely fashion. There were no warnings, the seats each failed suddenly upon regular use.

ENGINE
On July 19, 2026, I was driving my 2022 Chrysler Pacifica Hybrid on E-470 at approximately 75 mph. Without warning, the dashboard displayed "Stop Safely. Vehicle Will Shut Off Soon," and the amber check engine light illuminated. At the same time, the vehicle immediately lost all motor power, began slowing, and did not respond to accelerator input. There were six occupants in the vehicle, including four young children. I was traveling on a busy six-lane highway with little to no shoulder. I had to maneuver across traffic without motor power to reach the side of the road, placing all six occupants at immediate risk. After turning the vehicle off for about five minutes, I restarted it.
