The defect
Rollx Vans (Rollx) is recalling certain GMC Yukon, Sierra, Chevrolet Express 2500, Ford Transit, E-250, Toyota Sienna, Sienna Hybrid, Honda Odyssey, Chrysler Town and Country, Voyager, Pacifica, Dodge Grand Caravan, and Ram Promaster vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. Please refer to Rollx's recall report for specific model year information. The retractors may not lock, preventing the wheelchair from being properly secured.
The risk
An unsecured wheelchair can move during transit, increasing the risk of injury.
The remedy
Rollx will work with Q'Straint to inspect and replace the retractors as necessary, free of charge. Owner notification letters were mailed February 5, 2026. Owners may contact Rollx's customer service at 1-800-956-6668.

BACK OVER PREVENTION:DISPLAY FUNCTION

The defect
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
The risk
A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.
The remedy
Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.

The defect
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2025 Sienna Hybrid vehicles. The second-row seat rails may be welded improperly.
The risk
Improperly welded seat rails can prevent the seat from adequately restraining an occupant during a crash, increasing the risk of injury.
The remedy
Dealers will replace the second-row seat rails, free of charge. Owner letters were mailed May 15, 2026. This is a phased recall. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB12 and 25TA12.

The defect
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2021-2025 Sienna Hybrid vehicles. The bolts for the seatbacks on the third-row seat assemblies may not be tightened properly. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) numbers 202, "Head Restraints," 207, "Seating Systems," and 225, "Child Restraint Anchorage Systems."
The risk
An improperly tightened seatback may not adequately restrain the occupant during a crash, increasing their risk of injury.
The remedy
Dealers will tighten the seat assembly bolts, free of charge. Owner notification letters were mailed April 1, 2025. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's number for this recall is 25TA05.

I am filing a formal complaint regarding my brand‑new 2025 Toyota Sienna, purchased in February 2025 for nearly $70,000. I am still paying interest on this vehicle, which has failed to perform as promised. From early ownership, the vehicle had issues including multimedia malfunctions and unsafe lane‑change behavior on rough roads. In October 2025, Toyota issued a recall for faulty second‑row seats due to improper welding, creating serious safety concerns. This recall has effectively defeated the purpose of purchasing the vehicle. The Sienna was bought as a seven‑seater, but due to the recall it functions as a four‑seater, making it impractical and unsafe for my family. Daily use now causes:

The Supplemental Restraint System (SRS), specifically the airbag system, is the component of concern. During a two-impact collision involving my 2025 Toyota Sienna XLE, none of the airbags deployed despite significant front-end damage that resulted in the vehicle being declared a total loss. The vehicle is currently in the possession of the insurance company or its storage facility. I do not know whether it has been released for salvage, but it may still be available for inspection. We purchased a used 2025 Toyota Sienna a few weeks ago. The other day, my husband put his phone and wallet into the center console compartment and closed the lid. When we went to retrieve those items, the lid only opened partially. We were finally able to remove the wallet but the phone had slid down into the compartment where the door to that console opening slides into when you open and close it. The lid would not open all of the way and blocked our access to that space when you opened it. The only way to remove the phone was to take it to our local dealership where they spent hours removing seats and pieces of the console to remove the phone from the inaccessible area and then put
